2 Specialty Contact Lenses Anterior Segment Disease Dry Eye/MGD Enriching Lives Our Mission is to Enrich patient lives so that others may succeed to their greatest potential. SEG s Residency Program in Cornea and Contact Lenses mission is to provide optometric residents an opportunity to become leaders in their specialty while helping to: Enrich patient lives so that they can succeed to their greatest potential. Residents will obtain advanced competencies in the optometric management of anterior segment diseases and specialty contact lenses through one-on -one mentorship with specialists on staff. Specialty Eyecare Group Introduction: SEG was formed in 2007 when Dr. David Kading and Dr. Kristi Kading purchased a practice called Totem Lake Vision Center. At that time the practice focused on primary care optometry. Since 2007, Specialty Eyecare Group has grown into a two-location practice comprised of six specialties. 3 David Kading, OD, FAAO, FCLSA Dr. Kading is an internationally known anterior segment, contact lens and dry eye specialist. He has done hundreds of lectures and articles on the topics and is well known in the optometric community through his regular columns, written and video, and lectures at major optometric meetings. Following his residency, he began building his specialties in the Seattle area with the goal of enriching peoples lives so that they can succeed to their greatest potential. He created the residency program in order to train others to expand their focus and better serve patients. Teaching Specialty Eyecare Group welcomes interns from various optometric schools. We feel that one of the best ways to learn is to teach. The concept of life long teaching and learning is a core belief of Specialty Eyecare Group. As such, teaching is an emphasis during the residency program.

4 Objectives & Outcomes Provide a comprehensive residency program focused on the contact lenses, dry eye and anterior segment. Provide real-world-referral-based private practice experience. Provide anterior segment disease experience that is commonly seen in private practice. Provide specialty cornea disease and contact lens experience. Provide a full dry eye specialty experience that focuses on innovative diagnostic measures and treatment options. Help the resident gain an appreciation for service to the optometric profession. (Should not be politically motivated: service to furthering the profession and service to community?) Provide resident with real world & financial implications of their work in practice. The resident will have confidence in managing anterior segment disease or making an appropriate referral. The resident will have confidence in managing corneal disease and fitting contact lenses that may relate to the management of the disease state. The resident will be a leader in the profession with regards to dry eye knowledge and treatment methods. The resident will have educational knowledge and clinical skills which will escalate them in the profession to be capable of treating patients within their specialty with confidence, or allow them to enter into a teaching institution where they can instruct others in their specialty discipline. The resident will understand the financial implications that their services provide to a practice.

5 MEASURE AND REVIEW The resident will report numbers regarding patient encounters. The resident will meet regularly to report on their learning progress with a disease states and contact lenses. The resident will write articles related to their specialities The resident will publish work on their clinical research including research posters and white papers The resident will perform at minimum 2 lectures related to their specialties The Resident: The Resident will evaluate the residency on a semi-annual basis to determine the overall strengths and weakness of the residency program. They will also determine the areas that the resident needs more patients or residency training. Specialty Eyecare Group: The Resident will be evaluated on a semi-annual basis and through regular meetings to determine areas where the resident needs additional training. Extra Office Activities While in the Seattle area, the resident will be required to take part in the local society and local doctor meetings. Additionally, the resident will be asked to take part in several optometric conferences throughout the year in order to form relationships, network, share research and gain educational knowledge.

6 JULY 1st Through July 31st Residency begins July 1st and ends July 31st. Most weekly hours will be approximately 40 hours of clinical care and may include as much as 60 hours. This is dependent on the clinic schedule of patients. The clinics are available for patient appointments 9am to 5pm. Residents are expected to arrive at minimum 15 minutes before the office opens for a brief all-team schedule review and remain in the office until their last patient leaves. Most days there will be a minute break for lunch. There are no weekend hours regularly scheduled for patient care time. RECORD The resident must maintain a record of patient encounters. Resident is expected to participate in a minimum of 1000 patient encounters. This will include direct care, joint care, and observation. The break down of the encounters will vary depending on the resident s interest level. As a template it may follow: 40% Specialty Contact Lens and Corneal Disease 40% Dry Eye/MGD/Anterior Segment Disease 20% Primary Care SUPERVISION Supervision of the resident will be weighed heavily initially in their residency program. In addition to working to teach the resident in the specialty, this is to ensure that the resident is familiar with specialty equipment, electronic health records, and clinic methods. Initially the resident will do a fair amount of observation and joint patient encounters, by the end of the first month this will signifcantly decrease. Throughout the first six months, significant amount of joint work will be performed in clinic and in research. After the first half of the residency, a customized second half will be created in order to maximize learning in the area of the resident liking.

7 The resident shall receive compensation of $37,000 per year regardless of productivity. The resident shall receive 10 days of leave (includes vacation, sick leave, personal business leave, bereavement leave, etc.) Additionally, the resident has all holidays off that are taken by Specialty Eyecare Group. Professional leave is at the discretion of the residency supervisor, but certain meetings will be required such as American Academy of Optometry and Global Specialty Lens Symposium For certain professional functions, Specialty Eyecare Group may provide travel stipends. Residents will be encouraged to look into residency travel grants. Specialty Eyecare Group will pay for expenses that allow resident to practice in Washington State including fees for licensure. Specialty Eyecare Group will pay for professional liability and health insurance coverage.

8 RESIDENCY APPLICATION We welcome you to apply This program uses Optometry Residency Match (ORMatch). Applicants must complete the ORMatch application by the PUCO deadline of January 31, to be considered. Applicants are responsible for ensuring that all supporting documentation is submitted to each entity involved, e.g., the College, ORMatch, and the residency site supervisor. For our application, we request the following sent to : residency@specialtyeye.com Letter of interest explaining goals and interests in optometry Updated CV Include Education: (institution, dates, major, degree or pending degree) Brief Essay stating the reasons for applying to this program. Candidates must have passed the NBEO Part I and have taken Part II by the application deadline. The selected candidate must have passed Part II and be scheduled to sit for Part III. Professional Transcripts 3 Letters of Recommendation (include 1 from director level of contact lens or anterior segment section) Interview Specialty Eyecare Group will review and evaluate all applications. Applicants that are competitive for the residency will be asked to come to Seattle for an interview. Interested applications are encouraged to apply early to maximize their flexibility with scheduling should they receive an invitation to interview.
