4 Within NHS Tayside and based on the western edge of Dundee, Ninewells Hospital is one of the largest teaching hospitals in Europe. 7 1. JOB IDENTIFICATION Job Title Community Support Worker (Mental Health) Department(s)/Locati P & K Community Mental Health Teams on Number of Job 3 Holders 2. JOB PURPOSE As part of a multidisciplinary team the post holder will work with registered nurses to plan and deliver a flexible patient centred programme of therapeutic activities for individuals/groups of patients. This may occur within the patient s own environment or in a wide range of community settings. To undertake clinical procedures to service users with severe and /or enduring mental health problems under the direction of a registered nurse. 3. ORGANISATIONAL POSITION 4. SCOPE AND RANGE SENIOR CHARGE NURSE CHARGE NURSE COMMUNITY MENTAL HEALTH NURSE THIS POST The post holder will work as part of the Community Mental Health Nursing Team contributing to patient care as delegated by senior staff. The Nursing Team are a key component of the Perth and Kinross adult multidisciplinary Community Mental Health Teams delivering interventions to people experiencing moderate to severe mental health problems. The post holder will deliver a structured programme of activity aimed to promote recovery. The post holder will utilise specific skills e.g. cooking, group work, sports, to ensure the effective delivery of all therapeutic treatment. Patients may be informal or being offered treatment within the parameters of the Mental Health Act. The post holder will undertake clinical procedures as directed by a registered nurse for service users of a Community Mental Health Team. 5. MAIN DUTIES/RESPONSIBILITIES Induction Standards & Code of Conduct Your performance must comply with the national Mandatory Induction Standards for Healthcare Support Workers 2009 and with the Code of Conduct for Healthcare Support Workers. Under the direct or indirect supervision of a registered nurse: 7 of 18
8 To contribute to the assessment of service users needs and problems and to the development, monitoring and evaluation of treatment plans with the support and supervision of a registered nurse. To provide support and care for people with severe and/or enduring mental health problems in their homes and in the wider community. To work with and encourage service users to engage in day treatments and activity opportunities identified as part of a plan of care. To advise service users and carers as to how to access appropriate information, advice and emotional support. To enable service users to maintain social contact and avoid isolation. To Lone Work in the community setting for most or all of the day. To assist, encourage and empower service users to achieve an agreed level of functioning in relation to activities of daily living. To participate in and facilitate group work activities as specified in a plan of care. To be involved with the discharge and the discharge planning of persons about to leave hospital and engage with the community psychiatric services. Co-operate with and maintain good relationships with key workers, professionally trained staff, and external agencies involved in supporting service users and under the direct or indirect supervision of a registered nurse to make referrals to other agencies. To contribute to the ongoing monitoring of care plans by keeping appropriate records as directed and supervised by registered nursing staff. To contribute to patient reviews and Care Programme Approach discussions. Maintain good relationships and an empathic approach to service users, carers and relatives and refer them to a registered nurse for any questions they may have on a service user s condition or for any suggestions or complaints that they wish to raise. Report to a registered nurse any observed changes in a service user s ongoing physical/psychological needs and participate in maintaining accurate and up-to-date records in relation to such observations as directed and supervised by a registered nurse. To provide information as required that is needed for the monitoring and evaluation of the service, including that required under the Clinical Governance agenda. To work within NHS Tayside policies and procedures to ensure maintenance of safe working practices for patients and colleagues. Adhere to the procedures for the use of supplies and equipment in order to promote the effective and efficient use of resources. Participate in personal/career development planning to maintain skills and personally develop in order to maximise service delivery in an annual review of performance. 8 of 18
9 To be aware of health and safety issues in relation to the work undertaken. To participate in CMHT team meetings and to develop and maintain positive working relationships with other team members. To participate in the ongoing regular management supervision of the work undertaken. As necessary assist patients to complete benefit and housing applications. With the sanction of the Community Mental Health Team make referrals to other agencies and organisations, which provide a service to people who are experiencing severe and/or enduring mental health problems. Acknowledge the diversity of individuals and be respectful of person s rights, privacy and need for confidentiality. To undertake clinical procedures relating to the monitoring of medications and to participate in training relating to these procedures (e.g. Venepuncture, ECG, Temperature, Pulse, Respiration, and Blood Pressure). 9 of 18
