6 Statement of need: As navigates healthcare change, state of the art medical care is being provided on Long Island. This care is being enhanced and redefined by its innovative physicians and scientists to ensure that our patients and communities have access this new level of care. Cardiovascular disease continues to be the leading cause of death among Americans and over 1,800 Long Islanders die from heart attacks and 1,000 will die from stroke annually. Structural heart disease, including heart failure and valvular heart disease remain areas of tremendous growth in cardiovascular disease opportunities for complex intervention. This conference will focus on collaboration and team approaches to these diseases. Lecture and panel discussions will engage attendees in a review of current and best practices with opportunities to choose topics in the advanced clinical track, the basic bedside track and the new addition of healthcare delivery. The focus on comprehensive and innovative care will be the common theme. New healthcare strategies of will be defined. Program objectives: All CME activities are intended to improve patient care, safety and outcomes. At the conclusion of this conference participants should be able to: Utilize advanced imaging for the diagnosis of valvular heart and aortic disease and determine if/when surgical intervention of valve replacement is appropriate. Summarize the benefits of a collaborative team-based approach in the evaluation and treatment of cardiac patients. Apply new techniques and therapies for the early detection of cardiac risk and prevention of coronary artery disease. Establish strategies to navigate the leading healthcare disruptors as delivery moves towards value based care models and health care consumerism. Incorporate the American Diabetes Association s current guidelines on diabetes management, the American Heart Association guidelines on lipid therapy, and the Heart Rhythm Society's guideline for management of patients with atrial fibrillation in the evaluation and treatment of patients with valvular heart and aortic disease. Target audience: MDs, PAs, NPs, RNs, and case managers within the specialties of cardiology, internal medicine, family practice, emergency medicine as well as healthcare administrators and practice managers. CME accreditation: is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. Credit Designation: designates this live educational activity for a maximum of 6.5 AMA PRA Category 1 Credits. Physicians should only claim credit commensurate with the extent of their participation in the activity. MOC statement: Successful completion of this CME activity enables the participant to earn up to 6.5 MOC points in the American Board of Internal Medicine s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity.
