Beat the rush and call the appointment line AARCC for a school physical appointment. You may download the forms from our website. If there is any chance your child will be playing a sport or involved in girl or boy scouts make sure you download all the right forms for your state (Kansas or Missouri) and the forms for scouts. Please fill out the survey on the paper provided (prepaid return envelope included), online at the link provided, or call the number listed to complete the short telephone survey. The Health Center can receive up to $ for each completely satisfied survey returned. Surveys returned with satisfactory marks help keep our clinic up to date with the latest equipment and services available for you. Page 7
