1 ADVISEMENT INFORMATION OSU INSTITUTE OF TECHNOLOGY REGISTERED NURSING PROGRAM Associate of Applied Science in Nursing LPN-to-RN TRANSITION STUDENTS APPLICATION PERIODS: SUMMER ENTRY: March 1 st May 15 th :00 p.m. Revised May 2017 ADVISEMENT All students applying to the nursing program must come to the nursing department for advisement. Students may schedule an appointment to speak to a nursing advisor by calling (918) 29-57, or come to the office between 8:00 a.m. :0 p.m. while school is in session; advisors are not available from 11:0 a.m. 12:0 p.m. unless an appointment has been made. OSUIT Nursing Program is approved by the Oklahoma Board of Nursing and is accredited by the Accreditation Commission for Education in Nursing (ACEN). This program requires extensive use of technology, including required course texts delivered as e-texts, paperless clinical documentation and regular interface with the online classroom. For this reason, students accepted to the program must have computer and internet access at home and must supply their own tablet, size must be 8 x 11 or smaller. Only LPNs with an unencumbered Oklahoma nursing license, as verified via the OBN website, will be considered for acceptance. LPN s must maintain an unencumbered nursing license throughout the duration of the nursing program. If at any time during the nursing program the LPN is subject to action or discipline from the Oklahoma Board of Nursing they must report this to their nursing instructor and/or Dean immediately.
2 APPLICATION TO THE PROGRAM Students must submit an application to the nursing program separately from the college application. The nursing application must be submitted within the time frame indicated above. A new application must be received for each application period. Even If the student has applied to our program before they MUST submit a NEW application to the nursing program. All applications must be signed and dated. ADMISSION TO THE PROGRAM Students electing to be considered for the program are required to apply to OSUIT college this is a different and separate application than the one for the nursing program. Students must have the following data in their file in the nursing department by the final closing deadline dates listed above: 1. Apply and be accepted to OSUIT College; 2. Nursing Application signed and dated;. Entrance Exam: Students will take a Reading, Math, and Science exam Students must pass the Reading and Math at 55% or higher to be included in the selection process. Students must pass the Science at 5% or higher to be included in the selection process. The Exam is administered in the nursing Department in the computer lab, free of charge. Students are required to take the exam each application period. Exam can be taken only one time per application period. The exam is only valid for that application period. Students will be provided a calculator, scratch piece of paper. Students must provide a photo identification to take the exam.. All official college transcripts from previously attended colleges should be on file in the nursing department with the original in the OSU Institute of Technology Registrar s office. (It is the student s responsibility to make sure that all of their college transcripts have been submitted to the college and the nursing department has access to them.) 5. Student score of 55 th percentile ranking or higher on the Kaplan Fundamentals Challenge Examination, administered by appointment only in the nursing department. Same rules apply as stated above for the Kaplan Reading, Math, and Science exam.
3 Students applying to the OSU Institute of Technology Nursing Program are selected on the basis of a point system and seat availability. NO STUDENT IS GUARANTEED ADMISSION TO THE NURSING PROGRAM. The Kaplan Entrance examination and Fundamentals Challenge examination will be given by appointment only in the nursing department computer lab. The student is required to schedule, in person, to test through a nursing advisor dates will be provided for the exam. The exam will be administered one time per week throughout the application period. If a student calls to cancel their scheduled exam they are required to reschedule by coming into the nursing department and selecting an alternate date. NO APPOINTMENTS FOR TESTING WILL BE GIVEN OVER THE PHONE. It is recommended the student arrive 0 minutes early for registration. Please note the date/time of test when scheduled. No one is allowed in the testing room after the scheduled exam time begins. Students arriving late will not be allowed to test. When the exam begins the student cannot have anything on the desk; all cell phones and books will be put away, and a calculator and scratch paper will be provided. To be considered for the nursing program the student must score 55% or above on the Kaplan Entrance Examination Reading and Math portion, 5% on the Kaplan Entrance Examination Science portion and 55 th percentile ranking or above on the Fundamentals Challenge Examination. The score for the Fundamentals challenge examination is scored as a percentile ranking, this is not a percent. You will receive two scores on the Fundamentals Challenge examination and the score that is recorded is the percentile ranking. To be considered for admission to the program, students will have to maintain a minimum grade point average of 2.0 in required general education and support courses. Successful completion of the nursing program requires a C in all courses related to the Associate in Applied Science in Nursing Degree. A student will not be considered in the selection process with a D in any required courses on transcripts for current application period. 