5 SPECIFICATIONS FOR BID FOR PHYSICAL THERAPIST SERVICES FOR THE MERCER COUNTY CORRECTIONAL FACILITY FOR A PERIOD OF TWO (2) YEARS WITH THE OPTION TO EXTEND (2) YEARS BASED UPON THE INDEX RATE LOCATED AT 1750 RIVER ROAD LAMBERTVILLE, NEW JERSEY CONTRACT CONTACT: PATRICIA HUNDLEY: ext OR REGINA GRIMES: ext INTENT The County of Mercer requests bids for licensed physical therapists for inmate care at the Mercer County Correction Center. The services shall be performed on-site. Respondents shall perform services in accordance with the New Jersey Administrative Code Title 13 Law and Public Safety Chapter 39A State Board of Physical Therapy, N.J.A.C. 13:39A et seq. positions, standards, guidelines, policies, procedures and publications. Refer to the following link: The Contract shall be awarded for a period of two years with the option to extend for two (2) years based upon the index rate and actual services required. The number of evaluations and follow-ups are estimates for one year. The County reserves the right to increase or decrease the estimated amount. LICENSURE Services must be performed by a Physical Therapist licensed through the New Jersey Department of Law and Public Safety, Division of Consumer Affairs, State Board of Physical Therapy Examiners as required through The Physical Therapist License Act of 1983, N.J.S.A. 45: et seq. Respondents shall provide a copy of their licensure: COMPLY YES NO LICENSE NUMBER: EXPIRATION: The awarded contractor shall provide and maintain records in accordance with all local, state and federal regulations. The awarded contractor shall be responsible to provide all necessary clinical treatments and documentation for discharge in accordance with all local state and federal regulations.

6 PROCEDURE FOR PROVIDING SERVICES Medical services are provided by CFG, under the direction of Dr. James Neal, for inmate medical care. The Correction Center Medical Department will contact the awarded contractor for service as prescribed. The order for Physical Therapy originates from a clinician and physical therapy is based upon the injury and the severity of the injury. The awarded contractor shall respond within three (3) days of receipt of order from the consultation to schedule the requested services. All services shall be provided on-site within seven (7) days from the receipt of the physicians order. The awarded contractor shall provide in accordance with the physician s orders. In 2015, approximately 23 initial evaluations were provided and 490 follow-up appointments for treatment were provided. Services will be provided as necessary; therefore the numbers stated on the proposal page may increase or decrease. The awarded contractor shall provide services on-site and shall provide all equipment and supplies necessary to perform AND provide treatment in accordance with the physicians orders. In the event that specialized equipment or supplies are needed, the County will reimburse the awarded contractor at cost. Standard equipment and supplies used to perform Physical Therapy will be covered in the hourly rate. The County designee will coordinate appointments at the Correction Center.
