Shared decision indicates what specific, concrete action (e.g. take, call, list, write, go pick up, etc.) the client will take before their next medical appointment rather than use of vague language (e.g. continue to take medications as prescribed, follow appropriate treatment regime, return in one month to review progress). 24% 25-49% 5079% 8089% 90100%

4 Ground Fidelity Scale 4 III. Support Item 9. Specialists offer to do the following things with clients prior to completing the Health Report: Introduce the Homepage and point out new information. Review the Personal Medicine and updates if needed. Review the Power Statement and update if needed. Review the psychiatric medications and update if needed. Review My Wellness and update as needed Explain options for completing the report (listen, read, have peer support read questions, etc.) and offer assistance as needed. Specialists do 2 or less of the activities listed with most of at least 3 of the listed activities at least 4 of the at least 5 of the all of the

After clients have completed their Health Reports, Specialists offer to do the following activities prior meeting with their prescriber: Review the How I Am Doing (HIAD) Scale and look for areas of concern. Review Concerns about Medications and look for areas of concern. Review My Wellness and look for areas of concern. Assist client in identifying a goal for the appointment with their prescriber and type in any specific questions or comments into the goal section. Review auto suggestions generated for the client and offer assistance in using and other resources in the Ground Library.
