A Core trainee in Psychiatry spends 6 months at the CDC, Poole and 6 months in the Shelley Clinic, providing an opportunity for the ST4-6 trainee to provide supervision. There are close links with the paediatric department (hospital and community) and opportunities to further develop paediatric liaison and gain experience with patients with learning disability and other developmental difficulties. The clinic provides regular consultation and support to paediatrics, social services and education, as well as services in primary care, e.g. school nurses, Relate, Connexions. Experience can be offered in a wide range of treatment approaches, including family therapy, psychodynamic individual therapy, brief individual therapy, cognitive therapy, medication, liaison and networking. There are opportunities for partaking in the various CAMHS management groups, e.g. clinical governance, and a multiagency operational group. The ST3-6 trainee is expected to attend the monthly interteam clinical meeting and monthly consultants peer group. There is access to journals/books at the Postgraduate Library, Poole Hospital and the library in the psychology department at St Ann s Hospital. Maple Young Peoples Intensive Support Service and Pine Cottage, Poole Pine Cottage Adolescent Unit/ Pepper Lodge is an 8 bed adolescent inpatient facility (shortly to become an 8 bedded facility with Tier 4 Home Treatment experience in addition to inpatient experience) catering for young people between the ages of years across the whole county of Dorset (Dorset population approximately

The opportunity for examination coaching of CT1-3 trainees planning to undertake MRCPsych examinations is available. The Shelley Clinic, Boscombe The Shelley Clinic provides a general tier 3 community outpatient CAMH Service. Portsmouth and South East Hampshire In this placement the senior trainee is employed by Sussex Partnership NHS Foundation Trust or Solent NHS Trust (East). There are 3 community Child & Family Therapy Services in the Portsmouth and East Hampshire area which all provide a similar service and the senior trainee is based at one of these clinics. Although the 3 clinics are managed by different organisations, there are close clinical and managerial links. The clinics receive referrals of moderate to severe child and adolescent mental health problems (i.e. tier 3) but each area works closely with their own team of Primary Mental Health Workers who see children in the mild to moderate range of difficulties (i.e. tier 2). The Trusts have contracts with Leigh House Inpatient Adolescent Unit in Winchester (tier 4). Small scale district paediatric liaison experience is possible.

8 Each Community Child & Family Therapy Service sees patients up to 18 years of age. Each clinic receives approximately 500 referrals per year from a wide range of professionals. Almost by definition this is a forensic population and one that all agencies find very hard to engage and manage. The necessary processes of assessment and management are particular and comprehensive. Networking tasks are central to success. The trainee is involved in the multidisciplinary working of the complex caseload held by this service. Such work will involve assessment, therapy and management of the many facets of the cases. There are opportunities for providing consultation and clinical supervision of work undertaken by other agencies, as well as training of other agencies alongside other team members. The trainee is expected to be involved in managerial and internal training aspects of the team. Solent NHS Trust Senior trainees on placement in Southampton, and at Leigh House Adolescent Unit are employed by Solent NHS Trust (West). They are based in Eastleigh, Portsmouth, Dorset and Ashurst. The experience includes: spending time in a special school in a classroom for children with severe learning difficulties; seeing cases for assessment jointly with members of the learning difficulties team; attending clinics in special schools; liaising with others teams e.g. social services Children with Disabilities team, Safeguarding Children (Child Protection) teams and with the specialist Connexions personal advisor; and attending community paediatric clinics.

Most of the young people in the hospital have eating disorders, psychoses, major affective disorders or severe OCD and anxiety disorders. The average length of stay is four months. There is a well staffed school attached to the hospital and an impressive occupational therapy department. Young people and their families have the opportunity of a range of therapies including family therapy, cognitive behaviour therapy, cognitive remedial therapy, DBT and Mind Skills groups, psychodynamic group work and art therapy. There is a good working relationship between the different members of the multidisciplinary team regarding the management of cases and the functioning of the unit. The senior trainee has plenty of opportunity to take part in multidisciplinary assessments of complex cases and review meetings in addition to opportunities for training (supervision in cognitive behavioural therapy, family therapy and pharmacological interventions). There is weekly supervision, both individual and group, from the two educational supervisors. The Paediatric Unit, Southampton General Hospital Placement in this post is usually divided between 6 months based at Bursledon House Children s in-patient unit and 6 months based on the paediatric wards in Southampton General Hospital working with the child mental health team (Paediatric Liaison). 12 has input from paediatricians, psychiatrists, psychologists, occupational therapists, psychotherapists, other therapists and a small school which has become a pupil referral unit. Patients with a range of mental health, paediatric or mixed paediatric/mental health problems are referred, including those with psychosomatic disorders, chronic fatigue syndrome and neurological problems. Patients are referred from a wide range of geographical areas from psychiatrists and from acute and community paediatricians. Admissions range from short term assessments to longer term assessment and management, including rehabilitation e.g. following severe head injury. There is very close working between paediatricians and mental health professionals. Southampton General Hospital has a wide range of secondary and tertiary paediatrics, including respiratory, neurology, oncology and endocrine services and a paediatric intensive care unit. The paediatric liaison team provides input to all paediatric services in the hospital.
