2 Location: Hours: Market Street, Hartford, CT 35 hours per week, 52 Weeks Position Summary: The Vice President Clinical Support Services is responsible for the senior level leadership and management of all operational aspects of the entities in the Clinical Support Services Line. The successful candidate will provide strategic leadership, operations, and quality and safety management to assure a comprehensive Clinical Support Service Line for the agency. Reports to the CEO/President. Administers staff, operations and programs of the agencies Clinical Support area and other related clinical programs. Develops implements and evaluates program policies, goals and objectives and regulatory provisions. Assures program compliance with performance standards and other related criteria. Implements new program procedures as needed. Determines appropriate staffing levels and directs management and coordination of staff; assures staff performance review standards. Maintains contact with individuals both within and outside of the agency who might impact on programs and services. Serves as executive team member participating in development, implementation and evaluation of agency policies, goals and objectives. Manages departmental service systems; develops requests for proposals as necessary; administers state and federal regulations pertaining to agency Clinical Support care programs and other related clinical services and operations. Participates in provider contract negotiations as necessary. Responsible for managing budgets, contracts and fiscal operations for program areas. Identify opportunities for the expansion of clinical services. Provides strategic direction about growth opportunities and expansion. Ensures the quality, efficiency, and effectiveness of all programs and services provided. Coordinates and directs related subsidiary budgets and monitors program expenditures in accordance with established budgets. Maintains effective relationships with funding agencies as well as all regional and national organizations. Responsible for accreditation activities; including Joint Commission. May serve as agency liaison with external regulatory, Clinical Support planning and advisory groups and other regulatory and service agencies. Performs other duties as assigned. Requirements: An advanced degree with Clinical background. and minimum ten (10) years of professional experience in Clinical Support care and clinical services delivery. Five (5) years of the experience must have been in a leadership capacity running behavioral health clinic with PHP/IOP services, directing programs and operations relating to clinical and community service programs. Considerable working knowledge of relevant state and federal laws, statutes and regulations pertaining to Clinical Support and related clinical programs; considerable working knowledge of principles, practices and issues of Clinical Support care; working knowledge of administrative and clinical operations, functions and systems involving Clinical Support service delivery; considerable interpersonal skills; considerable oral and written communication skills; considerable organizational skills; considerable analytical skills.. Ability to relate to clinical personnel, medical staff and senior management. Track record in development and management of an integrated service continuum. Title: Clinician II (2 positions) Locations: 330 Market Street Hartford, CT (1) & 1921 Park Street Hartford, CT (1) Hours: 40 hours per week, 52 Weeks Position Summary: Under the supervision of a Licensed Lead Clinician or Clinician Manager, this position coordinates and provides clinical services and will also provide specialized group and/or one on one counseling services. Provide individual and group Clinical supervision. Provide training for staff towards certification or recertification. Participate and contribute to the fee for service delivery model ensuring that program reaches financial benchmarks. Complete yearly mandatory training including confidentiality and HIPAA compliance. In Lead Clinician absence assumes duties temporarily. Perform all essential functions of Clinician I; including but not limited to. Provides crisis intervention services in

Title: Clinical Manager Location: 330 Market Street, Hartford, CT Hours: 3 40 hours per week, 52 Weeks Position Summary: Responsible for customer outreach and the coordination of case management and supportive services to program participants. Provides specialized group and/or one on one counseling services. In a hospital outpatient behavioral health treatment program, provides specialized group and/or one-on-one counseling services for mentally ill clients. Supervises all other Clinicians and client groups. Overseas all aspects of a Behavioral Health Clinic s operations, including; staffing, finances, licensing, and quality of care. Supervises staff and workloads as assigned. Train new staff on process and documentation for services received and outcomes measures in the Behavioral Health department. Completes center data for medical records, data systems, and contractual and departmental requirements. Ensure center staffing level meets program demand. Collect demographic data, conducts pre-screening and assesses potential eligibility for CRT, DSS or other community-based resources. Provide information and referral services and documents outcomes resulting from such referrals. Represent CRT at community meetings and outreach events. Maintain caseload of approximately 20 customers in need of Behavioral Health. Assign new customers to available case mangers and clinicians. Conduct service coordination with other