2 Foundaton of the FSTB Certfcaton System Equal Access for All Regstratons for certfcaton wll be accepted from any canddate meetng the crtera lsted n the approprate certfcaton document. Applcatons wll be accepted wthout regard to race, color, creed, gender, martal status, or status as a mltary veteran. Applcatons wll be accepted wthout regard to age as long as the applcant s at least 18 years of age, and dsablty as long as that dsablty does not hnder the applcant s ablty to perform the necessary sklls and practces requred by the Natonal Fre Protecton Assocaton s Professonal Qualfcatons Standards. If you have questons or comments regardng the certfcaton, tranng, or other servces provded by the Fre Servce Tranng Bureau (FSTB), please call Legal Authorty Legslatve authorty for fre servce certfcaton was granted to the Fre Servce Insttute, Iowa State Unversty Extenson by the Iowa legslature n Wth the passng of HF 2492 and the permsson of the governng Internatonal Fre Servce Accredtaton Congress (IFSAC), authorty for fre servce certfcaton was granted to the Fre Servce Tranng Bureau, Dvson of the State Fre Marshal effectve July 1, Accredtaton The FSTB certfcaton system has been accredted by the IFSAC to certfy fre servce personnel to accepted natonal standards. Through ths process, Iowa fre servce personnel acheve nternatonal certfcaton. Accredtaton of the FSTB certfcaton system assures the Iowa fre servce that programs and standards used wthn ths system adhere to the most current recognzed and adopted natonal standards. The assurances ganed through the accredtaton process provde credblty and prestge for every frefghter partcpatng n the certfcaton process. Frefghters certfed through the Iowa system are recognzed throughout the Unted States (and the world) as qualfed and meetng the standards set forth by the Natonal Fre Protecton Assocaton (NFPA). Certfcaton After partcpants wthn the FSTB certfcaton system have met the performance requrements and successfully completed an objectve evaluaton process, the FSTB awards Natonal Certfcaton. Upon awardng Natonal Certfcaton, the ndvdual s name s entered n the Iowa certfcaton data bank as well as the natonal certfcaton data bank mantaned by the IFSAC at Oklahoma State Unversty. An offcal seal from the IFSAC s attached to every certfcate and ncludes an ndvdual natonal certfcaton number. Standards NFPA Professonal Qualfcatons Standards are used by the enttes accredted by the IFSAC. To mantan accredtaton, enttes must certfy fre servce personnel to the latest edton of the Professonal Qualfcatons Standards adopted by the ssung jursdcton. System Integrty The requrements, crtera, and processes explaned n ths document are used to ensure the hghest possble credblty and stature for partcpants n the FSTB certfcaton system. These processes help ensure that Iowa traned and certfed fre servce personnel are among the most hghly qualfed frefghters n the world.

3 Enterng the System Qualfed personnel wthn the Iowa fre servce may enter the certfcaton system for the purpose of seekng certfcaton by contactng the FSTB. Upon recept of a request, the FSTB wll forward approprate nformaton to members of the fre servce about any of the levels of certfcaton avalable. Applcaton & Fees Every ndvdual seekng certfcaton wthn the FSTB certfcaton system must submt an Onlne Certfcaton Testng Regstraton Form and a $50 Regstraton Fee to secure entrance nto the system. Approved methods of payment are: Cash, Purchase Order, Check, Money Order, Credt Card, or Bll to Fre Department. Canddates must submt ther Onlne Certfcaton Regstraton Form and payment at least three (3) weeks pror to the requested exam date to be accepted and allow for processng. NO WALK-IN OR LATE REGISTRATIONS WILL BE ACCEPTED AT ANY TEST SITE! Refunds of Regstraton Fees Regstraton fees may be refunded only wthn the frst two (2) months after beng receved by the FSTB. Regstraton fees wll only be refunded f the ndvdual has not taken any acton towards certfcaton, other than the submsson of the regstraton and fee payment. Regstraton fees wll not be refunded f any of the steps of the certfcaton process have been ntated (e.g., submttng local documentaton form, takng wrtten or practcal exams). Certfcaton Process Tmelne Canddates testng for ALL LEVELS (Frefghter I and/or II, Hazrdous Materal Awareness and Operatons, Drver/Operator - Pumper and/or Aeral, Fre Servce Instructor I and/or II, Fre Offcer I and/or II, Inspector I, and Fre Investgator) wll be gven twelve (12) months to complete the certfcaton process. The twelve (12) month tme lmt begns when the applcaton s entered nto the database by the FSTB Certfcaton and Accredtaton Unt staff. The FSTB wll NOT accept any documentaton requrments untl the canddate s regstraton has been entered nto the certfcaton database. After the twelve (12) month tme perod has expred, canddates not completng the certfcaton process wll be requred to start the process over by submttng a NEW regstraton form and fee payment. Canddates wll also be requred to retake the wrtten and practcal exams (regardless of the pass/fal status from the prevous exams) and re-submt all requred documentaton. All nformaton and paperwork from expred certfcaton attempts wll be removed from the FSTB database. Certfcaton Exam Avalablty Every qualfed ndvdual seekng certfcaton wll be allowed access to the certfcaton exam process (see Certfcaton Requrements ). Certfcaton exams are offered n numerous locatons throughout Iowa every year. They are conducted on a regonal bass at many of the Iowa Communty College regonal fre schools, select fre departments, and at the Annual State Fre School conducted by the FSTB. A Certfcaton Schedule s posted on the Certfcaton Page of the FSTB webste, Each canddate seekng certfcaton wthn the Iowa FSTB Certfcaton System must submt the Onlne Certfcaton Testng Regstraton Form. Restrcted test stes are also conducted for fre departments havng enough canddates to fll a test ste. Fre departments wshng to schedule a restrcted test ste should contact the FSTB for more nformaton.

