2 Death Certification Review Service The Certification of Death (Scotland) Act 2011 will strengthen checks on the accuracy of Medical Certificate of Cause of Death (MCCDs) by setting up a new national review system which aims to promote greater public assurance and confidence in death certification in Scotland. The new arrangements are designed to: improve the accuracy of death certification, and deliver a more cohesive approach.
3 Death Certification Review Service Key benefits of the new system include: effective, independent scrutiny of certifying MCCDs for deaths that do not require a Procurator Fiscal investigation, more accurate MCCDs, a more consistent approach where all deaths must be registered before burial or cremation of the body, and improved public health information about causes of death. The Act states that Healthcare Improvement Scotland will run the Death Certification Review programme.
4 Death Certification Review Service The Team Experienced and trained Medical Reviewers will review a random sample of 4,000 6,000 MCCDs every year to ensure that they are accurate and meet the required standard. Healthcare Improvement Scotland s reviews will focus on the accuracy of the paperwork and the description of the cause of death. They do not review clinical care or the quality of healthcare before death.
5 Death Certification Review Service Process If an MCCD is selected for any level of review: Any certifying doctor (or an informed member of that doctor s team), will be required to make themselves available to discuss the case with the Medical Reviewer if contacted. A timely response to such approaches will minimise delays to the funeral and the distress to the bereaved. If an MCCD is selected for a Level 1 Review: A Medical Reviewer will contact the certifying doctor (or another doctor in the team) before authorising completion of death registration. A Level 1 Review will take one working day to complete.
6 Death Certification Review Service Process If an MCCD is selected for a Level 2 Review: A Medical Reviewer will contact the certifying doctor (or another doctor in the team). Check electronic and/or copy of medical records, before authorising completion of death registration. A Level 2 Review will take a maximum of three working days to complete. Both Levels of Review: Medical Reviewers will provide feedback and training/education to NHS staff involved in completing MCCDs in order to improve the accuracy of certificates.
7 Timelines 6 August The new paper Medical Certificate of Cause of Death form was rolled out across Scotland. 29 April The new system for reviewing death certificates scheduled to be launched and electronic recording of MCCD data in the NHS to be rolled out.
8 MCCD Health Board Activities Local Implementation Groups formed for each Health Board combined meetings chaired by George Fernie, Senior Medical Reviewer. Responsible for: disseminating information and resources, ensuring relevant NHS staff are trained on new procedures before implementation, local operational implementation and associated communications (through Local Implementation Leads) with other key operational organisations, raise awareness of the new system with families and, where possible, inform them of potential selection for review before they attend a registration office.

10 Electronic MCCD (emccd) Scottish Government revised the original go-live date to allow for parallel introduction of..electronic Medical Certificate of Cause of Death (emccd) forms into the NHS. Certifying Doctors complete the MCCD on-screen, Print MCCD and sign, MCCD data is transferred to National Records of Scotland (NRS) at the point of printing, NRS apply randomiser process, Where selected, MCCD data is passed to Medical Review Team to complete the review.

Develop and implement a means of transmitting MCCD digital death data to National Records of Scotland (NRS). Targets: Available in all GP Practices from 29 April 2015 Secondary Care to be rolled out through 2015
12 Develop and implement a means of completing an electronic MCCD; Develop and implement a means of transmitting MCCD digital death data to National Records Scotland (NRS). emccd and SCI Gateway SCI Gateway will be the underlying transport mechanism to send data to NRS. NRS have specified that SCI Gateway is used to control and manage serial numbers to be applied to each emccd. Additional information requests will be transmitted from the Medical Review Team via SCI Gateway.

Development is required for access to the emccd via the referral route currently in place and included in both GP systems: EMIS Vision

Requirements Specification signed of by Project Board. SGPC engaged. Web services being developed in partnership with NRS.

Death Reporting to Procurator Fiscal - ef5 Current methods for reporting death to PF can take significant time 15mins to over 1 hour. Waiting times to report are increased after weekends and public holidays. COPFS accept reports during business hours only - (Monday to Friday).

Death Reporting to Procurator Fiscal - ef5 Electronic Reporting project is the secure electronic transfer of information between NHS and SFIU utilising existing IT applications and infrastructure. Aims to makes the recording of deaths more efficient and to provide better outcomes for relatives, NHS Medical Practitioners and SFIU staff. Joint pilot with Hospices commenced 15 October. Roll out across Primary Care during Winter Roll out to Secondary Care is packaged with emccd.
