Their daily responsibility is to make sure members receive quality care and service by focusing on: Quality assurance Reviewing medical services provided to our members to assure they are receiving the safest and highest level of care and service. If a below-standard quality of care or service is identified, an improvement or corrective action is put in place. Quality improvement Implementing an action plan aimed at addressing an issue and improving the quality of care or service. Quality assessment Evaluating how successful we are in making sure you experience the quality care and services you deserve. If there is anything we can do to improve the quality of your experience, please let us know. Caring for the eyesight of people with diabetes IU Health Plans wants to help people with diabetes preserve their eyesight. The longer you live with diabetes, the higher the probability you could develop complications like diabetic retinopathy. Early detection should be part of your preventive care plan. In the early stages of this condition, there may be no symptoms. Over time the condition can worsen and lead to blindness. For people with diabetes, screening for diabetic retinopathy is covered once per year at $0 copay. Early symptoms may include: n Floaters or dots and dark strings in your field of vision n Dark or empty areas in your field of vision n Blurry vision n Difficulty focusing n Vision that seems to fluctuate n Altered color vision n Partial or total vision loss Go to iuhealthplans.org/diabeteseyeexam to download your copy of the Diabetic Eye Examination Report form to take to your next eye exam. You can also call IU Health Plans to request a copy of this form. 2

3 Get a low-cost, phone-compatible hearing aid Hearing loss affects nearly 48 million Americans and can have an adverse effect on all aspects of life from putting a strain on relationships to increased risk of developing cognitive issues. Today s advanced hearing aids improve the lives of people with hearing loss. On the retail market, hearing aids cost an average of $2,000 $3,000 per aid which is why 20 percent of people with hearing loss decide to skip hearing aids altogether. IU Health Plans currently offers a benefit that drastically reduces your outof-pocket cost on hearing aids. Many members have already taken advantage of this benefit and have saved thousands through the program. This year, IU Health Plans is updating the program with a hearing aid selection offered through TruHearing Select that includes smartphonecompatible Flyte hearing aids that you can control wirelessly from your Apple or Android phone. Through your smartphone, you can stream phone calls and music, control hearing aid settings, switch programs, and more. Your hearing aid coverage for 2017 includes: n Initial hearing exam with a TruHearing provider n Three follow-up visits with the provider for fitting and adjustments n 45-day trial n Three-year manufacturer warranty for repairs and one-time loss and damage replacement n 48 batteries per hearing aid To schedule a hearing exam with a provider in your area, call TruHearing at Need to contact Cigna Dental? In addition, within 90 days of membership and during the annual enrollment period, members may choose optional supplemental dental benefit coverage for an additional premium that automatically renews each year. Five times per year, a focus group of members and caregivers meets in Indianapolis to identify and discuss topics relevant to how healthcare benefits and services are delivered. Send an indicating your interest in participating in the council to IUHPMedicare@ iuhealth.org, and a representative will get back with you. Those who reply to this special opportunity will be interviewed for open volunteer positions on the council. 4

HoosierRX n Indiana s prescription drug program for low-income people over age 65 n Receive assistance paying for MedicareRx and Part D prescription drug plan premium n To qualify, your annual income must not exceed the following amounts: $18,330, if single $24,600, if married and living together IU Health Plans Pharmacy Medication Therapy Management (MTM) n You may receive a call from our pharmacy team to offer a phone consultation if you: Take eight or more different maintenance medications Have at least three of the following health conditions: bone disease, chronic heart failure, high cholesterol, high blood pressure, diabetes, asthma, COPD Spend at least $3,919 on prescription medications per year n In addition, review your medications with a pharmacist over the phone. They will make sure each medication is appropriate and safe and help you find cost savings, too. Submit your receipts to IU Health Plans for reimbursement. Fractures can be a sign of osteoporosis Osteoporosis (meaning porous bones ) may not be found until a bone fractures. If you have osteoporosis, your bones don t look different, but they lose substance, as well as calcium and other minerals. As a result, your bones have less strength and are more likely to fracture, particularly if you fall. A fractured wrist or hip from a fall may be the first actual symptom of osteoporosis unless your doctor has been measuring your bone density. If you are female and have had a fracture within the past six months, notify your primary care physician. He or she may want to do a bone mineral density test or make changes to your treatment plan. 6

7 CARING FOR A LOVED ONE? Five questions caregivers should ask a doctor With all you do as a caregiver, you may feel overwhelmed, especially at doctor s visits. This list of questions may be helpful the next time you see your loved one s physician: 1. What is the diagnosis or condition? 2. What medicine are you prescribing and what will it do? 3. What side effects or symptoms should I call about? 4. What is the next step if this treatment option doesn t work? 5. Should any foods, medicines or activities be avoided? As a patient or caregiver, you are a vital part of the care team, so be sure to get the information you need.