4 Certfcaton Requrements Facal Har Polcy Persons wth beards or facal har n the area of the SCBA face pece seal (ncludng full-face beards, muttonchops, goatees, etc.) wll NOT be allowed to partcpate n the sklls exam for any certfcaton level. Ths polcy wll be enforced regardless of the sklls beng tested at any ste. No exceptons to ths polcy wll be granted. Ths s n conjuncton wth 29 CFR , Respratory Protecton Standard. Exam Results After canddates have completed the exam process, the FSTB begns the process of scorng and processng the test results. As soon as scorng and documentaton are completed, the results are sent to the canddate va regular mal. To ensure canddate confdentalty, certfcaton exam scores wll not be released over the telephone or any other mechansm. The canddate should receve test results wthn 30 workng days of the exam. If canddates do not receve ther test results wthn 30 workng days, they should notfy the FSTB. Exam scores wll stll not be released by phone, but the FSTB wll begn a trace of the notfcaton and/or explan the delay. Please DO NOT contact the FSTB Certfcaton and Accredtaton Unt pror to the 30-workng day tmeframe. Certfcaton Upon successful completon of the certfcaton process, the applcant s name wll be entered nto the Iowa certfcaton database and the Natonal Certfcaton Database mantaned by the IFSAC. Canddates wll also receve (at no addtonal cost), an Iowa/Natonally Certfed patch, a certfcaton level bar, and an ndvdualzed certfcate awardng Natonal Certfcaton from the FSTB. Ths certfcate wll bear a numbered seal from the IFSAC. When you have completed all of the necessary steps for your certfcaton level, please allow 60 workng days for the completed certfcate. If you have not receved your certfcaton wthn that tme perod, please contact the FSTB to verfy all materals are completed and receved. Please keep a copy of all your documentaton for your records. Recprocty Indvduals n possesson of certfcatons from out-of-state/other IFSAC enttes may request recprocty from the FSTB. The FSTB wll only grant recprocty for the levels of certfcaton that the FSTB s accredted to by the IFSAC. Indvduals must complete and submt the Onlne Request for Recprocty Form, avalable on the FSTB webste along wth a $50 Recprocty Fee. Once the provded IFSAC seal has been verfed, the certfcate nformaton wll be entered nto the Iowa certfcaton database and the FSTB wll recognze the ndvdual as havng attaned natonal certfcaton at the requested level. The FSTB wll not provde a new IFSAC certfcaton seal number or certfcate as the ndvdual has already attaned natonal certfcaton and recognton at sad level. If the ndvdual or ther afflated organzaton specfcally desres or requres an Iowa certfcaton for the level n queston, they must complete the steps for Certfcaton Testng Regstraton, as descrbed prevously, and complete the wrtten and practcal exams, along wth payment of the Certfcaton Regstraton Fee. Denal and Revocaton of Certfcaton The FSTB may deny or revoke a canddate s certfcaton f the canddate submts false, coped, or plagarzed nformaton to the FSTB or s found to be cheatng durng any phase of the certfcaton process.

