2 Objectives 1. Provide high quality health care to employees and families of the City of Chattanooga. 2. Identify patients at high risk of becoming seriously ill and work with them to mitigate risk. 3. Help patients with chronic conditions achieve the standard of care for their condition. 4. Provide injury treatment, case management, physicals, drug screening and other occupational health services for employees. 5.
