1 Mental Health Physical Review Template NHS England has stated that people living with severe mental illness (SMI) face one of the greatest health inequality gaps in England. This population group are at risk of dying on average up to two decades earlier than the general population. This disparity in health outcomes is partly due to physical health needs being overlooked, both in terms of identification and in receiving appropriate treatment. People are not being offered appropriate or timely screenings despite their higher risk of poor physical health. There is also a low up take of information, tests and interventions relating to preventable health conditions such as physical activity, smoking, alcohol, obesity, diabetes, heart disease and cancer. This is important given that: The majority of people with SMI will have their overall health needs met solely within primary care. The responsibility for monitoring and responding to physical health needs of people under the care of secondary mental health services reverts back to primary care. Primary care teams are responsible for carrying out annual physical health checks and follow up care for: Patients with SMI who are not in contact with secondary care services, including both those whose care has always been solely in primary care and those who have been discharged from secondary care back to primary care. Patients with SMI who are in contact with secondary care mental health teams, but have been so for more than 12 months and / or whose condition has stabilised. Primary care is well positioned to play a central role in the provision of care for people diagnosed with SMI. Primary care services can manage both physical and mental health needs holistically as well as addressing wider related health determinants which are so often overlooked. NHS England has recommended the use of a data entry template to assist GPs and their teams to perform the annual physical health checks so that they are prompted to do a comprehensive review and can easily record their findings. Clinicians in Bradford, West Yorkshire, have developed such a template in SystmOne and have tested it across their locality. NHS England have encouraged TPP to make the Bradford template available to all SystmOne users.

2 Instructions for template use To use this new template in SystmOne you can do one or more of the following: 1. Add the new template to the clinical tree. Ask your system administrator to do this by following these steps Setup > Users & Policy > Organisation Preferences. Open the clinical policy folder on the left and select tree configuration. Amend the required tree and add the template to the desired position on the tree. 2. Add the new template to the toolbar. Ask your system administrator to do this by following these steps Setup > Users & Policy > Organisation Preferences. Select toolbar on the left, select amend and then add the new template to the desired position on the toolbar. 3. Add the template to the Launcher (F12). Press F12 and then search for the new template. This can also be dragged to the left to save as a favourite.
