Cardiothoracic Surgery Clinical Nurse Specialist Sentara Heart Hospital June, 2014 to Nov Primary focus has been performance improvement activities related to Society of Thoracic Surgeons (STS) guidelines for practice. Early extubation with post-operative cardiothoracic patients was a critical project. June/July 2014 extubation rates in less than 6 hours were only 14% for cardiothoracic surgery. Led a multidisciplinary team to complete revisions to existing extubation protocol; provided staff education and inservices, and encouraged nurses to start weaning sedation earlier for hemodynamically stable patients. Extubation rate is currently averaging 60% of all cases are extubated in less than 6 hours with no untoward side effects or re-intubations. Working as team lead and clinical coordinator for Extracorporeal Membrane Oxygenation (ECMO) therapy. Data collection and patient outcomes review process initiated. Developed/updated policies and protocols, as well as initiation of therapy criteria for Cardiothoracic Surgery Service Line. Finalized application to become an Extracorporeal Life Support Organization (ELSO) certified ECMO program. 2
3 Restructured Ventricular Assist Device (VAD) training for post-surgical patients, in conjunction with the structural heart team. Initial competency training program development and maintenance of continuing competency training in progress. Clinical performance improvement initiatives included reduction of post-operative blood use, decreased incidence of post-operative atrial fibrillation, decreasing healthcare acquired infections, decreasing incidence of pressure ulcers, and improving mobility in critical care. Provide bedside clinical support to nursing staff, assist with orientation and critical thinking development activities for both didactic and hands on learning opportunities. CT surgery program recognized in the VCSQI consortium as the top performing program in reduction of atrial fibrillation, and reduction in hospital readmissions for 2015 and Cardiovascular/Critical Care CNS Wake Med Health & Hospitals August, 2008 to March 2014 Coordinated a multidisciplinary team to evaluate outcomes for the Cardio-Thoracic Surgical ICU, specifically the extubation rates per the Society of Thoracic Surgeons recommendations. Program experienced an increase in LOS, and an increase in pulmonary complications requiring return to ICU. By partnering with the respiratory care practitioner and targeting earlier extubation, able to increase our extubations in less than 6 hours to more than 45% (up from 14%). Able to identify contributing factors that lead to longer intubation times, and successfully decreased our prolonged intubations as well. As a result, our returns to ICU decreased to 4.5% (down from 7%) and the number of returns related to pulmonary complications decreased to 33% (down from 60%). Additionally, our ICU LOS decreased significantly and our overall LOS improved by more than 2 days. Clinical Team Leader for the implementation of IV smart pump technology Assisted in selection process, identified reduction in cost of disposables, identified opportunities for improvement in best practice behaviors for the use of IV therapies, coordinated training for over 3,000 nurses within our organization, and coordinated Go-Live activities for a successful product conversion. ($5 Million budget, lead to a reduction in disposables cost of over $500,000 annually.) Developed the Hospital Anticoagulation Resource team which evaluated patient complications related to the use of the anticoagulation therapies. Improved documentation of Anticoagulation therapies to 100%, and helped decrease related adverse drug events. Developed a Step-down development Task Force including the Management and leadership staff for the Acute Care division. Coordinated an Advanced Med-Surg Step down Core class for the organization that was tremendously successful. Developed a Clinical Care Pathway for CABG/Valve patients; incorporated a daily goals and progress note to be completed by nursing staff to ensure appropriate patient progression toward discharge. 3
4 Developed a VAD training program for CTSU staff, which included development of a competency validation tool, documentation flowsheet, and nursing care policy for the care of patients with a ventricular assist device. Actively participate in the Nursing Standards Quality Improvement Committee, Code Blue/Rapid Response Quality Improvement committee, Nursing Leadership committee, Public Reporting Quality Improvement committee, Nursing Policy & Procedure committee, and Critical Care Education committees. 3. Critical Care Instructor/Education Specialist Med-Ed Seminars January, 2009 to Present Course instructor for CCRN & PCCN review courses, Cardiac Surgery Certification review, Cardiac- Vascular Certification review, Excellence in Critical Care, Excellence in Cardiology Care, as well as Advanced Sepsis Management. Nurse Manager/Clinical Nurse Educator Cardio-Thoracic Surgical ICU Duke University Hospital Feb to July, 2008 Actively supported and promoted a professional practice model that encouraged staff participation in the development of clinical standards that was collaborative, collegial, and utilized current trends and data in professional practice. Responsible for the education of the critical care and step-down nursing staff for the CT surgery units. Roles included implementing a strategic plan for orienting new nurses as well as maintaining competencies for existing nurses in these units (160 nurses). Helped develop and implement a two-day training program for all of the Ventricular Assist Devices for both the step down and ICU nursing staff. Abstract submitted and accepted regarding training program and outcomes for the National Teaching Institute conference in Chicago, IL, through the AACN, Member of the Medication Safety Team Leader committee, Magnet Education team champion, Masters of Advanced Practice Program Committee (student representative,) Duke University School of Nursing, and the Thoracic Surgery Collaborative Action Team (TSCAT.) 4

Classes taught included the Cardio-Thoracic Core curriculum, Heart Center Core Curriculum classes, Transplantation Classes, Critical Care Core curriculum classes (Sepsis and Multi-system Organ Failure, in addition to Renal Failure and Heart Failure courses). Developed educational newsletter for the CT surgery staff as well as formed the Cardio-Thoracic Education Steering Team (CTEST) to act as the clearing house for all educational needs and offerings.
