Advancement of Local Healthcare Solutions, with focus on: Working with all of the HIP partners as a neutral facilitator to find opportunities to further the collaborative efforts in the County; and Researching successful healthcare solutions in other communities and accessing technical assistance wherever possible. 2. Support for Community Partners, with focus on: Providing support to the Health Improvement Partnership Council (HIPC), the Safety Net Clinic Coalition (SNC) and the Health Improvement Partnership Board of Directors (HIP) and the committees of the HIPC, the SNC and the Board of Directors, with objectives focusing on but not limited to agenda planning with the Executive Committee of the Board, preparation of materials for reporting and discussion, and background work to prepare individuals with agenda item responsibility; 1. Supervising staff who will convene work groups and committees needed to support the projects identified by the HIPC, SNC and the HIP Board, including facilitation of meetings, agenda preparation, minutes, and timelines; and 2. Planning and staffing an annual HIP Board of Directors meeting. 3. Grant Administration and Fundraising, with focus on supervising staff who will Identify funding opportunities to support the work of the HIP, convene appropriate partners to apply for funding, collaborate with grant writers to complete grant applications gather necessary data, letters of support, etc.; Work with appropriate partners to submit basic grant applications to potential funders; Manage the requirements of grants awarded to the HIP including interim reports, budget reconciliation, and final reports; Ensure appropriate grant implementation; and Attending funder conferences, grant meetings, etc. 4. Program Evaluation, Reporting and Operations, with focus on: Creating an annual process to evaluate the effectiveness of the collaborative and report the findings to the partners making recommendations as appropriate; Developing necessary policies and procedures to document the business requirements of HIP and assure the partners approve and comply with those policies. Negotiating contracts with and supervising independent contractors as needed to implement the work of the HIP or the requirements of a grant Assuring timely filing of annual tax forms; and Keeping appropriate records of HIP activities. 5. Community Relations, with focus on: Representing the HIP in community meetings, forums, with the press, and at Partner organization events. 6. Medi-Cal Administrative Activities, with focus on: Prepares reports and needs assessments to develop strategies to increase Medi-Cal capacity and close Medi-Cal service gaps. (F) Collaborates with others and prepares proposals for expansion and enhancement of health and Medi-Cal services. (F) Assists to implement and oversee Medi-Cal Administrative Activities claiming process. (G)

3 NOT USED Health Improvement Partnership April 2009 Administrative Assistant 1. Word process/copy/print a variety of materials such as minutes, reports, memoranda, invoices, correspondence, flyers, bulletins, etc. using MS-Office (Word, Excel, Publisher). 2. Coordinate Meetings / Trainings / Special Events: Planning, Scheduling and Preparation coordinate food, facility, equipment rentals needed, etc.; Maintain registration, mailing lists and other records; Prepare meeting packets; Take minutes at Safety Net Clinic Coalition meetings and other meetings as requested 3. Track HIP contracts and expenses; prepare check requests, payroll forms and other information required by United Way; reconcile invoices with contracts and expenses; maintain project tracking and audit binders as requested. 4. Develop and maintain HIP master files in coordination with United Way requirements, including financial and legal documents, reports and reference materials. 5. Collect data and prepare funder and grant reports for Healthy Kids and other HIP programs as requested. 6. Create and send e-notices for HIP programs, meetings and events using Constant Contact when appropriate. Manage and track replies and e-board votes. Maintain contact database. 7. Maintain inventory and ordering of supplies and program materials. 8. Serve as liaison with Central Coast Alliance for Health and United Way of Santa Cruz County regarding office procedures, supplies and technical support. 9. Gather relevant media and maintain internal archives. 10. Orient student interns and other administrative support staff to office procedures.

Participate with HIP Health Navigator program team members and staff associated with partnering agencies, including hospitals and safety net clinics in the planning and coordination of the project. (Medi-Cal related planning F) 2. Work with Project Connect Team to design a Health Navigator program that coordinates with the frequent users of emergency department program. 3. Provide input to the design, testing and improvements for the health navigator project model of services. (Medi-Cal related planning F) 4. Meet with family members to access, clarify and plan follow-up related to information they are provided at discharge, including treatment options (Medi-Cal related referral (A, B), diagnoses, medications, and recommended follow-up care and also self-care management, prevention practices and recommended behavior change. 5. Assess need for, and provide support related to client motivational factors involved in implementing hospital discharge plan 6. Serve as a reliable guide and ally for patients and family members to lean on for advice, support and direction as they navigate from inpatient hospital care to primary care medical homes, access medications and implement discharge plans 7. Provide input to final data collection protocols; collect data and document client services per protocols 8. Provide input to program evaluation design, assist in implementing aspects of program evaluation as identified, including conducting interviews with project participants, etc. 9. Provide information to high risk, high need HIP clients to provide information about services offered by Medi-Cal, and directs clients to application and eligibility staff for eligibility determination. Refers Medi-Cal eligible individuals and families directly to provider services. (A, B) 10. Assist individuals and families with aspects of the Medi-Cal and Healthy Families/Medi-Cal for Children application process. (C) 11. Work with community and government agencies to identify and fill gaps in health and Medi-Cal services by collaborating and planning for clients and families in need of such services. (F)

Lead and coordinate a decentralized team consisting of a nurse, mental health specialist, community health workers, and a consulting physician 2. Organize and lead weekly Health Navigator team huddles 3. Provide direct client health navigation services, as needed. Including: a. Informing about Medi-Cal program services and linking potentially eligible families to Medi-Cal or Healthy Families and/or linking Medi-Cal enrollees to Medi-Cal covered services (A, B) b. Linking clients or families and/or high risk groups with identified health needs to Medi-Cal covered services to meet their needs (A, B) 4. Interface and work collaboratively with partner agency representatives to enhance the delivery of health and Medi-Cal services. (Medi-Cal related planning - F) 5.
