3 About NHS Tayside NHS Tayside is responsible for delivering healthcare to more than 400,000 people living in Tayside. 4 Within NHS Tayside and based on the western edge of Dundee, Ninewells Hospital is one of the largest teaching hospitals in Europe. Number of Job Holders Pitlochry Community Hospital To ensure client group receives quality individualised nursing care, improving the health of individuals, based on best practice guidelines. Delivering high quality nursing care to a defined caseload of patients, while also maintaining dignity and optimum independence for each individual. The staff nurse will be expected to carry out relevant care without direct supervision. 3. ORGANISATIONAL POSITION Senior Charge Nurse (SCN) 9 Charge Nurse (CN) Registered Nurse / This post 4. SCOPE AND RANGE Health Care Assistant (HCA) Hospital Base The GP unit, Pitlochry Community Hospital, is an integral part of NHS Tayside Perth and Kinross CHP. It is one of four community hospitals which provide in-patient facilities for patients under the care of their own General practitioner. Pitlochry Community Hospital is a purpose build busy subactue locality unit providing care for patients transferred from Perth Royal infirmary and Ninewells Hospitals for ongoing treatment, rehabilitation, palliative care and general elderly medicine. General practitioners also admit patients directly from home for ongoing treatment. We also provide a minor injury and illness service and outpatient clinics to our local community. New developments in diagnostics and additional treatment clinics will offer additional opportunities for role development. The service provided includes: Admission, treatment and discharge of patients within the General Practitioner Unit, including the support of patients in Out of Hours periods when medical staff are not on site. Support for consultants at peripheral clinics as required, e.g. surgical and Medical clinics, pre-op assessments as required. Provide a minor injury service to treat low level localised injury or minor ailment in conjunction with local GP s

Working with the Community Nursing Team to provide care to a diverse client group some with complex health care needs, to ensure the highest standard of care is given which complies with current clinical guidelines and legislation. Working as a member of the nursing team and works collaboratively with other agencies both voluntary and statutory to optimise patient care and provide a seamless service to patients and their carers being cognisant of local and national health policies May be expected to work in other settings within NHS Tayside as service requires. 5. MAIN DUTIES/RESPONSIBILITIES Clinical Under the direction of the SCN, participate in the implementation of planned programmes of advice and support to all age groups, targeting individuals, families and vulnerable adults. Participate in the ongoing assessment, development implementation and monitoring of individualised programmes of health care ensuring that the highest standard of service and care is delivered in a consistent and safe manner within the home and clinical setting. Conduct a comprehensive nursing assessment of the care needs of patients using appropriate assessment tools. Devise plan of care to meet individual patient need incorporating review dates and wherever possible involving patient and significant others. Deliver planned care ensuring all staff meets identified patient needs. Provide advice and guidance on health promotion, health management and prevention strategies to patients, carers and significant others. Review and evaluate effectiveness of care delivered and where necessary initiate and record change. Apply a level of understanding of the effects of disability and ageing to ensure that care goals are met. Contributes to change for the benefit of patient care and contributes to the development of the extended role of the nurse.