organizations and resources. Develop comprehensive assessments, (intakes), for customers eligible for services at Connecticut Regional Mental Health Center, present customers for disposition for CRMHC services. Title: Case Manager Level I Location: 9 Moody Road Unit 3, Enfield, CT Hours: 19 hours per week, 52 Weeks P/T Position Summary: Based on CRT s Steps to Success model, the Case Manager is responsible for the coordination of supervision, intake, referral and discharge of customers. Daily monitoring of customer contacts and activities is a primary responsibility. Case Manager conducts assessments of customer needs and coordinates referrals to Community Renewal Team programs and initiatives, as well as to other community resources. Complete Intake Application on customer within STEPS System. Interview customers on and/or off site as required. Assess and evaluate customer needs based on the Outcome Scale Matrix (OSM) Questionnaire and Summary Matrix. Develop Individualized Service Plans, (ISP), for each customer that incorporates all special needs and outlines both short and long term goals. Coordinate customer referrals with social service system and other community support services. Monitor and document customer progress and adherence to Individualized Program including daily case file entries and/or possibly urinalysis collection if applicable. Provide reports to referring agencies including, but not limited to, progress reports, discharge summaries, aftercare plans and special reports. Provide individual counseling and facilitate groups. Provide community service work crew supervision if applicable. Ensure all intake information is, if required by program, accurately entered into STEPS and/or other systems as required. Collects demographic data, conducts pre-screening and assesses potential eligibility for CRT, DSS or other community-based resources. Facilitates information and referral services and documents outcomes resulting from such referrals. Facilitates customer access to community resources, including, but not limited to food, housing, clothing, school programs, vocational opportunities or services, life skills training and relevant mental health services. Maintains a caseload of customers in need of intensive case management or multiple service needs. Follow-up ever 3 months or as needed. Maintain customer file and information according to standard. Conducts home visits as needed. Participate in, retain and incorporate training as required. Provide effective customer service including, but not limited to; greeting, receiving and addressing clients with respect and dignity at all times. Perform all duties relative to special programs/ projects as required. Provide input and make recommendations regarding program/departmental policies, procedures and practices. Reports to and conducts additional duties as required by various funder guidelines and requirements. Filing, copying, scheduling appointments, answering phone calls and returning calls. Maintaining office equipment (copier and printer) and supplies. Performs all other duties as assigned.

5 Requirements: Associate's degree in Human Services, Criminal Justice, Social Work or related field, or responsible employment experience of at least three to five (3 to 5) years in Criminal Justice, Social Work or related field. Education may be substituted with experience on a year-by-year basis. Knowledge of Human Services, Criminal Justice and the Court systems is preferred. Knowledge of substance abuse and target population helpful. Counseling background preferred. Ability to interact with people of diverse backgrounds required. Ability to communicate effectively. Ability to compile written reports and maintain case notes. Must be available to work evenings, weekends, holidays and other shifts as necessary. Bilingual, Specialized training with HIV/AIDS, Drug and Alcohol Abuse and/or counseling, Domestic Violence and/or any other applicable training related to field of work. Intake and eligibility certification for all CRT programs including VITA, Energy Assistance, Early Care and Education, Eviction and Foreclosure Prevention and other CRT programs. Complete yearly mandatory training including confidentiality and HIPAA compliance. Conducts home visits. Content knowledge in employment education, financial literacy, prevention/ wellness education and housing. Community Services Title: Senior Case Manager Location: 330 Market Street Hours: 35 hours per week, 52 weeks Position Summary: Based on CRT s Steps to Success model, the Senior Case Manager is responsible for providing case management services to customers which may include the coordination of supervision, intake, assessments, referral, and discharge of customers and daily monitoring of customer contacts/activities. as applicable medical advocates Responsible for providing supervision, evaluating case managers, monitoring the integrity of the data entered in Agency s STEPS and Funder data collection systems and compiling required reports. Responsible for the length of stay policy at facilities and coordinates weekly case management team meetings. Coordinates referrals to CRT programs and initiatives, services, entitlement programs, networks throughout the community, and maintains relationships with other community action agencies. Requirements: Bachelor s degree in Social Work, Human Services or Criminal Justice with at least two (2) years of experience providing direct services to customers; minimum of two (2) years of experience with staff supervision in social services and an understanding issues relating to substance abuse, homelessness and mental illness. Education may