This is a unique role and we are seeking an exceptional candidate with the necessary experience, skills, personal ability and calling. THE VINEYARD COMMUNITY CENTRE The Vineyard Community Centre s aim is to firstly express Christian love by serving the local community within Richmond and secondly to see long-lasting change through the transformation of individuals in body, mind and spirit by the power of God s love. There are broadly three groups of people we help. Firstly, rough sleepers and those in crisis who need short-term support and immediate help through specific services as well as social and spiritual encouragement in addressing the causes of their problems. Our aim is to move people away from crisis to a stable and self sustaining life style in all that we do. Secondly are those who feel lonely, isolated and socially excluded with perhaps learning or mental health problems. Some may be vulnerable adults; other guests will be those moving from homelessness or crisis through to personal recovery. All are welcome at the Vineyard. We run regular skills based courses in sewing (Works of Love) as well as occasional courses in our cafe to train guests in barista skills. These courses foster a sense of community, build personal confidence and lead to the award of AQA vocational certificates. Thirdly, we help those who are in crisis and need emergency food supplies. We operate our main Foodbank at the Vineyard Centre and three other satellites provide this service elsewhere across the borough. THE REASON FOR THIS NEW ROLE The Richmond Borough Joint Strategic Needs Assessment states that an estimated 22,000 people in Richmond have a less severe, common mental disorder such as depression and anxiety. There are 6,164 adults identified with depression by local GP's'. We know that many visit our Centre. In the 12 months to March 2015 our Case Worker interviewed 140 new guests 1 to assess their overall needs. Most were in personal crisis. 30% had enduring health needs of which the majority suffered from a range of mental health illnesses. Many experienced anxiety and depression while a smaller number had a range of psychotic illnesses. Additionally, a further 12% of the total had social inclusion needs resulting from isolation, learning difficulties or those on the autistic spectrum. Some were registered with local GP s, or receiving help 1 We call those who use our services guests rather than service users or clients. 2
3 from the psychiatric services provided by Richmond Royal Mental Health Hospital. Many were not registered and receiving no regular medical treatment. Our Case Worker works with local partner agencies (such as to SPEAR, C.A.B, RBMIND or DIAS) by sign posting them so accommodation needs, substance abuse, unemployment, offending behaviour, learning difficulties etc can be addressed. We have created a new part-time post of Community Nurse to provide a dedicated home outreach service that integrates physical, psychological, emotional, spiritual, and social health for our guests in conjunction with services available through local primary health care facilities. In particular we will engage with two local surgeries to which we currently refer those guests who are not registered. We will receive professional support from Parish Nursing Ministries UK. Their training is validated through the Westberg Institute for Faith Community Nursing, based in the Church Health Centre, Memphis and has been adapted for the different context of the UK. The training courses are held in Selly Oak, Birmingham. The support includes specialist training and ongoing professional advice to the nurse. Dr Jenny Eades, MBBS, BSc, DRCOG, MRCGP, one of our Trustees, who is also a local GP, will provide input as well. 3. WHAT WE PRAY WILL BE ACHIEVED Our desire is for our guests to be compassionately cared for and healed. This additional service will address the enduring complex health needs of those whom we visit through the advocacy of our Community Nurse working collaboratively with local primary health care agencies. We estimate that care plans will be in place for of our guests in Year 1. This is what we want to achieve: (1) Collaborative working arrangements with local stakeholder a local surgery and partner agencies like the Community Mental Health Team & Richmond MIND. (2) A care plan in place for each guest patient referred to the new service and a pathway of recovery and healing developed for those in need of personal care and support delivered by a nursing professional who ensures co-ordinated NHS and other provider services. (3) Positive feedback from our guests who have been helped and supported through case working. 4. WHO s WHO Day to day running of the Centre is overseen by the Centre Manager, Desiree Shepherd with a small team of part-time staff which includes Norman Chingwaru, 3
