2 Family Coach Mums and Kids Matter Overview of Wesley Mission Wesley Mission is a Christian organisation that has been caring for people for over 200 years, continuing the work of Jesus Christ in Word and deed. Our position descriptions and performance plans are aligned with these five key result areas. 1. Program Overview Mums and Kids Matter is an initiative funded by the Australian Government through NSW Health. It is designed to support mothers living with severe and persistent mental illness and complex needs, and their children (0-5 years), by providing specialist short-term accommodation and support services. Care is provided via in-home interventions in local communities; specialist support in short-term non-acute residential care and/or tertiary support in specialist facilities. The program is underpinned by a basic belief that retaining intact families, rather than having children placed in out of home care, is beneficial for the child, for the mother/parents, for the immediate family, and for the community, as well as being more cost effective. 3 The primary focus of the program is to address the complex needs of mothers with severe and persistent mental illness and their children (0-5 years) by providing mental health care and parenting support, whilst also catering for needs related to physical health, accommodation, vocation/education, disability and other co-morbidities. 2. Overview of team The Family Coach team is responsible for working with the Mums and Kids Matter multidisciplinary team to provide each mother in the program with expert mental health care and interventions, support activities of daily living and build on parenting and independent living skills. The team works with the mother and her child(ren), in conjunction with key stakeholders including partners, other family members, members of the Mums and Kids Matter multidisciplinary team and other relevant agencies (like Local Area Mental Health and Family and Community Services) to provide tailored mental health interventions and family-focused care. Family Coaches, deliver focussed psychological strategies and other interventions in the mother s natural setting, in line with the mother s care plan, to promote recovery and to ensure the safety and well-being of her child(ren). Family Coaches also provide psycho-education, aimed at improving each mother s ability to manage her mental health and improve family dynamics. 3. Overview of role The Family Coach is an appropriately qualified, skilled and experienced mental health professional, who understands the person centred recovery model within the context of a step down mental health service. The Family Coach will support the mother and her child(ren) to implement strategies to help her work towards achieving goals as outlined in the care plan. The Family Coach will apply relevant knowledge, skills and experience to address the complex needs of each mother and her children, as directed by the Coordinator Allied Health. The Family Coach is to support the Family Coach Team to meet its KPIs within a Quality Improvement and Governance Framework. The Family Coach will work at the residential facility and also regionally, delivering outreach services as part of a state-wide service. The Family Coach will support the Coordinator Allied Health to promote cohesion, integration and cooperation between all program staff. 4. Major role responsibilities 5.1. Our clients Utilising a person centred recovery model, be an active member of the Mums and Kids Matter s multidisciplinary team to help develop and roll-out care planning, care reviews and transition and/or exit planning meetings, as directed by the Coordinator Allied Health Work effectively with mothers, their children and families and/or carers and liaise with members of the Mums and Kids Matter multidisciplinary team staff to identify needs, develop and deliver appropriate interventions as directed by the Coordinator Allied Health Ensure interventions are tailored to meet the needs of the individual mother and her children Conduct assessments as directed by the Coordinator Allied Health Implement intervention/treatment strategies as outlined in the mother s care plan Utilising a teams based approach, apply skill and expertise to deliver focussed psychological strategies, including, but not limited to: 1. Psycho-education (including motivational interviewing) 2. Cognitive-behavioural interventions including: - behavioural interventions - exposure techniques - activity scheduling - cognitive interventions 3. Relaxation strategies including: - progressive muscle relaxation - controlled breathing 4. Skills training including: - problem solving skills and training - anger management - social skills training - communication training - stress management. Deliver other therapeutic interventions within one s scope of training and experience e.g. DBT, art therapy as outlined in the mother s care plan and as directed by your supervisor Identify any barriers to the delivery of a therapeutic intervention with the client and work with the client to overcome them Provide parenting support and teach parenting skills to promote excellent care of children in the program Promote the healthy development of children in the program Promote the development of secure attachment between the child(ren) and the mother Family Coach Manager s initials Employee s initials 4

