4 What are we worried about? The family is worried that they do not have a place to live, have no financial support since minors were detained and do not have basic needs met. Complicating Factors: Substance Abuse (Meth) Homeless Mental Health Unemployment Parental History of Childhood Abuse What s working well? The family is open to receiving services and eager to work with agency partners to address child welfare worries. Linkages Service Components: Referred for substance abuse assessment Housing partnership participant Referred to Ashbury House for MH WTW eligible Eligible for Transitional Food Stamps
5 FTM HPP Goals Linkages Wrap DDC FAMILY Diff. Response ADM Review FMF Visitation Planning TDM
6 CFTs and CFT meetings are a fundamental part of the Core Practice Model. A CFT will be developed for each child CFTs include: child/youth, parents, caregivers, extended family members, and other supportive people from the family s community. A Shared Family Care Plan will be developed, reviewed, and revised during the CFT meeting. Regular, ongoing CFT meetings will occur throughout the case to address specific issues and work towards achieving specific goals. * Commonly known as CFT

9 Role of CalWORKs SWS in CFT-Linkages Pre Meeting: Review status of FCS case and appropriateness of Linkages meeting with Linkages Coordinator Review CalFresh/Medi- CAL/Welfare-to-Work Complete eligibilityrelated tasks Determine eligibility for CW auxiliary programs/supportive services Present CalWORKs findings During CFT: Discuss options with client and PSW After CFT: Offer supportive services as appropriate with Welfare to- Work participation Administer client supportive services Refer to auxiliary programs as appropriate. Follow up with CalWORKs eligibility/ab 429 tasks Update WTW-related activity/hours Monthly review FCS/ AB 429 case status and/or termination.
