4 2018 Summer Quarterly Education Conference TOPIC DESCRIPTIONS Why Do I Need the Quality Initiative and LTC Trend Tracker? Today s families and seniors shopping for long term care services expect the delivery of quality care. Any provider can claim that they deliver quality services to their customers, but do you have quality improvement systems and data tracking in place to prove that you do? This session will help you understand the recently retooled Quality Initiative goals and how to track and trend important operational data. Bring your laptop computer so you can log into LTC Trend Tracker to see how you can bench mark your program against others in Iowa and across the nation on quality measures. Customer Service is Worthless Customer Loyalty is Priceless In healthcare, we re in the relationship business and the product we re selling is our service. It s every healthcare consumer s right to receive safe, quality services that are delivered by competent professionals. The true key to success is establishing a formal customer service program that consists of standards and expectations that all employees must follow. This session will show you how to shift your staffs paradigm on how they see and serve your customers. Assisted Living Dietary Services Regulations Review When food service is provided by an Assisted Living facility, the administrator, or an individual designated in writing by the administrator, must be responsible for total food services and the day-to-day supervision of food services staff. This session will review assisted living dietary standards, food hygiene, food storage/temperatures, contracted food services, staff training, documentation requirements, and what to expect from DIAs health food inspections. Home Health Financial Intensive The financial landscape in home health care is ever changing, so it s important to stay informed about all the financial changes that impact home health care agencies. This session will identify some of the key drivers of financial performance in Home Health. Explore how changes in performance today impact future financial viability. Discussion will focus on federal impacts anticipated to home health finances and innovations in agency operations to improve cost efficiency and quality of care. Respecting Different Generations in Healthcare Settings Radio Babies, Baby Boomers, Gen X ers, Millennials We ve all heard the terms, and chances are that your healthcare setting contains a sampling of each of these generations working side by side. This engaging, interactive program is designed to increase understanding of the different points of view that each generation brings to the workplace, based upon their shared life experiences. Participants will leave armed with practical strategies for respecting generational differences and maximizing the benefits of working on, or leading, a multi-generational healthcare team. Potholes of Life: 8- Leadership Habits to Break Now You ve heard it before: people don t leave companies, they leave bad managers. Research has shown that there are several key leadership behaviors that managers can learn and develop. These behaviors are grounded in managing our social-emotional reactions to our surroundings, and by cultivating them, leaders can build a team around them that is accountable, trusting, resilient, proactive, and passionate about their work and company mission. This session will provide insight on the behaviors and habits that leaders should have and should break so they can become more successful in their role as a leader. Navigating and Making Sense of so Many SNF Quality Measures The CMS Quality Measures are tools that help quantify healthcare processes, outcomes, patient perceptions, and organizational systems that are associated with the ability to provide high-quality health care. These goals include: effective, safe, efficient, patient-centered, equitable, and timely care. This session will discuss the differences between the quality metrics tracked by various public agencies, how referral sources and consumers view these quality scores, and how these metrics have a revenue impact on providers. MCO Best Practices for Home Health Care Home Health Agencies continue struggling with the transition to managed care. This session will focus on the prior authorization submission and approval process, clean claim billing and Accounts Receivable management, as well as other miscellaneous MCO related topics. Participants are encouraged to come with questions and will be encouraged to share their best practices.
5 2018 Summer Quarterly Education Conference TOPIC DESCRIPTIONS Holding Accountable the Difficult or Underperforming Employee How do you, as a healthcare supervisor, hold accountable an employee who is performing below standards and/or who is just plain difficult to work with? We will start this meaty, practical program with strategies for setting clear expectations for team members, and for giving regular, ongoing performance feedback. We ll then work into a step-by-step framework for having a corrective feedback conference and for following through when an employee is not performing up to those standards. Using Data to Understand the Iowa Skilled Nursing Landscape This session will detail the operating environment of Iowa skilled nursing providers, both in terms of financial performance and quality metrics. By recognizing how your business compares to other Iowa facilities, providers can pinpoint gaps in financial performance and identify specific areas of improvement. This session is intended to move beyond the numbers to explore improvement opportunities by interpreting the right data to guide business decisions. Effective Strategies for Thriving in a Rural Healthcare Market Rural skilled nursing facilities (SNF) face challenges that are unique or exacerbated by their size and location. These include census, labor, and expense management. This session will focus on specific solutions for rural SNFs, including new strategies that innovative providers are implementing to address these challenges. There will also be a discussion on value-proposition development, value-based purchasing involvement, and guidance on developing formal partnership with other community care providers. Improving Quality in Home Health Care This session will help you rev up your quality improvement initiatives, including how to identify data sources to monitor trends and determine where improvements are needed. You will learn how to involve staff and to use current activities to meet Quality Assurance and Performance Improvement (QAPI) requirements. QI tools and resources will be identified. Lastly, we will talk about involving patients and families in your improvement efforts to enhance the care experience.. Self-Care for the Caregiver Almost all caregivers have times when their personal battery is running on a dangerously low charge. They might feel physically drained, emotionally exhausted, overwhelmed, or just plain burnt out. This upbeat, practical program is about ways to care for yourself and recharge your battery--so that you have the energy, patience, and strength to do your extremely important caregiving work every day. You will leave armed with several take-home strategies for rejuvenating yourself, so that you regain your can-do spirit and those you care for are able to benefit from the best you have to offer. A.L. Service Plans and 90 Day Reviews The core of assisted living care is individual support. Developing an assisted living tenant s plan of care is a crucial part of meeting an individual s interests, needs and desires. This session will focus on best practices for customizing care plans and will include a discussion on the 90-day review process of the care plan to insure you are meeting all the needs of each assisted living tenant you serve. Top Trends in Long-Term and Post-Acute Care This session will outline the top trends in post-acute and senior care and their impact. Top trends to be discussed include: continued investment and ownership changes, a renewed focus on rewarding value over volume, rural health challenges, rapid development of senior living properties, declines in occupancy and length of stay, increase in government oversight, increases in managed care, medicalization of housing models and the expansion of acute and postacute partnerships. CGS Billing and Face-to-Face Updates for Home Health Care Sandy Decker from CGS (the Medicare Administrative Contactor) will address recent and future changes in Medicare that have or will impact processing and payment of Medicare claims to home health agencies. We will also discuss common billing errors and resources for preventing and resolving these mistakes. There will also be a discussion on the requirements for successful face-to-face encounters and documentation to support the need for skilled care for both nurses and therapists. Attendees will be provided with information and resources to complete documentation accurately.
