Description of the Service we provide and Client groups who may access it: Community Mental Health Service for patients from 18 years upwards who have an enduring functional mental illness such as Severe depression Treatment resistant depression Schizophrenia Bi-polar disorder Borderline Personality Disorders Severe Anxiety states Eating disorders Phobic disorders Post traumatic stress disorder The team is made up of doctors, nurses, social workers and occupational therapists. The community Mental Health Service provides a range of therapeutic treatments including psychotropic treatments, group work, systemic work, CBT (Cognitive Behavioural Therapy) CAT work (Cognitive Analytical Therapy). Access to service is through GP s, other professional organisations and some self-referrals (in crisis). 5 Self-Harm Liaison Service Island Drug & Alcohol Services (IDAS) Inpatient Services including Crisis Home Treatment Team Rehab and Recovery Team Surestart Local Authority CAMHS (Child Adolescent Mental Health Services) AESOP (Early Intervention into Psychosis) Day Services Voluntary Organisations Employment Advisors (currently in the team and this role has good links with external agencies) IAPT (Improving Access to Psychological Therapies) Andrew to discuss with Stacey re: increasing involvement for students LEARNER INFORMATION Work Pattern 9 5pm Monday to Friday with one late clinic on a Tuesday evening until 7pm (eg Shift patterns, start & finish times) Dress / Uniform Code Dress code policy applies Staff room / Rest Room facilities Coffee lounge Library / IT Resources Access to the internet via trust computer system. This is done on a rota basis. Patients are allocated daily to practitioners in the team and treatment is commenced with regular reviews as directed by the care record and CPA Policy document and HoNOS, PbR clustering. The principle to involve service users and carers in W.R.A.P. is used as part of the patients treatment plan and relapse prevention. Cases are discussed at weekly clinical meetings, regular supervision and caseload management supervision is implemented to support the practitioners in delivering the appropriate and consistent treatment for the patient. Tools currently used to measure outcomes - PHQ-9 GAD-7 CMHT Phobic Scales HoNOS Personality Disorder Severe Anxiety Depression Phobic Disorders Social Anxiety Bi-Polar Disorder Schizo-Affective Disorder Anti-Social Personality Disorder Managing/treatment of complex cases Role of the Duty Team and its links with other parts of the service and other agencies CBT with the implementation of PbR, time limited DBT as above Motivational Interviewing Psycho-Social Interventions Care Co-ordination Mental Health Act Assessments Urgent Assessments Psychiatric Outpatient Appointments Psycho-dynamic group work Self-esteem and Anxiety Workshops Liaising with Children s Services Liaising with Employment Advisors As above Links with the In-patient services and mainland services that are needed for some of our patients who are likely to be subject to sections 37/41 of the Mental Health Act 1983 Page 6 of 7