4 ARTICULATION FOR LPN S Oklahoma State University Institute of Technology, a public two-year college accredited by the Higher Learning Commission of the North Central Association of Colleges and Schools, provides the opportunity for students that have completed a Practical Nursing Program and have an unencumbered nursing license in the state of Oklahoma to enter the OSUIT LPN-to-RN Program if they are selected. The LPN student will apply to the program by submitting an application and going through the selection and admission criteria process. The student shall have successfully completed a practical nursing program and have an unencumbered license as a Licensed Practical Nurse in the state of Oklahoma to be eligible to apply to the nursing program. Once the student has passed the examinations listed above they will be placed in the selection process for point total. Students are admitted to LPN-to-RN Transition in the summer based on total number points and space availability. Once accepted into the LPN-to-RN Transition course NURS 11, the LPN student will be given Kaplan Standardized examination Integrated 151 which equates to NURS 1229 Nursing Care of Families. The student is expected to pass this exam at the 55 th percentile ranking. If the student is not successful on this examination they must show written remediation and will be tested again. The student is allowed to retest on this examination only after proof of written remediation. Once the student passes NURS 11 LPN to RN Transition they will progress to NURS 2129 Nursing Care of Adults I in the fall. Once the student has completed LPN-to-RN Transition with a C or better they will be given advanced standing credit for NURS 1128, NURS 122, and six credits of NURS The student will then enroll in NURS 2129 Nursing Care of Adults I, this course will be followed by NURS 2229 and NURS Students are charged $5.00 per credit hour for every advanced standing credit hour they receive. They will be charged $80.00 for the advanced standing credit they will be receiving. LPN s that meet the selection process requirements and have completed an LPN course at a State approved Career Technology Center LPN Program are eligible to take an anatomy and/or nutrition challenge exam. Student must score a 70% or higher to receive credit. Students are eligible to take an Anatomy and/or Nutrition advanced standing exam challenge exam that has been created by the School of Nursing and Health Sciences. This is not a nationally normed standardized exam. This will be applied to the OSUIT transcript once student has completed the OSUIT Nursing Program successfully. This will not be awarded if student does not complete OSUIT s Nursing Program or transferrable to another institution before completing OSUIT s Nursing Program. Student will be charged $5.00 per credit hour to receive advanced standing for Anatomy and Nutrition.
5 ENTRY LEVEL ASSESSMENT AND PLACEMENT FOR COLLEGE COURSE WORK ONLY The preliminary screening instrument for recent high school graduates and non-graduates aged 20 years and younger will be the Enhanced ACT, with established cut-scores in the sub-tests of English (19), math (19), reading (19), and science reasoning (19). Adult students 21 years of age and older without ACT scores and students 20 years and younger who scored below the established ACT cut-scores will be required to undergo additional evaluation using the COMPASS Test to determine readiness for college-level work. Those students obtaining sub-test scores in the college-level range will have demonstrated competency and may pursue college-level course work, while those students with cut-scores below the college-level range will be required to improve skills in one or more of the following: reading, English, and science. Students will need to score a 21 or higher on the ACT Science subscore to enter Anatomy, Physiology, or Microbiology. If score is 19 to 20 may go into Biology 111, if score is 18 or below the student will be enrolled in Physical Science. If the student tests using the COMPASS Test (College placement test offered at OSUIT) the student will need to score above: Reading 81; English Writing 7; Math-Arithmetic 6; College Algebra Algebra 68 or CALG 5; Science = Reading + College Algebra = 126 or Reading + Algebra = 19 to be placed in College level Courses. The student must score the cut scores in all areas for science proficiency. If the student scores below the above scores the student will be placed in Remediation courses. PHYSICAL AND MENTAL QUALIFICATIONS The following minimal physical and mental qualifications are required to be considered for admission into and progression through the OSU Institute of Technology Nursing Program but are not limited to: 1. The ability to lift weights of up to 5% of recommended body weight independently. 2. The ability to move throughout obstacles typically found in patient s rooms and in work areas.. Visual acuity sufficient to observe and assess client behavior, prepare and administer medications, and accurately read monitors and utilize equipment.. Auditory acuity sufficient to hear instructions, requests, monitoring alarms, and to auscultate heart tones, breathe sounds, and bowel sounds. 5. The motor ability necessary to manipulate equipment and supplies to utilize palpation and percussion in client assessment. 6. The ability to proficiently speak, write, and comprehend the English language proficiently. 7. The ability to communicate in a professional manner, establish rapport with clients and colleagues, use problem solving skills, and function effectively under stress.
