4 Job Duties and Responsibilities Describe briefly each major duty that is performed in the normal course of work, including facts as to what the duty is, why and how it is performed, and the extent of the positions responsibilities (including authority, accountability, etc.). Accurately performs skilled nursing assessments including OASIS and develops plan of care according to the nursing process of assessment, plan, implementation, and evaluation of plan of care. 2. Establishes and complies to visit frequency according to patient s needs and physician s orders. 3. Demonstrates the ability to perform a wide range of nursing procedures independently including but not limit to wound care, IV therapy, injections, and catheterizations. 4. Demonstrates the ability to recognize signs and symptoms of complications and report to the appropriate professional in a timely manner. 5. Demonstrates the ability to adapt to different cultures and home environments. 6. Is independent in decision making regarding nursing care and development /changing plan of care. 7. Implements teaching appropriate to patient needs by assessing knowledge level and identifying learning needs. 8. Plans, teaches, supervises, and counsels regarding physical care, measures to promote improvement / recovery, symptom management, expected disease progression, social/emotional care, and community resources. 9. Demonstrates working knowledge of internal and community resources. 10. Utilizes supplies in a responsible and cost effective manner to achieve desired patient outcomes. 11. Collaborates with other healthcare provider to achieve expected outcomes of patient care and directs continuum of care for primary patient caseload through patient care conferences and interactive communication. 12. Maintains accurate patient record per agency policy and submits patient documentation as per agency policy. 13. Supervises patient care performed by home health aides and may assist with competency evaluation of home health aides 14. Discusses problems/concerns regarding patient care issues with supervisor. 4
