2 Overview MEDEX Spot is an entirely new experience in Primary Health Care providing greater convenience and lower cost for regular wellness checks, treatment of minor ailments and consultations with specialists for walkin patients at MEDEX Spot locations in supermarkets and airports. In recent years we have seen a move towards self-service at gas stations, supermarket checkouts and airport check in counters thanks to the shrinking cost of new technology. We are VideoKall Inc, an innovator in long distance video communications and remote equipment management, and we are about to bring the same self-service convenience to the healthcare market. UNMANNNED MICRO CLINIC Our latest innovation is MEDEX Spot, a new high tech service which enables the public to have access to medical practitioners on demand. VideoKall has developed what is believed to be the world s first Un- 2
3 manned Micro Clinic. We are bringing to the US healthcare market a selfservice primary healthcare system which enables members of the public to walk into Unmanned Micro Clinics fitted it MEDICAL CALL CENTER out with vital signs monitoring devices. These services will enable the condition of the patient s health to be analyzed and transmitted by satellite to a remote Medical Call Center (MCC) while the patient has a two-way video call with a medical practitioner. MEDEX Spot Medical Call Centers (MCC) will be located at five general hospitals or medical facilities located across the USA and staffed by Doctors and Nurse Practitioners equipped with VideoKall Telehealth Terminals which allow the medical practitioners to see the patient over a high resolution videophone link and simultaneously receive their vital signs data including pulse, blood pressure, body temperature, Body Mass Index, glucose, blood oxygen and more. 3
4 Other services which MEDEX Spot plan to provide, along with the MCC's, include consultations on pregnancy, erectile dysfunction, anger management and depression plus second opinion on cancer treatment with oncologists also on one-to-one video calls. Unmanned Micro Clinics (UMC) are located in air sanitized and sound absorbed cabins inside supermarkets and airports providing patients with a clean private personal space where they can have a confidential consultation with a medical professional they can select by gender and/or by language. Initially, the service will provide male and female, English and Spanish speaking medical personnel at MCC s, and other language capabilities will be added as the services rolls out across the country. The MEDEX Spot service is an affordable $25 for an initial 10 minute consultation with a hospital based medical professional with any extra time required charged on a per minute basis to a prepaid SMART card purchased at the supermarket checkout. The MEDEX Spot service is priced close to insurance co-pay charges and is a prepaidonly service which does not require or use any medical insurance. MEDEX Spot does not require patients to show a Social Secu- 4
5 rity number or Medical Insurance card, so it may also be used by overseas visitors and US citizens who do not have insurance or who are insured but are travelling out of state. At the end of a consultation a patient can expect to either receive a prescription delivered electronically to the pharmacy located in the same supermarket as the UMC, or receive a blood test request form electronically onto a SMART card which they can use at participating diagnostics clinics. Patients at airports will be able to receive prescriptions for most major drugs from a state-ofthe-art medical prescription vending machine located close to the UMC cabin. 6 The Service Anyone. That s right, absolutely anyone who needs to consult a medical practitioner for primary health care can walk in to one of our selfservice Unattended Micro Clinic s conveniently located in supermarkets with in-store pharmacies across the country to have a virtual consultation with a hospital-based medical professional. Our Unattended Micro Clinics (UMCs) are connected by private satellite links to five leading hospitals located on the east coast, west coast and the mid-west which operate Medical Call Centers for the MEDEX Spot System. The moment you walk in the door of our UMC you are in a different world. Outside is the hurly burly of a supermarket. Inside the UMC you are in a quite zone where you have complete privacy from the shoppers passing by outside. The air in the UMC cabin is sanitized so that every two minutes all the air you breathe and the surfaces you touch are cleansed of bacteria, the super clean kind of environment usually only found in specially treated areas. Able bodied patients enter the UMC cabin and sit on a seat provided. Disabled patients can also use the cabin which has been equipped to accommodate wheelchairs. 6
7 On one wall of the cabin is a large TV screen with a high resolution camera. This is a two-way videophone that allows the patient to see the medical practitioner at the hospital and more importantly allows the medical practitioner to see the patient. In this healthcare cocoon, the patient is not only protected from the noise and ambient infections of the sometimes hostile outside world, but also in this zone, the patient can experience a secure and caring environment called MEDEX Spot. MEDEX Spot provides a self-service micro clinic cabin where the patient can have a private and confidential consultation with a medical practitioner at a large hospital through a high resolution videophone connection and state of the art vital signs monitoring in the cabin which can relay the patient s height, weight, blood pressure, temperature, blood oxygen and glucose level data via satellite to the Medical Call Center at the hospital. The MEDEX Spot system enables the medical practitioner to not only gather vital signs data during the consultation but also through the videophone camera, to be able to see the patient and observe whether the patient is exhibiting symptoms which can assist with assessment and treatment. A second camera on a flexible goose neck extension allows the medical professional to request the patient to move the camera to any part of the upper or lower body for close-up examination so that skin problems and the healing 7
8 of wounds can be observed. The inspection camera can also be positioned to look inside the patient s mouth and importantly the patient s eyes. A look into the eye can reveal the silent damage wrought by high blood pressure and diabetes, and reflect the risk of a future stroke or heart attack. High blood pressure and diabetes damage blood vessels throughout the body may also be detected. Changes inside the retina can offer an early warning sign of damage to the heart, kidneys, and elsewhere that may influence treatment decisions, In addition to early detection of treatable eye problems such as glaucoma, regular eye exams also provide an opportunity for spotting subtle changes that can foreshadow the usually invisible damage from high blood pressure or diabetes The combination of two video cameras and vital signs monitoring equipment in the UMC enables the medical practitioner at the hospital Medical Call Center to able to conduct an intensive examination of the patient and make a diagnosis of the patient s condition through this remarkable telehealth system. The MEDEX Spot service can provide diagnosis of health conditions including those affecting the respiratory, circulatory and digestive systems, eyes or skin of the patient. At the end of the virtual consultation the medical practitioner will typically either send an elec- 8