10 6. COMMUNICATIONS AND RELATIONSHIPS May be in receipt of highly sensitive information from some users, other relatives and carers requiring empathy and listening skills. Need to develop positive relationships with service users with severe and complex mental health problems, their relatives and carers even when faced with challenging behaviour. Need to be able to maintain appropriate boundaries in relationships with service users, other relatives and carers so that non-treatment issues do not intrude into the agreed treatment plan. Need to be able to communicate clearly and effectively with service users, carers, relatives, key workers, other members of multidisciplinary teams, external agencies and other health workers Awareness of when information from service users, other relatives and carers, requires communication to key workers, registered nurses and other members of the multidisciplinary team. Use formal supervision with registered nurses to communicate any issues relating to involvement in treatment plans and arising out of relationships with service users, their relatives or carers. Be aware of the need to respect service users and staff in confidentiality. Be able to provide clear written reports regarding any involvement with service users, other relatives or carers. Contribute to induction of new staff and clinical placements of students. 7. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB Previous experience of working in the field of mental health. Ability to work alone in the community setting under indirect supervision of a registered nurse. Ability to develop and maintain relationships with people with severe and enduring mental health problems and associated complex needs. Ability to contribute to, and work as part of, a multidisciplinary team. A comprehensive induction period of six weeks for persons new to post. Compulsory training as required by NHS Tayside, including CPR Training, De-escalation and Breakaway, Mental Health Act Awareness Training, Moving & Handling, Infection Control, Fire Risk Awareness. Be involved in any essential training for CMHT staff. Effective written and verbal communication skills. Ability to carry out assigned tasks on a lone worker basis. Knowledge of community mental health issues, diagnosis and treatments. 10 of 18
11 Awareness and knowledge of risk management issues relating to working in the community with persons suffering from severe and enduring mental health problems. Knowledge of other agencies and organisations and the ability to liaise and network with these agencies on a frequent basis. Knowledge and ability to be able to undertake clinical procedures proficiently (Venepuncture, ECG, Temperature, Pulse, Respiration and Blood Pressure and consolidated experiential learning). Responding to non-predictable crisis situations. Concentration on writing records of patient contact. Emotional demands Communicating with people suffering from serious and enduring mental health problems. Communicating with highly distressed and anxious mentally unwell patients and/or their relatives/carers. Accompanying service users to recreational activities and social venues, including those service users in treatment under the Mental Health Act. Having contact with service users/their relatives and carers at time of bad news and/or following contentious treatment decisions. Dealing with service users with severely challenging behaviour. At times subjected to highly sensitive information about service user s history and problems, e.g. disclosure of sexual abuse. Dealing with group dynamics when accompanying groups of service users to recreational, sporting activities and/or social venues. Supporting colleagues emotionally in relation to the demands of the job. Working Conditions: Exposure to verbal aggression often. Exposure to physically aggressive behaviour owing to the nature of the client group. Exposure to all weather conditions on a frequent basis. Exposure to clients living conditions (e.g. fleas, dirt). Exposure to hazard of passive smoking in clients own homes. Regular exposure to high-risk locations to fulfil treatment plans Regular exposure to high risk bodily fluids (e.g. service user with HIV or Hepatitis requiring blood sample to be taken) 10. DECISIONS AND JUDGEMENTS Make decisions on a daily basis regarding involvement with service users, their relatives and carers, acknowledging the limits of the non-registered role and its responsibilities. Make decisions on a daily basis as to what information from and about service users, their relatives and carers, requires communication to key workers, registered nurses and other members of the multidisciplinary team. As a member of a multidisciplinary community mental health team contribute to the decision-making process with regard to patient care. Report to the appropriate registered nurse any observed non-compliance with NHS Scotland, NHS Tayside and local policies and procedures. Whilst undertaking clinical observations make judgements on reporting any abnormalities/concerns to a registered nurse. 12 of 18

MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Working with patients with severe and enduring mental health problems who have highly complex problems, can present with challenging behaviour and having to deal with crisis situations or situations requiring assertive engagement. Working alone with patients whose clinical presentation can change between visits. Physical demands of undertaking activities with patients. More frequent and longer contact with patients can raise issues in relation to emotional dependence and boundaries that are difficult to negotiate Time management and prioritisation of workload Making decisions as to what information provided by service users, their relatives and carers, requires communication to others in the multi-disciplinary team. 16 General Information POST REF NO: D/LR/248/17 JOB TITLE/GRADE: COMMUNITY SUPPORT WORKER BAND 3 LOCATION: PERTH AND KINROSS COMMUNITY MENTAL HEALTH TEAMS Conditions of Service Remuneration The terms and conditions of service for this post are those determined by the NHS Staff Council. The current salary scale for the post is 17,760 to 20,727 per annum (pro rata for part-time staff). Placing on the scale on appointment is normally at the minimum but may be higher subject to verification of previous relevant service. Salary is paid monthly by Bank Credit Transfer. Hours of Duty The hours of the post are 37.5 hours per week, Monday to Friday 9am- 5pm The contracted weekly hours may be worked over any reference period, e.g. 150 hours (pro rata for part time staff) over four weeks or annualised hours, with due regard for compliance with employment legislation, such as the Working Time Regulations. Start and finish times will be determined by the needs of the service. Superannuation New employees are automatically enrolled into the NHS (Scotland) Superannuation Scheme, but may opt out later should they choose. The pension scheme is provided by Scottish Public Pensions Agency.