5 Testng Accommodatons The Amercans wth Dsabltes Act (ADA) requres that reasonable accommodatons be provded to qualfed ndvduals wth dsabltes. Ths law defnes dsabltes as physcal or mental mparments that substantally lmt one or more of a person s major lfe actvtes (e.g., walkng, hearng, speakng, seeng, readng, or wrtng). The FSTB wll provde reasonable and approprate accommodatons to qualfed candates who have documented dsabltes and demonstrate a need for accommodatons durng the examnaton. A canddate must ntate a request for accommodaton by completng the FSTB Request for Accommodaton Form, avalable on the FSTB webste. In addton to submttng the on-lne request form, a canddate must provde documentaton from a qualfed health professonal, to nclude: 1. A statement of the specfc dagnoss of the dsablty. 2. Cte the dagnostc crtera and tests gven, wth dates, results, and nterpretatons. Cte how the results support the dagnoss. 3. Descrbe the canddate s functonal lmtatons due to the dsablty, and the mpact of those lmtatons on physcal, perceptual and cogntve abltes. 4. Recommend specfc accommodaton(s) and for each accommodaton, provde a ratonale as to how t wll reduce the mpact of the functonal lmtaton(s). 5. State your professonal credentals, tranng, work experence and any lcenses you hold that support your qualfcatons to dagnose and/or treat ths canddate s dsabltes. 6. If no pror accommodatons have been made for ths canddate, explan why. If they have - state what was done and provde past documentaton of pror accommodatons. Please note: The aforementoned supportng documentaton provded by a qualfed health professonal must be receved at the FSTB at least 10 calendar days pror to the test date. Please mal supportng documentaton to: Certfcaton Unt Fre Servce Tranng Bureau 1015 Haber Road Ames, IA Appeal Process If certfcaton s dened or revoked, the ndvdual s enttled to due process, ncludng an appeal and hearng. Any person belevng that they have not receved far treatment wthn the certfcaton process s also enttled to the appeal process. The frst step n the appeal process s for the ndvdual to contact the Certfcaton Manager at the FSTB. Ths ntal contact s ntended to be an nformal revew of the specfc detals. The offcal appeal process must be ntated n wrtng by emal wthn 30 days of the acton beng appealed. The appeal must nclude the followng: Indvdual s FSTB Identfcaton number. Current address, phone number and emal address. Statement of the ssue(s) nvolved & date ncdent took place. Remedy requested.

6 The Certfcaton Manager shall, wthn 21 workng days from the date the appeal s receved, provde a wrtten response by emal to the ndvdual requestng the appeal. If the appeal s not resolved durng ths step, the ndvdual appealng may move to the second step and appeal to the Bureau Chef of the FSTB. Ths appeal must be submtted n wrtng by emal to the Bureau Chef wthn 7 workng days from the Certfcaton Manager s response. An ndvdual may request a formal revew of the appeal through an n-person meetng wth the Bureau Chef. The Bureau Chef shall wthn 21 workng days after the 2nd step appeal s receved, provde a response n wrtng by emal to the ndvdual requestng the appeal. The decson of the Bureau Chef wll be fnal. 7 Introducton to Fre Apparatus Drver/Operator Pumper Certfcaton Onlne Certfcaton Testng Regstraton Form Certfcaton Unt emal: FSTB webste: FSTB Certfcaton phone number: Each ndvdual seekng certfcaton wthn the Iowa Fre Servce Tranng Bureau Certfcaton System must submt the onlne Certfcaton Testng Regstraton Form, and a $50 Regstraton Fee, to secure entrance nto the system. The form s avalable on the Certfcaton Page of our webste at or scan the code to go drectly to the regstraton form. A Certfcaton Schedule (located on the Certfcaton web page) s updated regularly to reflect a current lst of avalable wrtten and practcal test stes. Canddates are gven twelve (12) months n whch to complete the certfcaton process. Ths tme lmt starts when the canddate s FIRST regstraton to test form n submtted and entered nto the FSTB database. Certfcaton canddates are gven two (2) attempts to pass each component, wrtten and practcal, of the examnaton process for each $50 regstraton fee pad wthn the twelve (12) month tme lmt. Canddates falng the wrtten exam are responsble for notfyng the FSTB of ther desre to retest by regsterng for another scheduled exam on the FSTB Certfcaton web page. Canddates may not take the wrtten exam more than once per day. If the canddate has faled both attempts at the wrtten exam, they must submt another $50 regstraton fee, whch wll allow them two (2) more attempts. Canddates wll have two (2) attempts to pass each skll staton durng the practcal exam. If the canddate fals both attempts at any skll staton, ths consttutes a practcal exam falure for that specfc certfcaton level and the canddate wll be responsble for notfyng the FSTB of ther desre to retest by regsterng for another scheduled exam on the FSTB Certfcaton web page. Canddates may not take the practcal exam more than once per day. If the canddate has faled both attempts at the practcal exam, they must submt another $50 regstraton fee, whch wll allow them two (2) more attempts. If the twelve (12) month tme lmt expres and the canddate has not successfully completed all the requrements for the certfcaton process, the canddate wll have to restart the process. All nformaton and documentaton relatng to the prevous certfcaton attempt wll be removed from the FSTB database. Local Documentaton forms and/or Projects must be submtted and approved wthn the twelve (12) months from when the canddate s certfcaton applcaton was entered nto the FSTB database. An offcal pcture ID (e.g., drver s lcense, state-ssued dentfcaton card, mltary ID, etc.) must be presented for admttance to ALL wrtten and practcal exams. Each canddate applyng to certfy at ths level s requred to brng NFPA complant Personal Protectve Equpment (PPE) and a Self-Contaned Breathng Apparatus (SCBA).

