This service includes advanced health screening assessment, referral and advocacy for the clients within the Supported residential facility, their family and/or guardians; provision of information, training, consultation, advice and support to other service providers. The Position is a part of the broader CALHN Intermediate Care Service providing clinical intervention, advice and/or education where applicable to support Outpatient or Emergency Department avoidance strategies and early discharge from the acute sector. The SRFHAT clinical nurse is accountable for his/her own practice standards, activities delegated to others and the guidance and development of less experienced staff. 9 Knowledge, Skills and Experience ESSENTIAL MINIMUM REQUIREMENTS Educational/Vocational Qualifications Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate Personal Abilities/Aptitudes/Skills: Effective written and verbal communication (including IT literacy Computer skills, word processing and ), problem solving, conflict resolution and negotiation skills. Ability to work effectively within a multidisciplinary team. Central Adelaide LHN also governs a number of statewide services including SA Dental Service, SA Prison Health Service, BreastScreen SA and DonateLife SA, and has financial administrative responsibility for Statewide Clinical Support Services incorporating SA Pathology, SA Medical Imaging and SA Pharmacy. Health Network/ Division/ Department: SA Prison Health Service (SAPHS), a former CALHN state-wide service and the existing Primary Health Care (PHC) Directorate were integrated as a single directorate in July Our aim is to provide best practice healthcare for those clients serviced by Primary Health Care and SA Prison Health Services Primary Health Care/Intermediate Care 9 of 11

12 Services provided by Primary health Care/Intermediate care have a focus on hospital avoidance, Outpatient and Emergency Department avoidance and supporting early discharge for those whose care needs can be met safety in the community setting. Primary Health Care/Intermediate Care also provides care to vulnerable population groups with complex chronic disease to improve their capacity to manage the condition/s, prevent complications, and improve their health and wellbeing. It provides early intervention services that support people to be good self-managers of their health, so as to maintain good health and wellbeing, and to prevent future complications and possible ED admissions. 10 of 11