Acquire and practice advanced clinical skills as identified for service provision with approval of SCN and Head of Nursing e.g. ILS, Basics Training, Verification of Expected Death and Scopes (ECG, male catheterisation and venepuncture). Deliver end of life care, e.g. palliative care and symptom control. Administers and understands complex drug regimes and common side effects and follows Nursing and Midwifery Council (NMC) Drug Administration Guidelines. Initiate, establish and maintain good working relationships with patients, carers and members of the Primary Health Care Team to support multidisciplinary working. Establish and maintain effective communications and relationships/partnerships with statutory and voluntary agencies, which promotes collaborative working and effective coordination of services for individuals and groups. In collaboration with the Nursing team facilitate health promotion activities in line with the health Improvement Agenda, ensuring inclusion of all relevant groups, collaborating with Primary health Care teams, other professionals and organisations both statutory and voluntary. Identify and provide, in collaboration with the SCN, ongoing support to patients with complex problems, who may be unable to access other support services. Teaching, advising and providing support to patients and/or carers, throughout treatment, illness and rehabilitation. Organise and/or take part in immunisation providing information for audit as necessary. To contribute to the prevention, management and control of communicable diseases Ensure child Protection and Vulnerable Adults Policies and Procedures are followed, and undertake on-going training, where appropriate, initiating action to identify and minimise risk. Documentation Ensure that up to date written electronic records are maintained in accordance with NMC, NHS Tayside and local standards supervising ward based staff and student record keeping as appropriate. Ensure documentation reflects accurate record of patient information whilst maintaining confidentiality and dignity. Complies with relevant national legislation e.g. Freedom of Information Act, Data Protection Act. This post holder is required to comply with all Statutory Policies and Procedures of the NHS Tayside and NMC Code of Professional Practice, Guidelines and Standards e.g. Health and Safety, promoting attendance at work, data protection, freedom of information act, risk management measures and IT security policies. Professional Ethics Complies with and reports any non-compliance with the NMC Code of Professional conduct and local and national policies and procedures. Ensure attendance at compulsory updates for self and junior staff. Participate in appraisal of designated staff and ensuring development of appropriate Personal Development Plans. Provide effective guidance and supervision for all junior staff. Review and reflect on own practice and performance through effective use of professional and operational supervision and appraisal. Communicate effectively, to the SCN information regarding patients/carers on caseload. Encourage patient/public involvement within local communities and ensure service users are empowered, and participate in the planning, delivery and evaluation of all Community Nursing activities. Training Staff and Students Participate in development of ward training plan for pre-registration students and permanent ward staff. Contribute to pre-registration supervision and teaching by acting as mentor, using continuous assessment and providing progressive learning environment. Alert SCN of any training needs/deficit identified for self and staff. Ensure personal development is maintained by participating in specific study relevant to service needs. Participate in induction and orientation programmes. Service Development and Delivery Contribute to the operational planning and implementation of policy and service development within your team, leading on delegated projects. Professional Development To maintain portfolio in line with requirement for NMC Revalidation. Deliver extended nursing roles as required for service provision, e.g. male catheterisation, phlebotomy and syringe driver, ECG, venepuncture. Clinical Governance, Quality and Standards Gather information as required by Clinical Governance colleagues. Contribute to the Clinical Governance Agenda to ensure that standards of care are in line with best practice, applying research and evidence to decision making in collaboration with clients, groups and committees. As delegated by SCN, act as Link Nurse for a specified clinical remit, e.g. nutrition, continence, speech and language, blood transfusion, palliative care, cleanliness champions, Pressure Ulcer prevention, CPR and Moving and Handling. Encourage and participate in audit and research, service/project evaluation to support and improve patient care. Facilitate the use of best practice and evidence based nursing care, monitoring and reviewing this case.

Complies and ensures staff compliance with all relevant NHS Tayside and local policies and procedures including those relating to Health and Safety, Risk Management, Confidentiality of Information, Infection Control, Safe and Secure Handling of Medicines, Moving and Handling, Lone Worker Policy and Fire orders and all mandatory training, e.g. Fire Lectures, moving and Handling update, CPR (annually) Follows NHS Tayside Policy when handling and learning from complaints in conjunction with manager and relevant departments. Ensure staff compliance with the Safe and secure handling of Medicines Policy and take remedial action with non-compliant staff. Administer controlled drugs assisted by a registered nurse. Management Skills Act as named nurse to a designated group/caseload of patients and staff. Contribute to regular minuted nursing team meetings encouraging discussion of relevant clinical and other issues. Co-ordinate the day to day activities of nursing staff in the absence of SCN. Contribute towards the management of patient s property and money, as per NHS Tayside Policy for hospital inpatients. Delegate responsibilities appropriately within the team and in line with the NMC Code. Be involved in staff appraisal and personal development plan reviewer for junior staff. Research and Development Contribute to research and and/or audit projects relevant to clinical area, assist with disseminating findings at local level broaden research and development skills through participation in local audit and research projects.

