We are a Medicare plan specifically designed for people with long-term care needs. So you re in good hands. And your caregivers are too. Finally, our specialized network of UPMC and community doctors and facilities is uniquely qualified to deliver personalized care. The result: more peace of mind for you and your loved ones.
3 If you re in a nursing home: You and your loved ones will be supported by a nurse care manager who will: Provide you with a detailed whole person care plan with the goal of helping you live a higher quality of life. Coordinate your care with staff, identify issues, and speak up on your behalf. Provide health updates to you and your doctor. With your OK, he or she will communicate with your loved ones too. Be your extra pair of eyes to ensure proper care. Coordinate transportation, if needed, for medical services. Do everything possible to make sure your wishes and preferences are respected. If you re at home: Your loved ones will have peace of mind knowing that you have access to a program that lets you stay at home. It provides personalized, in-home service to help you remain independent. The program will: Provide you with a detailed whole person care plan with the goal of helping you live the healthiest life possible. Help you remember to take your medicines. Help you schedule doctor appointments. Coordinate transportation to your doctor appointments.
4 Are you enrolled in the Aging Waiver? Our plan works with your current services to help keep you independent at home. We will support your decision to stay in your home. You will not lose your waiver supports by enrolling in our plan. We will be a resource for how Medicare and Medicaid interact. You will not pay any member premium or cost-sharing. To be eligible for UPMC for Life Options, you must: Be enrolled in Medicare Part A and Part B Be assessed as having long-term care needs Live in your home or a long-term care facility such as a nursing home Live in the service area Not have end-stage renal disease, unless certain criteria are met Are you a caregiver? There is support for you too. If you are the point-of-contact on your loved one s behalf, our care managers will: Keep you informed. Coordinate communication between all of your loved one s health care providers. Our goal? A whole person continuum of care. Prevent minor health issues from becoming bigger ones. Provide effective caregiver resources and tools.