4 our part-time Case Worker. Our Foodbank, which is part of the Trussell Trust, is run by our part-time Manager who is Caroline Ferrier. Our team of volunteers play a crucial role in the Drop in, Café, Foodbank and the Charity Shop. The Vineyard Community Centre has a Board of Trustees responsible for the legal and financial affairs of the charity. The Trustees are Bob Kimmerling, who is Chair, Peter Flower, Roger Smith, Dr Jenny Eades, David Smith and Lydia Flower. They are all members of the Vineyard Life Church; the church is the landlord of the basement premises from which the Community Centre operates. The Community Centre is very much part of the ministry of the church. 5. WHAT WE BELIEVE We are a Christian organisation and we seek to share the love of Christ in all that we do; prayer is foundational to our ministry. The Nicene Creed was originally written in 4th century A.D. It is the most widely accepted creed in Christianity and continues to be used in the Catholic, Orthodox. Anglican and other Protestant Churches. You need to confirm your faith in Christ expressed below. Where appropriate and required visit individuals in their homes subject to Lone Working protocols Integrate faith and health through providing a holistic assessment of needs Advocate for individuals with their health issues and support them through collaboratively planned care provision. Deliver, or facilitate the delivery, of planned health support in community locations Monitor and evaluate the effectiveness of the care provision, adjusting the care plans as required Where appropriate support risk assessment to ensure that health needs are being met Where appropriate attend Network Meetings of the Health and Social Care Professionals to ensure continuity in care Ensure clinical/nursing activities and advice are evidence based and supported by current research Health Education Monitor the health education priorities through assessment of health needs of those who use our services Provide health education individuals aiming increase understanding of health concerns and empower people make changes minimise the impact of their health concerns Promote Health Education as an integrated aspect of care delivery through well men s clinics for service users run by GP s. 6
7 Create packages and / or displays of health education materials relevant for the use of our guests as needed Health Advocacy Provide an advocacy role that supports people in their access of health services Initiate and support referrals other health professionals and support agencies in order support the range of need of a person requesting support Work with Health, Social Care and Voluntary Sector services support the best solution for holistic care Referrals Liaise with appropriate agencies and authorities in support of individual s health needs. Refer Health, Social Care, Faith Organisations and Voluntary Sector service access the most suitable resources for holistic care Professional Management Attend Centre team meetings and regular one- one meetings with line manager Work as an autonomous and lone practitioner, following systems for Lone Working. Maintain accurate, systematic and timely record keeping of health interventions in keeping with NMC Record Keeping guidance, ensuring safe storage of documentation Keep statistical information required by the Centre, any Funders and PNM UK Participate in annual development review with direct line manager Adhere to Centre policies, NMC and Parish Nursing UK requirements Promote Safeguarding of Children, Young People and Vulnerable Adults in all Community Outreach activities Ensure confidentiality and data protection processes are in place, and policies are adhered at all times Maintain the Parish Nurse Quality Standards, annually reviewing these with the Regional Coordinator Take appropriate action in regard any accident or incident occurring a volunteer, guest, staff member or visitor Take opportunities to promote Parish Nursing in the local area. 10 8. MAIN TERMS OF EMPLOYMENT 1. SALARY 18,974 p.a. (AFC Band 6 23pt) 2. HOURS OF WORK [25 hours per week] Monday Friday (mainly mornings but specific hours to be arranged) 3. ANNUAL HOLIDAY Equivalent to 25 days per year, excluding public holidays (pro-rata). 4. SICK PAY Statutory Sick Pay on joining and then 2 week s paid occupational sick leave after 6 month s service. 5. PENSION Our Workplace Pension Scheme is the People s Pension. You are required to join this scheme unless you chose to opt out; the employee contribution is 1% of salary and the employer contribution is 3%. You may choose to pay a higher employee contribution if you wish. 6. PROBATIONARY PERIOD 3 months. 7. NOTICE PERIOD 2 weeks during probationary period increasing to 4 weeks after confirmation in post. 8. TYPE OF CONTRACT This position has been funded by specific grants so is for a fixed term period of 12 months. 11 We will pay for Nursing Council Registration Fees and professional indemnity insurance. Attendance at the Parish Nursing UK 5 Day residential Introductory Course held between May th is a requirement and the course fee and travel costs will be paid for. Attendance at further training and other meetings arranged by Parish Nursing UK is a requirement of the role. We will also pay for Continuing Personal Development. The cost of calls from your mobile phone will be paid for; a parking permit issued by Richmond Borough Council will be supplied. Business mileage for your car will be paid for travel incurred in the course of your work. 10. OTHER BENEFITS Staff discount of 50% in our Charity Shop after 8 weeks service. If you work during the Morning Drop-In coffee and tea is available free. In the afternoon, if you work then, you may have a complimentary food dish and a hot drink provided the kitchen is open.
