Book appointment with Reception Parents automatically contacted by Child Health Services If you believe your child is overdue a vaccination, contact Reception. a) To secure a GP/Nurse referral, make an appointment with Reception to see GP/Nurse or b) (n.b. assessment for eligibility will be made) Ask Reception for Clinic details. Usually, for weighing and general questions no appointment is needed. Explain your requirements to Reception who will advise on process Book a Child Vaccination appointment (these are done during Baby Clinic on 1 st and 3 rd Tuesday afternoons). a) Book an appointment with Chiropodist, telephone b)complete the Chiropodist Form (available from Reception), if eligible, telephone for appointment Contact Reception for GP appointment specifying whether coil or implant required (for implant a prescription must be ordered prior to appointment). For these, the patient will be contacted automatically:- Aneurysm Screening Men aged 65 Bowel Cancer Screening Men and Women aged 60-69, every 2 years* Breast screening Women aged (first one before age 53) Cervical Cancer Screening (Smear Test) Chlamydia Retinal Screening All diabetics 12yrs or over * More information can be found on ` Women every 32 years and years every 5 years* Recommended for sexually active patients under 25 (this does not need to involve an examination and can be checked with a test you do yourself)