5 Help mothers develop and/or strengthen their independent living skills including but not limited to budgeting, cooking, cleaning, effective decision making (that takes into account their child(ren) s needs) and how to access public transport with their (child)ren Support clients (with skill and expertise) as they: - Identify and develop goals - Manage their mental health - Recover their mental health - Strengthen their capacity to parent - Develop independent living skills. Support mother and the family within the natural day to day environment towards selfefficacy, utilising coaching methodology Identify learning opportunities, model behaviour and help build the mother s confidence, resilience and skills Work with the mental health nurse team to remain cognisant of the current, clinical status of clients Closely monitor, review and regularly report on the progress of the mother and her child(ren) Attend care review meetings, as directed by the Coordinator Allied Health Communicate effectively and professionally with other members of the multidisciplinary team Report client/carer needs, concerns and feedback to the Coordinator, in a timely and professional manner Ensure key assessments are completed, analyse data and continuously monitor each client s progress and the child(ren) s progress Where appropriate under section 16a of the Child and Young Persons (Care and Protection) Act 1998 and as per Wesley Mission s Information Exchange Policy gather additional information from prescribed bodies, in relation to identified Child Safety issues to support ongoing family case management Identify, mitigate and manage concerns relating to child safety, health and well-being by: a. Identifying risks and potential risks early and in association with each mother s unique circumstances b. Acting according to relevant policy and procedures, to ensure the safety and well-being of child(ren) c. Discussing concerns with the Coordinator, the Operations and Clinical Manager and/or other relevant staff as required d. Working in collaboration with Mums and Kids Matter staff, family and other stakeholders to develop safety plans to address identified risk of harm and risk of significant harm concerns e. Continuing to monitor risk f. Making reports to the NSW Department of Family and Community Services in line with mandatory reporting legislation that includes, but is not limited to, the NSW Mandatory Reporter Guide g. Making reports, as relevant, in line with Wesley Mission s policies and procedures h. Identifying early if the risk becomes too great for the mother and/or her child to live safely at Mums and Kids Matter or in the community with her child(ren). 6 Support the Coordinator Allied Health to meet the needs of all clients in the program, including those located in regional locations across NSW Discuss travel requirements in conjunction with the Coordinator Allied Health prior to making travel arrangements Travel intrastate to work with clients in their homes or to complete other Wesley Mission duties as required Invite consumer feedback and report complaints or grievances to the Coordinator Allied Health and help support remedial actions, as directed If allocated to a family, the Family Coach is to: a. Welcome the mother and her children into the program b. Introduce the mother and her children to members of the care team c. Conduct a site tour, including the mother s unit (applicable to residential clients only) d. Respond to any questions the mother may have about the program e. Encourage the mother to complete program activities, as outlined in her care plan and celebrate achievements f. Conduct assessments and undertake intervention work as directed by the Coordinator Allied Health and as outlined in the mother s care plan and record results in relevant care plans and in Carelink+ Upload relevant documentation to Carelink Performance measures If allocated to a family, complete 100% activities as outlined above in 5.1a. to 5.1g. Deliver 100% interventions as outlined in the mother s and children s individual care plan, as directed by the Coordinator Allied Health All care plans are complete and interventions are recorded appropriately and uploaded to Carelink+ 100% of care plan activities are actioned, monitored and taken back to care meetings for review Report at care review meetings any barriers identified in delivery of the interventions and utilise care reviews as an opportunity to explore possible solutions Ensure all assigned duties are completed in a timely and professional manner, by the due date Our people (our team) Assist the Coordinator Allied Health to maintain a positive service culture within Mums and Kids Matter (MaKM) that is proactive, non-judgmental and reflective of Wesley Mission s Christian values Actively and positively participate in all team, care planning and care review meetings Support other MaKM staff in their work Work effectively and positively with all members of the MaKM team and other Wesley Mission staff Positively work alongside members of the Nursing and Community Transitions and Connections Team to ensure quality service provision to mothers and their children Complete all reporting requirements in a timely manner and to a professional standard Maintain shared office space and workstation in an appropriately neat and tidy state at all times Family Coach Manager s initials Employee s initials 6

