Please pay attention to the instruction given below: 1. Reporting to AIIMS, Patna will be decided on the day of Counselling. Student will have to report for Admission at the following address: Administrative Block, All India Institute of Medical Sciences, Phulwarisharif, Patna , 2. Medical Examination to be held at AIIMS Patna by a medical board. If you are found medically unfit, then you will not be admitted to the course. The decision of Director/Dean/Medical Board shall be final. 3. Institute will not reimburse any expenditure incurred by you on account of travel and maintenance for joining the Institute. 4. Those who fail to report on stipulated date, their seat will be considered cancelled. 5. You are required to bring an anti-ragging affidavit on a non judicial stamp paper of Rs. 10/- Format of the same is available on Page No. 6 & 7 as annexure I and II. 6. You are required to furnish the undertaking of authenticity of documents at the time of joining the course as in proforma on Page No. 8 as annexure III. 7. Following original documents are to be submitted at AIIMS Patna at the time of joining which will be returned after completion of the course : a or equivalent examination passing certificate. b. Date of Birth Certificate: The certificate from the board which you passed the high school/higher secondary examination showing your date of birth. c. Mark sheet of or equivalent examination. d. Admit Card & Counselling Call Letter same as produced at the time of counselling. e. Caste Certificate on the prescribed form as appended in the prospectus showing that you belong to Scheduled Caste/Tribe/Orthopaedic Physically Handicapped Category. f. Declaration of the candidate along with Caste Certificate (OBC Non Creamy Layer). Applicable only if you have claimed in your applications that you belong to OBC Non Creamy Layer available on Page No 8 as Annexure-IV. g. Current passport size photograph (Both ears should be visible) 10 copies. h. Original allotment letter issued from AIIMS New Delhi.

3 10. Admission fees is to be deposited at AIIMS, Patna at the time of joining. 11. Hostel -It is mandatory for the students to stay in the hostel during the tenure of the course. The institute shall strive to provide accommodation on the day of joining. The students are advised to bring/arrange following useful items at their own to ensure a comfortable stay: a. Utensils like Plastic Bucket, Mug, Umbrella etc. b. Two locks one for the main door and other for Almirah. c. Blanket, bed sheet, pillow etc. 12. Messa. Mess facilities are available in the hostel. b. The Mess charge for the whole year will have to pay in full at the time of admission. c. Mess charge is forty thousand (40,000) for which Demand Draft to be submitted at the time of joining.
