13 HCH Program Updates New Clinic Site Schedule February 2017 New sites for HCH due to Coordinated Entry System and new CARE Centers (previously known as Multi-Service Centers) Schedule updates were also based on monthly assessments of clinic productivity and patient focus groups. HCH Team Members will be conducting outreach for the updated HCH services and clinic schedules. Flyers will be located in all health centers and entry points into the CCHS system. Does anyone have any suggestions on where we should outreach and/or post our flyers?
