The VA Black Hills Health Care System (VA BHHCS) is located in the beautiful Black Hills of western South Dakota, where we are known for our blue skies, pine forests, rich history, moderate climate and friendly western hospitality. VA BHHCS provides primary and secondary medical and surgical care, along with residential rehabilitation treatment program (RRTP) services, extended nursing home care and tertiary psychiatric inpatient care services, for veterans residing in South Dakota and portions of Nebraska, North Dakota, Wyoming and Montana. Care is delivered through the Fort Meade (near Sturgis) and Hot Springs VA Medical Centers, as well as through a number of community-based outpatient and rural outreach clinics. The Clinical Pharmacy Specialist (CPS) will be a resource on mental health psychopharmacology for VA BHHCS as a whole. The Clinical Pharmacist Specialist, Pharmacy Department, applies clinical knowledge and skills when performing pharmaceutical functions and patient care activities in the Pharmacy and Medical Center. Incumbent serves as a provider with prescribing privileges as outlined in the individual s Scope of Practice for Clinical Pharmacy Specialist (CPS). The primary responsibility of the Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care services through both interaction with patients and providers. The pharmacist monitors and manages drug therapy for patients referred with chronic disease states (anticoagulation, etc.). The incumbent provides pharmaceutical care utilizing an advanced in-depth knowledge of current literature and contemporary practice of medications. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The scope of this practice includes patient teaching, quality assurance, staff education and consultation, and is often in association with or in support of pharmacists, providers, nurses, dietitians, and other health care practitioners.
