2 Introduction This brief overview will introduce you to: Personal Experience Outcomes The PEONIES Project Ways that PEONIES can be used to improve care planning and quality of life

3 What are Personal Experience Outcomes? Personal experience outcomes are the dreams, desires, hopes, wishes, goals and preferences that each person has for him/herself. Achievement of these personally desired outcomes are the essence of quality of life. At any point in time, most people will have some outcomes that they have achieved and want to maintain, and some that they are working toward.

4 Personal Experience Outcome Areas The personal experience outcomes used in the Wisconsin Medicaid program are organized into 12 areas. Each area addresses a specific aspect of quality of life which extensive research has shown is important to people. Within each of these areas, people define their own desired outcomes.

5 Personal Experience Outcomes Areas CHOICE I decide where and with whom I live. I make decisions regarding my supports and services. I decide how I spend my day. HEALTH AND SAFETY I have the best possible health. I feel safe. I am free from abuse and neglect. PERSONAL EXPERIENCE I have relationships with family and friends I care about. I do things that are important to me. I am involved in my community. My life is stable. I am respected and treated fairly. I have privacy.

6 What is the PEONIES Project? PEONIES stands for Personal Experience Outcomes integrated Interview and Evaluation System. PEONIES was developed by researchers at the University of Wisconsin Madison s Center for Health Systems Research and Analysis and the School of Nursing, with extensive input from professional and consumer stakeholders. PEONIES was developed to use personal experience outcomes as the foundation of care planning, quality improvement, and quality assurance activities

7 Overview of the PEONIES Approach PEONIES uses semi-structured interviews to identify: the outcomes desired by the individual in each area; whether those outcomes currently are achieved and if not, why not; the types of supports need to help the person maintain or achieve each outcome; and the status of those necessary supports. In most cases, information is obtained directly from the individual. When necessary, information is obtained from proxies and confirmed with the individual to the extent possible.

8 Unique Aspects of the PEONIES Approach outcomes are defined by the individual a single approach to measuring quality that can be used across populations and across long-term care programs a common approach for quality review and care planning facilitates individualized approaches to care planning and a standardized, reliable way to measure quality at the aggregate level an outcomes-driven system leading to continual quality improvement

9 Application of Information Information from the interviews can be used to: Develop cost-effective care plans Improve quality of life Measure organizational quality Compare quality across units within an organization and across different organizations PEONIES is designed for use by care managers and also by external quality reviewers.

10 Available PEONIES Tools PEONIES materials include interviewing tools, data collection tools, measurement tools, a manual, and training materials. Tools are designed for use with all of the target population groups adults with physical, intellectual, or developmental disabilities and elders with chronic illnesses or disabilities. Additional supporting materials are available to help understand how issues can vary by target group and living arrangement, and by the purpose of the interview.

11 Using PEONIES for Care Planning Care managers can use PEONIES to learn about the outcomes that are important to consumers, and to structure discussion of how to best support those outcomes in costeffective ways. The PEONIES tools explicitly recognize the value of natural supports and other community supports, in addition to supports available from care organizations, and the importance of integrating these supports to maximize quality of life.

12 Using PEONIES for Quality Improvement Protocols have been developed to use information from individual interviews to create measures of quality. Within each Personal Experience Area, we measure the percent of individuals with an identified outcome, the percent of outcomes that currently are achieved, and the percent of outcomes that are fully supported. This information can be used to identify areas for review and the potential for quality improvement.