be substituted with experience on a year-byyear basis. Knowledge of Human Services, Criminal Justice, Court Systems and Substance Abuse may be required. Counseling background preferred. Bilingual preferred. Ability to interact with people of diverse backgrounds required. Ability to communicate effectively a must. Ability to compile written reports and maintain case notes required. Title: Location: Hours: Salary: Family Service Worker Unit F- Clinton, Portland & Middletown, CT 40 hours per week, 36 weeks $14.23-$15.55/hour Position Summary: The Family Service Worker ensures maximum participation of eligible families in the ECE program by reaching out to and establishing relationships with families. Also facilitates parent involvement by securing social services/resources for families as indicated by the Head Start Performance Standards. Requirements: Bachelor s Degree in Sociology, Psychology, Human Development, or related field with at least one year of experience working in social services. Knowledge of Head Start Performance Standards, School Readiness Regulations and social service functions. Knowledge of local and state social service resources, agencies and regulations and knowledge of case management is essential. The ability to establish and maintain effective working relationships with children and families regardless of race, religion or culture, ability to build effective team relationships with co-workers, education staff and other administrative staff of the ECE program is also critical. Good time management, oral and written communication and assertiveness skills; accurate data collection and report writing skills are required. Title: Enrollment Outreach Specialist Location: 555 Windsor Street, Hartford, CT Hours: 40 hours per week, 52 weeks Position Summary: Under the Supervision of the Family Service Manager ensures full enrollment of eligible families who are experiencing homelessness, or receiving child welfare services or children in foster care into the ECE program by planning and implementing the coordination of ERSEA management system. Collaborate with Family Service Manager and Enrollment Coordinators to create goals and plans for the enrollment process. Recruit and enroll the neediest children and families living within our service area. Establish a community network with community agencies. Recruit and enroll underserved children who are experiencing homelessness, or receiving child welfare, especially children in foster care. Responsible for maintaining an accurate log of calls and files that come through community agencies and other sites. Work with the Family Service Coordinators and the Family Service Manager to maintain & update enrollment level on a weekly basis and organize on-going recruitment activities. Requirements: High school diploma/ged or Associate s degree in the Human Service field. Minimum 1 year of experience working in the area of social services, non-profit, outreach or enrollment. Microsoft office competency and prior experience with the CAPS recording system. Valid CT driver s license. Some use of personal vehicle is required. Bi-lingual English/Spanish preferred.

7 The Retreat Title: Neighborhood Coordinator Location: 90 Retreat Ave, Hartford Hours: 35 hours per week Position Summary: Candidates should have background in social services/social work with seniors. Candidates will conduct, organize, and implement enrichment programs for The Retreat members based on the Life Enrichment Scale established by The Retreat. In addition, involve team members, volunteers and the community in different aspects of enhancing the member s life through purposeful engagement. Some of the responsibilities is to maintain approved schedule of activities and community events, develop and implement new programs and retain, orient and assign volunteers to various programs. Requirements: Experience providing activities in a geriatric or health care setting preferred and must have or be willing to obtain a public service license. Veterans and Housing Services Title: Residential Aide Location: McKinney Shelter Hours: Part Time 2nd Shift Weekends Position Summary: Provide services of safety and security to all residents and staff at the shelter. Provide guidance in personal hygiene and maintain client personal hygiene supplies. Assists with admissions/intake procedures by providing security and illness/injury intoxication screening. Distribute medication and coordinate medical transportation. Provide surveillance of client population to prevent disruptive behavior. Make hourly rounds inside shelter as well as outside and maintain hourly logs. Record all necessary information including log entries, incident reports, demographic reports and bed rosters. Communicate with Probation Offices. Participate in conduct review committee. Engage with clients give words of encouragement and inspiration while they are at their lowest point of life. Dissolve physical confrontations. Supervise activities of client population who are at times intoxicated and potentially violent. Risk of exposure to Tuberculosis and other contagious diseases. Risk of exposure to blood borne pathogens. Requirements: High school diploma or Associate s degree in the human service field. Candidate must have one (1) year of experience in related field. Must be knowledgeable in mental health and substance abuse, have excellent written and oral communication skills and working with the homeless population. Non-violent Crisis Intervention, CPR and Community First Aid and Safety license is required. A medical and/or physical examination, including a Tuberculin test is required prior to appointment.
