1 1 of 5 Effective Date The Admit Patient order has been redesigned to meet CMS guidelines. Effective May 8, 2012, three orders will replace the Admit Patient order: Patient Status Initial Inpatient Patient Status Initial Outpatient Patient Status Change The Admit Patient orders will be removed from caresets/powerplans and replaced with the appropriate Patient Status Initial order. Patient Status Initial Inpatient Order The Patient Status Initial Inpatient order is used to place the patient in an inpatient status. When the Patient Status Initial Inpatient order is entered at the adult facilities, it will contain three required order details: At Le Bonheur, there will be an additional required order detail of: Care Team.

2 2 of 5 Patient Status Initial Outpatient Order The Patient Status Initial Outpatient order is used to place the patient in one of three potential outpatient statuses/services: OP Ambulatory Surgery OP Diagnostic Procedure OP Observation Services When the Patient Status Initial Outpatient order is entered at the adult facilities, it will contain four required order details: Outpatient Status/Service At Le Bonheur, there will be an additional required order detail of: Care Team.

3 3 of 5 REFERENCE TEXT Reference Text is available to review when completing the order details for the Patient Status Initial Inpatient and Outpatient orders. The Inpatient only Medicare list of surgeries link will open a listing that will be updated on a regular basis. PATIENT STATUS ORDER NEEDED ALERT If a Patient Status order has not been entered into onechart when the admitting, attending, fellow or resident enter orders, the Patient Status Order Needed alert will display. ACTION NEEDED: Select the appropriate order from the listing provided on the alert and click OK to proceed.

4 4 of 5 Patient Status Change Order The Patient Status Change order is used when changing from one status to another. For example, if the Patient Status Initial Outpatient order was entered and the patient condition or services being rendered warrant a change to inpatient, the Patient Status Change order is used. NOTE: There should be only one Patient Status Initial order per hospital encounter. The Patient Status Change order contains five required order details: Status Reason for Change TRANSFER AND PATIENT STATUS ORDER The Transfer order remains available and is used when the patient is moved from one level of care to another, e.g. the floor to critical care. The Transfer order and the Patient Status order are both entered if the patient is being moved from one level of care to another (floor to ICU) and the patient status is changing (observation to inpatient). EMERGENCY DEPARTMENT AND Icons that represent each of the Patient Status orders display on the Patient Care column after the order is entered. When the event is manually completed by Patient Access Services, the icon changes to a bed icon. This bed icon is the same color as the original event icon and indicates completion of the task. 5 5 of 5 EMERGENCY DEPARTMENT AND TRANSFER ORDER When the Transfer order is entered, the Transfer Patient icon displays on the Patient Care column. When Patient Access Services manually completes the event, the Transfer PT Complete icon (yellow bed) displays. EMERGENCY DEPARTMENT AND SERVICE TRANSFER (UNIVERSITY ONLY) When the Service Transfer icon is placed on the Patient Care column and then manually completed (after the resident enters the Patient Status order), the Serv Trans Complete icon (green bed) displays. ASSISTANCE If assistance is needed when entering any of the Patient Status orders, discuss with the Case Manager for the area.
