2 Course Objectives The goal of this program is to educate and inform our community partners who care for paediatric heart transplant patients. The course will provide information on many aspects of follow-up care after heart transplantation including growth and development, behavioural issues, psychosocial concerns, medical issues, immunosuppression, infectious diseases, and research endeavours. It is presented in an interactive format that encourages audience participation including case presentations and lunch time table discussion groups. The target audience is interdisciplinary and includes paediatricians, paediatric cardiologists, family doctors, pharmacists, and allied health caregivers including physiotherapists, occupational therapists, nurses, and dietitians amongst others.

3 Lunch Table Topics A sit-down buffet style lunch is included in your registration fee. Interdisciplinary team members will lead discussion at each table on selected topics. Sex, Drugs & Rock and Roll Adolescent Issues Come and explore the challenges that face all of us in caring for the teenage patient with a chronic medical condition. Focus will be on risk-taking behaviours, strategies to deal with non-adherence (a significant cause of death), and transition. Eating, Learning, Playing, Competing Rehab Issues Come and explore the challenges post-transplant including growth, feeding, development, physical fitness and others. Kids will be Kids: Joys and Challenges of Parenting Post-Heart Transplant. Going back to school, making friends, and becoming independent. What strategies work and what resources are needed. Offense Defense Welcome to the Team! Building effective partnerships for transplant care. We would like your input on what you need from us. Asthma or Aspergillosus? Medical concerns when should I worry? Discuss common presenting complaints and how to approach them. You ask the Interprofessional Team! topic and discuss with our team of experts.