7 Assist with: a. general cleaning, as directed by the Site Administrator b. removal of rubbish and putting the site bins out c. general duties associated with maintaining the various MaKM sites Comply with all Wesley Mission policies and procedures Meet all personal and team KPIs of the Family Coach Team Support the Operations and Clinical Manager and the Coordinator Allied Health to implement Wesley Mission s strategic plan and the MaKM business plan and lead by example Exercise any other authority which may be delegated from time to time with due care and professionalism, maintaining an awareness of organisational expectations and standards at all times Performance measures Meet and/or exceed individual KPIs Meet 100% reporting requirements Active and positive participation as a member of the MAKM multidisciplinary team Active and positive participation in team meetings Positive feedback from staff, including the Coordinator Allied Health and the Operations and Clinical Manager Our stakeholders (those we serve or work with outside the organisation) Work with each mother in the program, to understand the role that key stakeholders, and the broader community, have in her life and how to work positively with these stakeholder(s) to support her recovery As outlined in the care plan and/or as directed by your supervisor, work alongside key stakeholders (such as Local Area Health, and local Family Community Services) in order to provide individualised and continuing care planning for mothers and their children Work with members of the Nursing and Community Transitions and Connections Team to identify new stakeholders and partners to support the mother and her children s recovery as they transition into community As directed by your supervisor, engage with new stakeholders to assist with the rolling out of the mother s care plan requirements Ensure the reputation and integrity of Wesley Mission is maintained at all times As directed by your supervisor, build strong and effective relationships with key stakeholders and harness their support for the Mums and Kids Matter Program Performance measures Mothers develop effective and workable relationships with 100% identified key stakeholders Positive feedback from key stakeholders Positive partnerships developed and maintained. Our systems Complete case notes and maintain contemporaneous records to a professional standard e.g. in the client information system (Carelink+) Maintain client files (both electronic and hard copy) to a high standard Assist with monitoring, reporting and evaluation activities by collecting data, obtaining outcome measures, recording outcomes as directed by your supervisor to assist with program evaluation and to monitor each mother s and her child(ren) s recovery Adhere to all relevant Wesley Mission and MAKM policies and procedures Comply with the Mums and Kids Matter risk and compliance framework and ensure that this is applied to all relevant program documentation and activities Remain abreast of new developments and technological innovations relevant to Wesley Mission s work e.g. Carelink+ Support the regular review of systems and processes to seek more efficient and effective methods of delivery and to ensure continuous improvement Actively support continuous improvement principles in the evaluation and review of work and work methods Participate in quality audit and accreditations standards and relevant industry legislation and best practice guidelines Ensure adherence to Wesley Mission s project delivery processes Be an advocate of the Wesley Mission brand, ensuring brand compliance and use of correct templates Performance measures 100% care plans are complete and interventions are recorded appropriately and uploaded to Carelink+ Zero external major non compliances in relation to individual work 100% compliance with all relevant policies and procedures 100% reports, assessments and outcome measures completed by the due date and to a professional standard Our financials Operate within the financial policies and budgets of Wesley Mission Utilise credit cards responsibly and in accordance with all relevant policies and procedures Be a responsible steward of Wesley Mission s assets including vehicles, mobile phone and internet access, protect the asset from harm or loss and keep discretionary and personal costs to a minimum Performance measures Operate within budget Promote cost-saving measures Ensure only reasonable and necessary expenditure for authorised business travel is incurred All financial processes are followed. Professional responsibilities Personally model excellence in ethical service delivery and professional standards Ensure a high level of staff morale and collegiate culture that supports professional excellence Utilise a range of complex professional clinical skills including communication, clinical reasoning, reflection and evidence based practice skills Actively promote excellence in service delivery utilising evidence based approaches Ensure services are delivered in line with the National Standards for Mental Health Services (non-government community services) Complete continuing professional development and ensure that all relevant accreditations are maintained e.g. AHPRA, AASW etc. Apply and promote a person centred recovery approach to practice and service delivery Manage all aspects of your workload including meeting and/or exceeding your individual KPIs as advised by the Coordinator Allied Health and amended from time to time Provide reports and complete project activities as requested by the Coordinator Allied Health Complete all evaluation activities which includes data collection and monthly reporting requirements against program performance measures As an employee, be responsible under the Work Health & Safety Act for the health and safety of all persons you come into contact with, during employment. Essential skills/knowledge Registered Allied Health Professional with AHPRA Demonstrated experience in the mental health sector (at least two years) Experience of working in a multidisciplinary team, including working in settings that adopt both a medical/clinical model as well as a therapeutic approach Demonstrated ability and willingness to work within a recovery oriented model, utilising a person centred approach Excellent verbal and written communication skills Thorough attention to detail Excellent time management, multi-tasking and follow-up skills Computer confidence, especially using Microsoft Office Word Be able to meet all Working with Children and security/police checks.