6 CLINICAL RECORDS 1. Two MMR vaccinations (Measles, Mumps, Rubella) or a positive titer showing immunity to all three 2. Hepatitis B series or titer showing immunity. Two varicella vaccinations or a titer showing immunity. TdAP 5. Tuberculosis (ppd) testing showing negative status yearly 6. Influenza vaccination yearly 7. AHA BLS CPR certification current 8. Negative Federal Criminal Background check (sex-offender is included) 9. Negative Drug Screen performed by a company chosen by OSUIT Nursing Program information will be provided to the student 10. Physical Assessment form (obtained by OSUIT Nursing Program) completed by a provider as stated on form showing that the student can meet all physical and mental requirements as listed above FINANCIAL AID Students may obtain financial aid packets, verify filing deadlines, and get assistance filing forms from the Student Financial Services office, STUDENTS ARE RESPONSIBLE FOR FOLLOWING FINANCIAL AID GUIDELINES. This is the officially recognized organization for nursing students. Membership in the OSUSNA is encouraged as it provides the student with opportunities for professional development; activities related to leadership, education and service are emphasized.
7 PLAN FOR SUCCESS Because of the vital nature of nursing, this is a demanding program of study. Success depends primarily on adequate preparation and planning. The following suggestions are offered to help assure success: Complete as many of the support courses as possible before beginning the nursing courses. Anatomy, Physiology, and Microbiology are particularly important to provide a knowledge base for understanding nursing concepts. Plan three to four hours study time each week for each credit hour in which you are enrolled. Example: Students enrolled in 12 credit hours should plan 6 hours of study time each week, in addition to class and lab time. ELIGIBILITY TO SIT FOR BOARDS OKLAHOMA BOARD OF NURSING 2915 North Classen Boulevard, Suite 52 Oklahoma City, Oklahoma The Oklahoma State University Institute of Technology - Okmulgee is approved by the Oklahoma Board of Nursing. Graduates of this stateapproved program are eligible to apply to write the National Council Licensure Examination (NCLEX) for registered nurses. Applicants for Oklahoma licensure must meet all state and federal requirements to hold an Oklahoma license to practice nursing. In addition to completing a state-approved nursing education program that meets educational requirements and successfully passing the licensure examination, requirements include submission of an application for licensure, a criminal history records search, and evidence of citizenship or qualified alien status (G.E.D.) [59 O.S & 567.6]. To be granted a license, an applicant must have the legal right to be in the United States (United States Code Chapter 8, Section 1621). In addition, Oklahoma law only allows a license to be issued to U.S. citizens, U.S. nationals, and legal permanent resident aliens. Other qualified aliens may be issued a temporary license that is valid until the expiration of their visa status, or if there is no expiration date, for one year. Applicants who are qualified aliens must present to the Board office, in person, valid documentary evidence of: 1. A valid, unexpired immigrant or nonimmigrant visa status for admission into the United States; 2. A pending or approved application for asylum in the United States;. Admission into the United States in refugee status;. A pending or approved application for temporary protected status in the United States; 5. Approved deferred action
8 status; or 6. A pending application for adjustment of status to legal permanent resident status or conditional resident status. The Board has the right to deny a license, recognition, or certificate to an individual with a history of criminal background, disciplinary action on any professional or occupational license or certification, or judicial declaration of mental incompetence [59 O.S ]. These cases are considered on an individual basis at the time application for licensure is made, with the exception of felony convictions. An individual with a felony conviction cannot apply for licensure for at least five years after completion of all sentencing terms, including probation and suspended sentences, unless a presidential or gubernatorial pardon is received [59 O.S & 567.6].
