4 Application Deadlines To start Winter Quarter 2018 Application deadline: October 13, 2017 To start Spring Quarter 2018 Application deadline: January 26, 2018 To start Fall Quarter 2018 Application deadline: July 3, 2018 Courses cannot be in progress when submitting an application Take TEAS by deadline and have results electronically submitted to ATI database for LWTech viewing. Do not have scores ed to LWTech
5 GPA BEARING COURSEWORK REQUIRED FOR COMPLETE APPLICATION ENGL& 101 MATH&146 (Statistics) BIOL&241 (Anatomy & Physiology 1) BIOL&242 (Anatomy & Physiology 2) PSYC&200 (Lifespan Psychology) Cumulative GPA of 3.0 or better Each course 2.0 or better Points for this application element are tabulated by multiplying the cumulative GPA by 10 Science courses must have been completed within last 10 years
6 REQUIRED APPLICATION ELEMENT: INTRO. TO CHEMISTRY CHEM &121(a required application element) not part of cumulative GPA Completed with 2.0 or better
7 REQUIRED APPLICATION ELEMENT: RECOMMENDATIONS Two (2) recommendation forms needed: 1 from must be from a college faculty/instructor 1 from an employer/supervisor or coworker Supervisor and/or co-worker can come from a volunteer setting Recommenders fill out forms provided in application, place in sealed envelope and give back to you. You submit with app.
8 REQUIRED APPLICATION ELEMENT: Certified nursing Assistant license or medical assistant license or LPN license or Military JST for military nurse or corpsman A non-points bearing requirement Applicant provides official proof of license or military related medical service License must be verifiable on the department of health website. Please note that a registered nursing assistant is not accepted for this requirement.
9 REQUIRED APPLICATION ELEMENT: SHORT ESSAY RESPONSE Review details regarding essay on page 7 of the 2018 supporting information packet found on the nursing program page. This is a points bearing element. Question: Describe a circumstance, obstacle, or conflict in your life and the skills and resources you used to resolve it. What did this teach you and how will that knowledge support your success in the nursing program at LWTech?
10 REQUIRED APPLICATION ELEMENT: SHORT ESSAY RESPONSE: continued Essay considerations:» Have your essay read by somebody else to edit for grammar and spelling» Consider using campus resources for editing and writing style guidance such as: Writing Center; Learning Lab and/or advice from previous ENGL course instructor if available» Answer all parts of the question» Your essay will be scrutinized for content, clarity of message, grammar and spelling» If evidence of plagiarism or academic dishonesty is discovered, your application will be rendered unacceptable and materials will be forwarded to the conduct officer for review and possible adjudication
11 TEAS Version 5 or New ATI TEAS Details Testing Policy: Take test first time, then wait at least 30 calendar days before taking test again. After that retake, you must wait 365 calendar days before the testing cycle can begin over again. Scores are valid for two years The LWTech offers the TEAS test. Plan ahead and schedule an appointment in advance of the deadline. Spaces fill up quickly and seating is limited. You may find that other assessment centers in the area can accommodate your need for a test date up to or on the application deadline.
12 REQUIRED APPLICATION ELEMENT: TEAS (VERSION 5) = TEST OF ESSENTIAL ACADEMIC SKILLS Reading paragraph comprehension, passage comprehension, and inferences/conclusions (48items, 58 minutes) Math whole numbers, metric conversion, fractions, decimals, algebraic equations, percentages, and ratio/proportion, stats (34 items, 51 minutes) Science science reasoning, science knowledge, biology, chemistry, anatomy, physiology, basic physical principles, and general science (54 items, 66 minutes) English and Language Usage punctuation, grammar, sentence structure, contextual words, and spelling (34 items, 34 minutes) GRAND TOTAL=170 questions in 3 hours & 29 min. LWTech.edu/assessment for TEAS questions
13 Q: WHAT ELSE CAN MAKE ME A COMPETITIVE APPLICANT? A: TAKE THE CO-REQUISITE CLASSES IN CASE OF A TIE (2.0 OR BETTER IN EACH COURSE) Co-requisite courses are: NUTR&101 BIOL&260 CMST &210 or 220 or 230 You are making a decision about making them a part of your class plan now, or taking them as part of the Nursing AAS-T program itself in quarters IV and V or during a summer quarter

You cannot add levels together. This is an optional application element
15 OPTIONAL PRIOR EDUCATION: continued A points level is available for applicants completing NAC training at a community college where course was delivered through non-credit continuing ed. or was facilitated as non-credit bearing program At the Associate s Degree points level: Applicants that may have begun a baccalaureate program and accumulated credits to a level equivalent to junior standing at a baccalaureate degree granting institution qualify for points at the Associate s Degree points level
16 SELECTION CRITERIA Prerequisite courses & admission requirements not mentioned in calculation below are still required for your application to be considered complete 40 pts 30 pts Cumulative GPA Taken from the five prerequisite courses Multiply cumulative GPA by 10 TEAS Multiply your TEAS Composite Score by 0.30 for your total number of points. 0 pts Recommendations 25 pts 5 pts Short Essay Response Optional Prior Education from Regionally Accredited Institution 1 pt completion of non-credit, community college NAC program 2 pts for cert. of completion health care related (less than 45 cred.) 3 pts for cert of proficiency health care related (more than 45 cred.) 4 pts Associate Degree (or equiv; equiv. defined as Junior standing at a baccalaureate granting institution) 5 pts Bachelors or above 100 total points possible

Note that application instructions are contained in the companion Supporting Information for Nursing AAS- T document Read the Supporting Information online it does not have to be printed off and included with your application submission
18 Once application is submitted Enrollment Services reviews application packet determines completeness Cross-check what you self-reported with documents you provided, and tally your points Recommendations and essays are reviewed by committee and assigned points for the essay. Top 24 applicants are offered admission to the Nursing AAS-T Program There is no wait-list system at LWTech
