3 Introduction The Doncaster Older People s Community Mental Health Service offers a wide range of services based on the St. Catherine s site. We provide assessment and treatment to older people aged 65 and over with mental health problems and under 65 with a diagnosis of dementia. All services work on a Monday to Friday basis between the hours of 8.30am and 5.00pm but there is also an emergency service. All referrals come from a GP who will contact the Community Mental Health Team (CMHT) via fax to the single point of access Tel: Out of hours: The CMHT will screen the referral and decide whether a member of their team should visit you or whether another team would be more appropriate. Community Mental Health Team Cherry Tree Court The CMHT offers advice and support to older people living in the community with mental health problems. Once referred to the service a Community Mental Health Nurse, Social Worker or an Occupational Therapist (OT) may see you. Community Mental Health Nurse A Nurse may visit you at home and assess your mental health. They will then co-ordinate care and treatment. Occupational Therapist OT s look at all the aspects of daily life a person may be having difficulty with, from self-care, to cooking, to accessing meaningful activities. Social Worker Social workers form the link between social services and the NHS. A social worker may visit you at home to help sort out problems with care, health, housing or benefits 1
4 Carers Support Group Forest Gate Day Hospital There is a carer support group run on the 1st and 3rd Wednesday of every month for those with dementia. Memory Clinic Forest Gate Day Hospital Memory Clinic staff will complete an assessment of your memory and monitor this if required. Forest Gate Forest Gate Day Hospital Forest Gate is open two days a week for dementia patients and once weekly for patients with mental Illness. The Staff here provide ongoing assessment and treatment. Community Memory Therapy Services Catherine s Close The Community Memory Therapy Service provides therapeutic day care and ongoing carer support for those supporting people with dementia. Young Onset Dementia Service Forest Gate Day Hospital The Young Onset Service will provide support and assessment for those adults who were diagnosed with dementia while under the age of 65. Liaison Service Tickhill Road Hospital Fax for referrals The Liaison Service provides advice, assessment and support to people in both residential care and general hospital care. Emergency Contacts Emergency Social Services Can be contacted to deal with any social care emergencies outside normal office hours or at weekends. 2 Doncaster services
5 Crisis Resolution and Home Treatment Team This is a 24 hr service and provides mental health assessment and treatment on an emergency basis. Information directory Benefits Enquiry Line There is a wide range of financial support available. Monday-Friday 8am-6pm. FirstStop: Advice for Older People Contact FirstStop for housing advice, care advice, finance advice and rights advice. Social Services- Adult Contact Team Contact the duty team to make a referral. They may arrange for a social worker or Occupational Therapist to complete an assessment of your needs. Services available include home care and equipment/ adaptations. Leger Bus Service The Leger bus is wheelchair accessible transport that will pick you up from home. Alzheimer s Society National Dementia Help line Mon-Fri Work to improve the quality of life of people affected by dementia. They provide carer support, a befriending service and a memory café. The website contains useful factsheets providing information, advice and support to people with all forms of dementia, not just Alzheimer s. MIND Aim to improve the quality of life for people affected by any mental illness. 3
6 Age UK Carers helpline Advice hotline The UK s largest charity that works with older people. The Doncaster branch provides home care services, carer training, daycare services and cleaning services. Citizens Advice Bureau The citizens advice service has offices in Doncaster, Mexborough and Thorne. They help people resolve their legal, money and other problems by providing free information and advice. Carers Direct These services are designed for carers wanting information and advice about services and support. DIAL DIAL provides information, advice and support to people with any disability. Doncaster Partnership for Carers Doncaster partnership for carers provides a service, which supports carers in their caring role. NHS Direct 111 Service NHS Direct is a 24hours a day, 7 days a week service to offer advice and support regarding medical issues. PALS- Patient Advice and Liaison Service As a patient, relative or carer sometimes you may need to turn to someone for help, advice and support. This is where the PALS come in. South Yorkshire Centre for Inclusive Living (SYCIL) The independent living unit provides impartial advice about equipment and practical aspects of daily living for disabled people. They also have an advocacy service who will help you fill in forms etc. 4 Doncaster services
