2 WELCOME North Carolina s first effort in developing medical homes for Medicaid recipients began in Wilson County in The NC Foundation for Advanced Health Programs, in partnership with the NC Division of Medical Assistance (DMA), approached the Kate B. Reynolds Charitable Trust (KBR) for help in improving the participation of primary care physicians in Medicaid and reducing inappropriate emergency room utilization. Some 1,500 Medicaid recipients enrolled in the pilot. The first year results? A 58 percent decline in emergency room use among enrollees and $300,000 in cost savings. Two years later the Foundation, DMA and KBR proposed spreading the medical home concept to other counties in North Carolina with the goal of building a statewide medical home program. As the program grew, supporters such as the Duke Endowment and Blue Cross Blue Shield of North Carolina joined the effort. Today, Community Care of North Carolina s medical home/care management model covers the entire state and has been replicated across the country. Approximately 80 percent of Medicaid recipients are enrolled in CCNC and more than 1,800 primary care practices participate in the program. CCNC is a national model for reducing health care costs, improving outcomes and expanding access to underserved populations. The organization s success is based on a quality-first orientation that physicians can stand behind. As one of CCNC s founders said early on, We ll do what s right. We ll do it well.
