2 Sabbatical Report The Plan for Integrated Simulation in Nursing Diane Pestolesi Saddleback College Fall Activities a. Describe in detail the activities conducted during the sabbatical b. Explain how these activities achieved the goals stated in your application c. Provide a timeline or calendar showing when these activities were carried out The activities and timeline of the sabbatical included; Research into Core Competencies for new graduate RN s (August/ September) Education on current best practices in simulation (attended 2 conferences) (California Simulation Collaborative Level 2 Training Course Nov 5 th and 6 th, and the Laerdal Simulation Users Network Simulation Conference Dec 1 st and 2 nd ) Detailed evaluation of simulation currently utilized in the nursing program (October/November) Review of QSEN Core Competencies currently addressed in existing simulations (November) Identification of Standard Parts of Simulation in the Saddleback College Nursing Program (November) Development of a capstone simulation for the Advanced Nursing course (N176 ) including a template for further simulation development. (November/December) Piloting of the N 176 simulation (Dec 13 th ) Detailed Activities The activities of the sabbatical included; research into Core Competencies for new graduate RN s. As a result of an extensive literature review and research into profession publications for the most current and widely accepted Core Competencies for new graduate RN s, QSEN Core Competencies and the NLN Competencies for Graduates of Associate Degree Nursing Programs were reviewed in detail. The QSEN Core Competencies include: Patient Centered Care, Teamwork and Collaboration, Evidenced Based Practice, Quality Improvement, Safety and Informatics. Since these Core Competencies subsume the NLN Competencies for Graduates of Associate Degree Nursing Programs and it was noted that they are addressed as required parts of the nursing program curriculum per the Board of Registered Nursing Regulation 1426 (d) and after discussion with the Director of the Nursing Program, it was decided to integrate the QSEN core competencies into the nursing curriculum through simulation. Once the QSEN Core Competencies were decided upon, a review of the existing simulation scenarios utilized throughout the nursing program was undertaken. An overview of the QSEN Core Competencies was presented to each of the lead faculty members and more detailed literature about the competencies was provided. Together with the lead faculty of each of the core nursing courses the simulations were reviewed and the Core Competencies that were addressed within the simulation were identified. (See Core Competency Overview Table) The Standard Parts of a Simulation Experience in the Saddleback Nursing Program were identified and with input from the simulation technician and Skills/Simulation Lab faculty member a binder with each nursing courses simulations and standard parts of simulation was developed. 3 version of the binder was also developed and provided to both the simulation technician and Skills/Simulation Lab faculty member. A simulation for the advanced nursing course (N 176) was developed that integrated each of the QSEN Core Competencies. This simulation includes student preparation, a faculty lesson plan, a simulation worksheet, chart forms, role cards, a debriefing guide and a post simulation survey. This simulation and forms included in its development may be used as a template for future simulations that will be developed or revised in nursing or health science programs. The simulation that was developed for N 176 was piloted in December. As a result of the pilot, with input from the students and simulation technician involved, revisions and refinements were made in preparation for its implementation in April The Sabbatical Project was presented on January 6 th at the nursing faculty meeting. The binder and electronic references were provided to the simulation technician and Skills/Simulation Lab faculty member and the appropriate simulation information was posted on the N 176 Course Blackboard site. Goal This project achieved the goals stated in the sabbatical application. This project reviewed simulation used in the nursing program course by course. This project identified the most appropriate competencies for RN new graduates to be the QSEN Core Competencies. Inherent in the QSEN Core Competencies is the use of teaching strategies that include; patient centered care, teamwork and collaboration, evidence based practice, quality improvement, safety and informatics. These competencies were then identified in the simulation all ready in place in the nursing program. The QSEN Core Competencies were identified as a key component of the learning outcomes for simulation in the nursing program and served as a organizing framework for simulation in the nursing program. A capstone simulation was developed for the final nursing course in the program, N 176 Advanced Nursing. This simulation integrated each of the QSEN core competencies and utilized a standardized approach to simulation that was introduced by the Skills simulation Lab faculty this semester. Documents and policies that reflect best practices in simulation were developed for the N 176 simulation that can be used as templates for the revision and or further development of simulation for nursing or other health science courses. The simulation for N 176 nursing was also developed so that it can be expanded from the acute critical care environment to the outpatient environment and to the medical surgical environment so that other disciplines in the Health Sciences Division (Like EMT, Medical Assisting and Medical Surgical Nursing) can become part of the simulation experience. d. Impact of these activities The Plan for Integrating Simulation in the Nursing Program will have a significant impact on teaching and learning in the N 176 Advanced Nursing course and the Saddleback College Nursing program. This sabbatical project identified core competencies for new graduate RNs that when integrated into a nursing program curriculum will keep the curriculum current, evidenced based and relevant. It has identified how the core competencies are currently being addressed in simulation and in the curriculum and made recommendations to enhance these competencies in throughout the curriculum. The N 176 simulation that was developed incorporated all of the QSEN Core competencies and served as a template for revision or development of future simulation experiences in the nursing program and potentially including other disciplines such as medical assisting, emergency medical technician, phlebotomy and paramedics. This sabbatical project will impact teaching by further developing simulation and standardizing the approach to simulation in our nursing program. This will make it easier for faculty to revise or write simulations with targeted competencies or student learning outcomes. This project will impact learning by providing students with a standard approach to simulation so they will know what to expect from simulation and how to prepare in order to meet the learning outcomes and competencies. This project will provide students with simulated patient care experiences that are seen as essential to develop core competencies by the time they graduate from the Saddleback College Nursing Program. By integrating the QSEN core competencies into simulation in our curriculum the program addresses curricular requirements for accreditation by the Board of registered Nursing. This sabbatical project impacted my professional development by the research that was done on competencies and curriculum, and through the variety of conferences and courses that I took related to simulation and best practices. Over this past semester my knowledge in these areas has grown dramatically to the point where I have been able to write/revise a simulation to include QSEN core competencies, evaluate existing simulation experiences and expand upon efforts in the nursing program to develop a standardized approach to simulation. The benefit of these activities to students is that the students will have a current and relevant curriculum. They will participate in simulation through the nursing program and especially in the advanced Nursing Course that makes it possible for them to meet the QSEN Core Competencies for New Graduate RNs. Current research on simulation reports that students who are exposed to simulated patient care experiences are more able to apply theory to actual patient care. 3. Dissemination of results The Plan for Integrated Simulation in Nursing sabbatical project was disseminated to the nursing director and faculty at the January 6 th faculty meeting. (see PowerPoint attached) The simulation that was developed for the Advanced Nursing N 176 course will be utilized February 16 th and 22 nd The products developed or revised in the completion of this sabbatical will be available to any nursing or health Science faculty upon request.
