Accountable Care Systems are an evolved version of an STP working as a locally integrated health system to deliver faster improvements in quality and population health, but how does this evolution take place? What are the technical, structural and cultural challenges that need to be faced to build a service which is based around the individual and all their health and care needs. From the perspective of a Clinical and an Information Officer, Dr John McCormick and Nick Hopkinson will share their experience in working to deliver a digitally enabled ACS in Devon.
3 09:05-09:25 Digital Care Delivering Digital Care into the Economy Colin Skoyles CIO Tameside and Glossop Integrated Care NHS FT & Tameside and Glossop CCG The purpose of this initial scheme is to support the care of residents within both residential and nursing homes. A team of nurses operate out of our Digital Centre based at the hospital and provide clinical advice to carers and residents allowing the person to be treated remotely where this is clinically appropriate. This is enabled through the use of Skype for business, installation of Wi-Fi enabled tablets within care homes, observation equipment such as BP monitors, tympanic thermometers, oxygen saturation monitors and other simple diagnostic tests. It is supported through a standardised training programme to care homes. One of our aims is to reduce attendances to and admissions from the Emergency Department for residents of Care and Nursing Homes in Tameside and Glossop. 09:25-09:45 Solution Provider Case Study 09:45-10:05 Transformation Through An Electronic Patient Record Cindy Fedell Director of Informatics Bradford Teaching Hospitals NHS FT Implementing an EPR is costly, takes a lot of time and is often thought to be very challenging to deliver. It is all of these things. We spend our efforts managing these challenges, but the heart of any implementation is the benefits. Properly preparing to realise benefits is not something we do well regardless of the type, which varies from patient safety to helping us work more efficiently. They can be complex and difficult to convert into a plan. How do we ensure maximum benefit, after we go live, whilst managing these challenges? Nina White Head of Transformation Shropshire CCG 16:30-16:50 STREAM THREE Transformational change through organisational merger Angie Smithson Deputy Chief Executive/ Integration Programme Director Aintree University Hospital NHS FT Three years ago over 200 senior doctors from Aintree University Hospital NHS Foundation Trust (AUHFT) and The Royal Liverpool and Broadgreen University Hospitals NHS Trust (RLBUHT), the two largest hospitals in Liverpool with a combined turnover in excess of 850 million, made the clinical case for the merger of the two organisations. Since then, this clinically led vision to deliver better quality and more cost effective care, has been developed with Clinical Commissioning Groups (CCGs), the University of Liverpool, the two Trust Boards and other partners. Angie Smithson, Integration Director, will talk about the challenges and learning as the two organisations work towards merger.

The work took him to places Paediatricians are rarely found. The work became about patient safety and improving outcomes for patients. Making those improvements into a systematic process led him in to the implementation of IT systems. The future of healthcare will be underpinned by IT systems, investing now is essential! We will go through the process that we went through to agree a budget allocation formula and to develop a fair funding formula for general practice that rewarded those practices that remained within budget. We will also look at how that is working within individual practices and across Primary Care Home teams based around patient populations of 30-50,000 people. We will look at the impact on outpatient referrals elective and non-elective activity. 08:45-09:05 Integration In County Durham - Are We An ACS, ACO, ACN or ACP? Who Knows! Nicola Bailey COO NHS Durham Dales, Easington and Sedgefield Clinical Commissioning Group This talk will look at the development and framework of integrated services in County Durham and more widely across the STP under the accountable??? banner. The presentation will outline how County Durham s health and social care services have moved together to create an Accountable Care Network that has innovative integrated delivery and commissioning at its heart. The commissioner and provider landscape is blurring with primary care centre stage as the only truly universal service accessible by all. Nicola will explain how are they are managing to integrate two ways i.e. at scale with a number other CCGs, and locally to retain a true place based focus with primary and community services FT partners and their local authority partners.

Using a case study example of the successful reconfiguration of a regional clinical service, the key issues, limitations and initial barriers to success will be highlighted. The steps and mechanisms employed to address and overcome those obstacles will be identified and explained. Using case studies based on 20 years working in programme management and capital development, Dr Anna Barnes will outline some of the key concepts regarding the leadership and management of change within the NHS/Social Care. A key facet of her presentation will be the importance of acknowledging the human factors which can hinder the implementation of large scale change programmes. 12 10:45-12:15 Business Meetings 12:15-12:35 Transforming Stockport Together Hugh Mullen Deputy Chief Executive & Executive Director of Support Services Stockport NHS FT In Stockport they are implementing a very innovative transformation programme called Stockport Together which aims to radically change the way they provide traditional hospital services for their Elderly population. Stockport Together combines Stockport FT, which provides the acute service on the Stepping Hill site and Stockport Metropolitan Borough Council which provides Social Services. Viaduct Healthcare is their GP partner organisation and provides Primary Care and Pennine Care FT, which provides mental health services. They have now formed an Alliance Provider Board ahead of moving to an Accountable Care Trust in Hugh s session will set out the journey Stockport have been on to alter the way they provide services and significantly reduce the need for Secondary care by establishing 8 neighbourhood teams. Hugh will set out the performance criteria they need to achieve and the progression they have made to date. 12:35-12:55 User Centric Digital Transformation Nicola Haywood- Alexander CIO and Director IMST and PMO, Sheffield Health and Social Care NHS FT Sheffield Health and Social Care, as part of the South Yorkshire and Bassetlaw ACS have long engaged clinical and service users in the design of their digital technology, from an in-house EPR to self-service applications to support well being and recovery. The design of solutions and cases for investment are all user case driven, and are centered on patient stories. This is an integral part of their forward digital strategy for both place and system. Nicola will share the journey to date from both the Trust and ACS perspective.

CLOSING KEYNOTE The first wave of ACSs are already assuming accountability for local operational and financial performance. From April 2018, they will begin to gain new financial flexibilities and to use new tools for better understanding local health data. This session gives you the chance to ask the panel their views on the future of ACSs and the benefits or risks that will come with it.