8 Fre Apparatus Drver/Operator Pumper Certfcaton Requrements Prerequstes: Canddates seekng certfcaton, for Drver/Operator Pumper, wthn the Iowa Fre Servce Tranng Bureau Certfcaton System must meet the followng: 1. Be a current member of an Iowa fre, emergency or rescue organzaton. In addton to the membershp requrement, all persons must be at least 18 years of age. 2. Submt the Drver/Operator Pumper Local Verfcaton Form, sgned by the Fre Chef or the Tranng Chef, verfyng the followng requrements: a. Vald state drver s lcense. b. Approved by Chef of Department to drve all vehcles the canddate s expected to drve. c. Demonstrated ablty to document routne tests, nspectons, and servcng functons per department protocols and procedures. d. Demonstrated ablty to operate a fre department pumper under adverse envronmental condtons. e. Demonstrated ablty to operate a department pumper usng defensve drvng technques under emergency condtons (e.g. lghts and srens ). f. Operate fre department pumper on publc roads. 3. Canddate must be competent n all objectves lsted n NFPA Standard 1002, Standard for Drver/Operator Pumper Professonal Qualfcatons, 2014 edton, Chapter 4 and Canddate must be prevously natonally certfed NFPA 1001 Frefghter I and NFPA 472 Hazardous Materals, Operatons Level. Wrtten Examnaton: The Drver/Operator - Pumper wrtten certfcaton exam s based on Requste Knowledge objectves lsted n NFPA 1002 Standard for Fre Apparatus and Drver/Operator Professonal Qualfcatons, 2014 Edton, Chapters 4 & 5. Canddates are requred to score a mnmum of 70% on the wrtten examnaton. 9 Practcal Sklls Examnaton: The Drver/Operator - Pumper practcal sklls porton of the certfcaton exam s based on Requste Knowledge and Requste Sklls objectves lsted n NFPA1002, Standard for Fre Apparatus Drve/Operator - Pumper Professonal Qualfcatons, 2014 Edton. A mnmum of three (3) sklls wll be selected to be tested at each exam ste. Canddates should be prepared to test on the sklls lsted n the standard. Practcal sklls exams are graded on a pass/fal bass. Canddates must successfully complete all skll statons at an exam ste to receve a passng grade for the skll exam. The canddate s allowed two (2) attempts at each staton. Canddates falng the practcal sklls exam are responsble for notfyng the Fre Servce Tranng Bureau of ther desre to retest by regsterng for another regularly scheduled exam. NFPA , JPR 4.3.6, Operate Fre Department Vehcle Usng Defensve Drvng Technques Under Emergency Condtons: The canddate has successfully demonstrated the ablty to safely and successfully drve a department pumper ncludng, but not lmted to, the ablty to operate passenger restrant devces; mantan safe followng dstances; mantan control of vehcle whle acceleratng, deceleratng, and turnng; operate the vehcle under adverse envronmental or surface condtons; and use automotve gauges and controls, all whle operatng under emergency condtons (e.g. lghts and srens ). 5. NFPA , JPR 4.3.1, Operate Fre Department Vehcle: The canddate wll be requred to safely operate a fre department pumper on publc roads. That wll test understandng of operatons n dfferent areas. You shall observe and obey all applcable traffc laws and department regulatons whle operatng the vehcle. Drvng must be documented usng the attached sklls sheet that s to be turned n to FSTB.
