Joanne August, Heather Wood, QuadraMed Preparing for Your Affinity to QCPR Implementation (target audience is Affinity and Pharmacy clients) This session will prepare you for the migration from Affinity to QCPR. You will learn how to prepare your organization for the change, and how to ready your implementation team. We will walk through the Implementation Methodology step by step so that you know what to expect throughout the project. Susan Vickery, Dava Zavodnick, Santina Allen, QuadraMed Multidisciplinary Initial Patient Assessment (target audience is QCPR clients) West Park Health Care center is implementing a multidisciplinary initial patient assessment electronically within Quadramed CPR. While many sites implement assessments using the assessment functionality along with the visit and patient historic assessment categories in CPR, unconventionally we decided to utilize the result documentation functionality and use data element dependencies (DEDs). This approach allowed us to create complex, highly clinically relevant screens that are easy for clinicians to navigate. We were also able to pull information forward from previous documentation using more complex criteria than would be possible with assessment data elements alone. Gordon Chong-Kit, West Park Healthcare Centre Christopher Kebbel, West Park Healthcare Centre

6 WEDNESDAY August 5 11:45am 11:45am 11:45am Essex C The Road to Successful BCMA Implementation: A Process Overview (target audience is Affinity, Pharmacy and QCPR clients) QuadraMed s Barcode Medication Administration (BCMA) system was successfully implemented in the Respiratory Department of Seton Medical Center in Daly City, CA. In this presentation, we chronicle the introduction of an integrated medication documentation and delivery system in a 357-bed acute community hospital. We detail the logistics involved with planning, construction, and staff training of an advanced barcode system within the respiratory department of a major hospital. Lessons learned will facilitate future implementation of a barcode system across multiple disciplines in a hospital setting to reduce medication error and improve patient safety. Grace Liao, Pharm. D., Daughters of Charity Health Systems (Seton Medical Center) Kim Schmidt, RN, Daughters of Charity Health Systems (Saint Louise Regional Hospital) Monte Schachner, RRT, Daughters of Charity Health Systems (Seton Medical Center) Meaningful Use What Will it Mean and How Will I Get There? (target audience is Affinity, Pharmacy and QCPR clients) Review of current proposals for defining meaningful use. Discussion of the impact to QCPR sites at varying stages of implementation, and description of the services available from QuadraMed to assist our clients in demonstration of meaningful use. Linda Baum, QuadraMed University Health Network's Medication Report Suite (target audience is Affinity, Pharmacy and QCPR clients) Medication Ordering and administration is a powerful part of Quadramed CPR. University Health Network implemented inpatient Medication Order Entry/Medication Administration Record (MOE/MAR) in two acute care facilities, in multiple care settings including ICU. Clinicians found that while the built in CPR functionality offered many different views of medication information, it was still difficult to get a comprehensive view of a patients medication on a single level. In response we developed a series of HTML and printable reports to present the users with a clinically view reminiscent of the paper MAR. They are divided into event (dose) centric, order centric and procedure centric views. We also developed a report for system downtime. Christopher Kebbel, University Health Network

7 11:45am 1:30pm- 2:30pm 1:30pm- 2:30pm 4:45pm Panel Discussion: AcuityPlus Data Utilization and Management (target audience is AcuityPlus clients) During this session the panel members will describe their organization and their role in managing the use of the AcuityPlus system within it. They will highlight the importance of an RN in the role and relate how the position is justified within their respective organizations. The speakers will also describe the challenges related to how various system components are established and maintained; how relevant information is communicated up and down within the organization; techniques for promoting system visibility and managing staff orientation and on-going system education. Additional discussion will focus on opportunities for system utilization related to organizational structure. Candy Douglass, Charleston Area Medical Center Ruth Plumb, Sharp Healthcare Yolanda Jennissen, Mayo Clinic Clinician Practice Council (target audience is Affinity, Pharmacy and QCPR clients) Cindy Bolton, Kingston General Hospital QCPR Team, QuadraMed Never Events Should Never Happen (target audience is AcuityPlus and QCPR clients) This year, as part of the rules for determining how hospitals would be paid in 2009, CMS responded to concerns that hospitals were being reimbursed higher DRGs for some complications that were actually acquired in the hospital. Consequently CMS issued its Final Rule that applied to inpatient Medicare services for the 2009 fiscal year, starting October 1, This session was designed to assist hospitals in looking at the prevalence of Hospital Acquired Conditions (HAC) using national data sets, understand how to capture and analyze the historical data and access current exposure and identify process improvement initiatives. Virginia Marcus, Senior Product Manager, QuadraMed Joanne August, RN, MS, Manager, QuadraMed Heather Wood, RN, MBA, Product Manager, QuadraMed Clinician Practice Council (cont.)
