1 Strategic Plan Vision Centre de santé Saint-Boniface is known for its delivery of innovative primary health care services that promote the health and wellness of its community. Mission Centre de santé Saint-Boniface stimulates and supports the French-speaking population of Winnipeg and the residents of St. Boniface in taking charge of their own health and wellness. The Centre collaborates with the community to promote health and to prevent, treat and manage health problems. Given its unique role, the Centre operates in French and provides services in both official languages. Mandate The mandate of Centre de santé Saint-Boniface is to provide primary health services in both official languages to the French-speaking population of Winnipeg and St. Boniface residents through an interdisciplinary team of bilingual professionals. 4 2 EQUITABLE ACCESS Winnipeg s French-speaking population and St. Boniface residents have equitable access to health care services provided by Centre de santé Saint-Boniface. Identify the needs of priority populations, barriers to access and best practices to reach these populations Provide services and programs tailored to the priority populations Assess client needs in terms of equity in services Launch service and program projects with key partners Harm reduction (reducing negative consequences associated with drug use) and distribution of Naloxone Teen Clinic Cooking classes for newcomers Expand Project Pathway, which aims to ensure equity of access for disadvantaged populations to health care and services provided by Centre de santé Priority populations Persons living with disabilities in the community Persons living with a mental health disability and homeless persons Newcomers Young children and adolescents Seniors and persons living with a chronic illness

Improve client flow and service efficiency Make it easier to navigate French-language health care services in Winnipeg by working closely with the Winnipeg Regional Health Authority (WRHA) Master best practices in primary care and community services Adopt the advanced access method to organize, plan and schedule client flows as efficiently as possible Design and implement group medical visits Organize a community health and wellness event for a good cause

Provide a stimulating professional work environment for staff Contribute to the training and development of bilingual professionals Encourage future generations to pursue a career in health care, and provide them with guidance Establish an annual professional training plan for each discipline and each staff member Host students and interns in several fields Work with secondary schools and universities
7 ABOUT Founded in 1999, Centre de santé Saint-Boniface is the only community health centre in Manitoba that provides primary health services in both official languages. The Centre serves Winnipeg s Frenchspeaking population and St. Boniface residents.
