2 Welcome to Springbrook Nursing and Rehabilitation Center is located in Silver Spring, Maryland and provides comprehensive nursing and rehabilitation services. A Nursing and Rehabilitation Center is a place for people who do not need to be in a hospital but can no longer be cared for at home because they need personal or medical care. Springbrook is a place where everyone who lives, works, visits, or volunteers feels welcome and is a part of our family. In addition to the short term rehabilitation and long term care services, Springbrook features inpatient dialysis resident care and an on-site Hemodialysis center. The center provides dialysis for Springbrook Nursing and Rehabilitation Center residents and outpatient services for individuals in the community who require ongoing treatment for end-stage renal disease. Springbrook offers services in a convenient and comfortable setting, with a private entrance and lobby area. With services provided six days per week and three shifts per day, patients can receive the care that they need with the attention that they deserve. The Dialysis center is staffed by Certified Dialysis Technicians and Registered Dialysis Nurses. Washington Nephrology Associates provide physician medical coverage. There is a Director of Nursing, social worker, and registered dietician on site. Medicare, Medicaid, and other insurances are accepted. Cable television and internet access are at the treatment stations. There is free on-site parking and community education seminars are held on a regular basis.
3 Your Care Living Well In a Caring Community In this community, your family member will be cared for as if they were one of our own. Personalized treatment plans are developed to help each person reach the highest level of health possible. Their coordination of care is managed by our dedicated, experienced healthcare team. Highly skilled nurses and geriatric nursing assistants provide round-the-clock nursing and personal care assistance to each resident. The staff is dedicated to reaching every individual s goals, resulting in positive outcomes for your loved one. A wide array of rehabilitative activities guide the way to a greater level of independence. Each of our residents participates in a comprehensive program developed and delivered by experts in the fields of physical therapy, occupational therapy, speech language therapy, and therapeutic recreation. Medical professionals provide care when the patient needs it. Along with each resident s physician, there are podiatrists, dentists and nurse practitioners that attend to our resident s healthcare needs right here in the center. Psychiatric counseling services, mobile X-ray, laboratory services and electro-cardiogram (EKG) services are also available. Resident s dietary needs are met with three meals a day plus nutritious and delicious snacks which are planned by our dieticians. Diets can be tailored to meet specific needs as ordered by a physician. Meals are served in a common dining area, where residents can enjoy their meals and the company of others. For those who are in need of assistance, aid in personal care and hygiene is given by nursing assistants. Support with dressing, grooming and bathing is given and laundry services are available to ease the burden on family members.
4 Your Community Enriching Your Life in A Caring Community While at our community, residents will enjoy features resembling the comforts of home such as warm and inviting common areas, perfect for socializing and a pleasing décor. With the choice of either semi-private or private room arrangements residents are encouraged to personalize their rooms by adding pictures and accessories from home. The housekeeping is taken care of by our nursing center staff. Security systems, smoke alarms, sprinkler systems and round-the clock nursing care give our residents a sense of security, while away from home and family. Activities play an important part of enriching our lives. Residents have a wide range of activities available to them. Exercise classes, visits from pets, afternoon socials, and special holiday functions keep your family member feeling like a part of our family. There is even a beauty and barber shop to help residents feel their best. Religious services from a variety of denominations are offered in order to respect the spiritual aspect of each resident s life. Activities are scheduled on a daily basis to ensure your loved ones are constantly stimulated and fully engaged in their life here at Springbrook Nursing & Rehabilitation Center! 5 Selecting the right facility for your family When you or your loved one is in need of services provided by a nursing and rehabilitation center, some important factors should be considered in the decision making process. The following can serve as a guide to help you navigate through this task. Location. The type of care that you or your loved one requires may not make it practical to choose a skilled nursing facility in or close to your community. However, if circumstances permit, consider choosing a skilled nursing facility that will allow family and friends to visit often and is convenient for them. In addition, choosing facilities that are within a healthcare system allow patient access to a wide array of services that may become necessary. Level of care. Each facility defines its own level of care; not all facilities accept residents with complex medical problems. To select a facility that is best for your needs, it s important to know and accurately describe the services you or your family member will require. You should ensure that medical and dietary needs can be met by the facility. Payment sources. Investigate the facility s accepted forms of payment including private pay, health insurance including Medicare and Medicaid, commercial insurance, and long-term care insurance. It will be important to find out if the facility has both Medicare and Medicaid beds so you can switch from one to the other if needed instead of moving to another facility. Unscheduled visits. Visit the facilities that you are interested in or have your friend or family member do it for you. Make unscheduled visits at different times to observe as many interactions as possible. Pay attention to the interior and exterior of the facility. Notice if the center is clean and odor-free and whether the residents are clean, dressed and active. Examine the expressions of the residents and take notice if they seem happy and enjoy engaging with the staff. Physician affiliation. State and federal guidelines require all skilled nursing facility residents to be under the care of a physician. This is so that their care plan and medications can be evaluated every 30 days. Find out if your physician is affiliated with the facility so that he or she can continue to follow your care after you move there. Availability. You will need to find out if there is a bed available and if not, find out when one will become available. Be sure to discuss the procedures regarding the facility s waiting list. Activities. Ask about what kind of activities are provided. The types of services you may want to inquire about are religious services and social activities. Communication. Discuss the methods and frequency of communication that the facility will have with family members, if that is requested. Contract and fee schedule. You may want to ask for a copy of the nursing facility s admission or contract agreement and fee schedule to compare charges at different nursing facilities. The fee schedule does not include the cost of a required physician s visit at least every 30 days or expenses incurred for treating other illnesses. These costs represent additional expenses you are responsible for any that are not covered by your insurance.
