2 Community Mental Health Practitioner - Relief Worker Position reference Position type Casual Classification Remuneration Area/division/state Effective date Community Mental Health Practitioner (CMHP) Level 2/1 $27.24 per hour plus 25% casual loading, any applicable penalties, 9.5% superannuation and salary packaging. South East Community Services/South East Victoria /Victoria November 2017 To provide services to customers, families and carers in line with Mind s model of Recovery Oriented Practice and according to the values of Mind. To be available to work in a range of service types and quickly orientate to the service and customer s needs, to provide strength focused support. This position is to support services in the Melbourne South area which includes the Inner South Metro Area, Bayside, The Mornington Peninsula and Outer South (Dandenong and Cardinia). Mind services within this area include: Adult Residential Sub-acute Intensive Outreach Community Care coordination Family and carer
3 Provide direct support to individual customers Work with customers to enable them return to their place of residence and a meaningful life through supporting them in key stages of their recovery: Welcoming and engagement Skill and capacity development Maintenance of natural supports Service exit and on-going self-management support Assist the customer with actioning their individual recovery plan in a range of areas including: Understanding and managing customer s own mental health Developing daily living skills and capacity for self-care Crisis and incident management Addressing stigma Having an awareness of trauma Managing issues associated with drug and alcohol issues Support to maintain or create meaningful activity through participating in community life, including education and employment and utilising public transport Provide support to families and carers Support family and carer roles through understanding their concerns and the provision of information Work intensively with families and carers at the time of transition back to community Undertake group work Deliver group work programs that will assist customers to build their skills, focus on their recovery and work towards transitioning back to their natural Work with local service providers Assist the customer with accessing a range of local service providers, in line with their individual recovery plan including: clinical mental health, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation and social connections Housekeeping Contribute to the day to day operations of the residential service through undertaking a range of housekeeping duties including: Preparing rooms for new residents Washing linen Food shopping and meal preparation Ensuring all communal areas are home-like and welcoming at all times Work with clinical partners Work within a multidisciplinary team: Supporting recovery oriented practice Supporting clinical interventions Actively participating in team, case and handover meetings Work with other services in the residential cluster Run group workshops and outings with other services Work at other services as required Other Documents all activities using Mind s ICT system and processes Actively participate, contributing to your team and wider organisational initiatives Undertake relevant training and professional development, including regular supervision Take personal responsibility for the quality and safety of work undertaken Other duties as delegated
4 Reports directly to the Service Manager Core Requirements Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind Current valid Driver s Licence Current National Police Record Check Current Working With Children Check (Employee) Not listed on the Disability Worker Exclusion Scheme list 1 Responsiveness (PA-Res-2) Models resourcefulness and a positive, solution focused approach to a changing environment 2 Customer service (RS-Cus-2) Models and promotes flexible and customer focused standards of customer service 3 Communication (RS-Com-2) Models effective clear confident communication, active listening and supports others to do the same 4 Commitment to team (RS-Ctm -1) Actively cooperates and contributes to team discussions with a focus on creating solutions 5 Planning and prioritising (RE-Pap-1) Prioritises individual tasks and priorities in line with broader team goals and monitors progress 6 Safety (BE-Saf-2) Models, creates and supports behaviours that drive a high standard of health and safety in the workplace
5 At different times throughout the year, Mind services need to employ casual and fixed term staff to ensure continuity of service. Cover during these times is provided by staff selected from the Mind list of workers available for relief work. Staff on this list may be asked to work to cover a staff member who is on extended leave, or they may work much shorter periods to cover unexpected short-term vacancies. Shifts may be weekdays or weekends, and could be daytime shifts, evening or sleepover. Relief workers need to be willing to undertake fixed term contracts as well as unplanned shifts. Relief Workers are inducted into the operations of the service in which they are rostered to work. Staff on the Relief Worker Register come from various professional backgrounds. Relief work offers a flexible form of employment to suitably qualified staff who may continue the development of their professional skills and receive workplace support. Relief work can form an initial opportunity for career development within Mind, or it may allow staff who have other responsibilities or interests to continue to work in a field that they find rewarding and fulfilling. Casual work is paid as per Mind s Industrial Agreement (which includes 25% loading). Accrual of leave, salary increments and payments for public holidays not worked will not apply to casual work. Fixed term work is paid as per Mind s Industrial Agreement. Sick leave and annual leave will be accrued. Fixed term workers will be paid for public holidays or substituted days that fall on their usual or rostered days of work. Penalty rates apply for evening and weekend shifts, for both casual and fixed term work. Mind requires a minimum availability of 12 hours or two shifts per week that can be worked on weekdays or weekday sleepover shifts. Appointment is contingent on your name not being listed on the Disability Worker Exclusion Scheme list. Employer superannuation contribution will be paid as legislative required.
