2 GOAL The patient with acute kidney injury (AKI) remains at a high risk for increased in-hospital and outpatient morbidity and mortality. Emerging data show that a patient who had AKI remains at risk for CKD and patients with CKD have a higher mortality than the general population. The goal of this intense, full-day conference is to provide updated information on all the stages of chronic kidney disease and the primary care team s role in managing the care of these patients. Join your colleagues and gain the latest updates and practice guidelines in kidney care! Didactic presentations will be combined with panel sessions to best meet individual learning gaps. Electronic handouts will be made available to conference attendees. TARGET AUDIENCE Family practice physicians, hospitalists, intensivists, nurses, and other allied health care professionals involved in the care of patients with chronic kidney disease will benefit from participation in this activity. ACCREDITATION The is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. DESIGNATION The University of Pittsburgh designates this live activity for a maximum of 7.5 AMA PRA Category 1 Credits. Physicians should claim only the credit commensurate with the extent of their participation in the activity. Other health care professionals are awarded 0.7 continuing education units (CEUs), which are equal to 7.5 contact hours. NURSING ACCREDITATION is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center s Commission on Accreditation. 5 OBJECTIVES At the conclusion of this activity, the participant should be able to: Outline the long-term clinical consequences of a patient who experiences AKI. Recognize the risk factors for development of AKI in the community and in the hospital settings. Describe the fundamental pathology of AKI. List tests available and in development to diagnose AKI. Outline what steps the PCP can take to avoid AKI in his/her patient. Outline the clinical burden of CKD as comorbidity and what it means clinically to the patient diagnosed with CKD. Review risk factors for the development of CKD. List measures of kidney function: serum creatinine, egfr, and proteinuria; discuss limitations on the use of egfr as a screening test for CKD in the general population. Discuss the National Kidney Foundation definition of CKD, its stages and clinical action step for each stage. Summarize the PCP s role in care of the patient with stage I CKD. Describe the relationship between serum creatinine, creatinine clearance, and glomerular filtration rate. Discuss why a valid 24-hour urine collection where GFR is calculated out is better than checking 24-hour urine for only creatinine clearance. Discuss why it is important to catch a patient in stage II CKD rather than stage V CKD. Outline how the Erie VA experience can be applied to any patient population. Discuss the limitation of stage III CKD nomenclature and the clinical need to distinguish the patient with stage CKD IIIA versus CKD IIIB. Identify, diagnose, and describe the medical management of complications of stage III CKD. Review the interventions to prevent the progression from stage III CKD. Describe why reversibility of kidney function is unlikely by the time a patient reaches stage IV CKD. Describe the signs, symptoms, and laboratory parameters of advancing CKD and outline indications for initiating renal replacement therapy. Discuss the importance of timely discussion about renal replacement therapy with a patient with stage IV CKD. Describe options of dialysis modalities. Review the fundamentals of various types of hemodialysis and peritoneal dialysis. Outline strategies that can be implemented by both the primary care team and the nephrologist in caring for a patient receiving dialysis. Outline steps to refer a patient for pre-emptive renal transplant in the state of Pennsylvania. Describe the renal transplantation process from patient candidacy, work-up, and evaluation for acceptability. Outline the basics of renal transplant surgery and post-op care. Review the steps for a person showing interest in being an organ donor. Discuss the role of the primary team in the management of a kidney transplant recipient. Outline how the primary team and the nephrology team can coordinate the medical care of a patient with a transplanted kidney. Define cardiorenal syndrome. Outline the clinical manifestations of a patient with cardiorenal syndrome. Describe current management strategies of cardiorenal syndrome and their limitations.