9 tronic prescription to the in-store pharmacy close to the UMC or electronically send a request for a diagnostic test (blood, urine, stool etc) to a SMART card connected to the system which the patient can take to a participating lab for processing. If the medical practitioner decides the patient needs to be seen by a specialist they can either send referral data to the SMART card or connect the patient immediately by videophone to a specialist for an initial virtual consultation. If the patient appears to require immediate treatment at a hospital, the medical practitioner can call for an ambulance to collect the patient at the supermarket. MEDEX Spot provides this state-of-the-art telehealth service for $25 per typical visit * --- just a fraction more than a typical insurance company co-pay. So now just about anyone can afford a primary health care visit. We don t ask to see your medical insurance card because we don t charge your insurance company for the primary health care visit. We just charge you $25 per typical visit. How can we provide healthcare treatment at such low cost? Well, we use state-of-the-art technology that enables us to provide a self-service healthcare facility without requiring a nurse to be physically present with the patient. By clustering our healthcare professionals at 5 large hospitals and using space on a satellite which can be rapidly switched between any UMC and any one of hundreds of medical practitioners, we are able to use expensive medical personnel resources far more efficiently than a typical walk-in clinic 9
10 How it Works Let s say you have had a persistent cough for a couple of weeks that despite all the cough mixtures you have taken it simply won t go away. You decide to try our MEDEX Spot service. On our web site you enter your zip code to find the location of your nearest supermarket with a MEDEX Spot Unmanned Micro Clinic cabin. When you arrive at the supermarket you go to any checkout and from the Gift Card display select a MEDEX Spot SMART card and hand it to the cashier with a payment of $30. That is $25 for the typical medical consultation plus a $5 deposit in case the cabin requires cleaning after your visit. 10
11 Walk over to the MEDEX Spot cabin in the store and select either of the two E-check-in terminals located on the outside walls of the cabin to start your consultation. Insert the SMART card in the E-check-in terminal slot provided and use the touch screen to enter your personal health details and the reason for the visit. The E-check-in terminal is a standalone device that is NOT connected to the internet so the details you enter are only stored on the SMART card. When you have entered those details the E-check-in terminal will prompt you to stand on the digital scales and height measuring unit beside the E-check-in terminal where your height and weight will be collected and stored on your SMART card. You will be asked to agree to an NPP (Notice of Privacy Practices) agreement which is required by the participating hospital. When prompted press the FINISHED key. The terminal will advise you if the cabin is now available for you to enter. If the cabin is busy the E-check-in terminal will display your ticket number and invite you to take a vibrating pager (same as those used in some restaurants) so that you can walk around the store while you wait your turn. When the cabin is available your pager will vibrate and you have 3 minutes in which to go to the cabin and enter. Your ticket number will also be displayed on a large video screen above the cabin. When the cabin is available for use then insert the SMART card in the slot by the door. The electric door will open automatically. Enter the 11
12 cabin then sit on the seat provided and insert your SMART card in the slot. The door will close automatically. The video screen on the wall will display a greeting and ask you which language (currently English or Spanish) you wish to conduct the consultation. You will also be asked if you have a preference for the gender (male or female) of the medical practitioner you wish to visit. Then a short instructional video will be screened on the TV screen in front of you. While you are watching the video your patient record on the SMART card will be uploaded automatically to the medical practitioner at the hospital with the Medical Call Center (MCC). When the instructional video is finished you will be greeted on the video screen by the medical practitioner. A small picture-in-picture display on the screen will show you the picture of you which the medical practitioner is viewing. Typically the medical practitioner will ask you to place your arm in the blood pressure cuff to take your blood pressure with pulse and then lean your chin on the rest provided to take your body temperature with the infrared no contact thermometer then ask you to clip the oximeter contact on your finger to measure your blood oxygen count. If you are a diabetic then you may also be asked to provide a blood sample using the glucose monitoring device with a sanitized replaceable tip. If you have cardiac issues, then you may be asked to place probes from the EKG machine on your chest. Data from all these vital monitoring devices is sent instantly over the satellite link to the medical practitioner at 12
13 the hospital for analysis. You may also be asked to point the inspection camera at either eye so that the medical practitioner can examine your eye to be able to gain additional information about your state of health. At the end of the session the medical practitioner will probably prescribe a medicine and an inhaler to treat your persistent cough and these can be purchased from the pharmacy close to the cabin. When you remove the SMART card from the slot the electric door will open automatically and then close once you have exited. An automatic system will detect the cleanliness of the cabin. When you leave the cabin you take the SMART card to the supermarket check out to get a refund of $5 if you have left the cabin clean and 13
14 have not spilt liquids or left food or containers on the floor. Your SMART card may also have received an electronic coupon from the MCC for purchasing medical products related to your complaint. The SMART card contains your latest medical records including your latest prescription so that when you next visit a MEDEX Spot UMC cabin you do not have to reenter your health record again and only enter new data.
