Episode of Care Definition: A home health episode of care begins with a start of care or resumption of care assessment. A home health episode of care ends with a discharge, transfer to inpatient facility,or death at home assessment. Only completed home health episodes of care (those with an OASIS C assessment for both the beginning and end point) will be included in reports.