12 6. COMMUNICATIONS AND RELATIONSHIPS Contribute to the establishment and maintenance of agreed communication networks within the ward environment. Establish and maintain communications with patients, significant others, multi-disciplinary team, external agencies and other health workers. Ensure effective communication is maintained with SCN and line managers. Provide and record formal, informal support and counsel to other team members. Provide verbal and written reports. Develops effective communication strategies for patients who have barriers to communication, e.g. stroke patients, learning disabilities, deaf, blind, mute, language difficulties (foreign visitors). Convey information of a sensitive and emotive nature when it is contradictory to patient and care expectations and desires. On a shift basis communicate highly sensitive information to patients and carers requiring empathy and reassurance skills. Maintain the effective two way flow of communication within the ward respecting patient and staff confidentiality. Good history taking skills to relay patient s condition to GP s by telephone. The post holder will communicate confidential and sensitive information with various people e.g. GP s patients, other members of the primary health care team and social Work Department, through face to face, telephone, IT or written correspondence. 6. continued... Communication skills in persuasion, motivation, negotiation and empathy to patients, carers and other professionals. These skills are necessary to provide nursing care to palliative and terminally ill patients in their own homes as well as regular communications with distressed and anxious patients or relatives. Support for patients and carers following receipt of bad news and providing bereavement support. The postholder needs to communicate with a range of people on a range of matters in a form that is appropriate to them and the situation and demonstrate skills in improving the effectiveness of communication. Manages barriers to effective communication. Keeps accurate and complete records consistent with legislation, policies and procedures. The postholder should also be able to present complex and/or sensitive information to senior nurse.

SYSTEMS AND EQUIPMENT Contribute to the maintenance of accurate patients records and ensure their team maintain records in accordance with NMC and local guidelines. Examine any equipment required for the job prior to and during use, as per training and ensure that all junior staff do the same. Ensure staff are aware and trained in the correct use of equipment and follow the reporting systems for faults. On a regular basis instruct other members of staff on the safe use of equipment. Maintain the safe custody of controlled and other drugs in the area in accordance with NHS Tayside policy. Contribute to the effective ordering and use of all stock within the ward area. 14 9. PHYSICAL DEMANDS OF THE JOB Physical Demands Exerts frequent physical effort to manoeuvre equipment and adult patients (over 15kg) several times on each shift. No portering staff, therefore movement of equipment, i.e. oxygen cylinders and furniture, beds/chairs, as limited space within ward areas. Moving and handling of equipment, stand on scales weight 4kg Stand and/or walk for major part of working day, climbing stairs carrying equipment. Frequent prolonged physical activity on each shift responding to emergencies, bathing, and personal care. Recognition of deterioration of in-patients and outpatients and take appropriate action. On each shift move people and equipment using mechanical aids on a frequent basis, including patients who are unpredictable, i.e. confused, agitated, aggressive. Calculate, prepare and administer intra-muscular injections. Venepuncture. Use of screening equipment, e.g. blood pressure monitors, carbon monoxide monitors. Cardio Pulmonary Resuscitation and treatment of anaphylaxis in adults. A broad range of technical skills required. Dexterity and accuracy required for manipulation of fine tools such as injections and syringe pumps. Mental and Emotional effort As Nurse in Charge, immediate response to emergency, e.g. cardiac arrest, aggression, fire alarm. Challenge of ward, MIIU department and outpatient clinics leading to potential high stress situations. Frequent periods of concentration when working with patients on a one to one basis up to 30 minutes. Communicating with distressed/anxious/worried patients/relatives and caring for patients in distressing situations e.g. families experiencing news of a poor prognosis. Depending on shift pattern, the post holder may be the most Senior Nurse on duty in the hospital and can only make contact with NHST Duty Manager. 19 General Information POST REF NO: D/CH/262/17 JOB TITLE/GRADE: Registered Nurse, Band 5 LOCATION: GP Unit, Pitlochry Community Hospital Conditions of Service Remuneration The terms and conditions of service for this post are those determined by the NHS Staff Council. The current salary scale for the post is 22,440 to 29,034 per annum (pro rata for part-time staff). Placing on the scale on appointment is normally at the minimum but may be higher subject to verification of previous relevant service. Salary is paid monthly by Bank Credit Transfer. Hours of Duty The hours of the post are 30 per week. The contracted weekly hours may be worked over any reference period, e.g. 150 hours (pro rata for part time staff) over four weeks or annualised hours, with due regard for compliance with employment legislation, such as the Working Time Regulations. Start and finish times will be determined by the needs of the service. Superannuation New employees are automatically enrolled into the NHS (Scotland) Superannuation Scheme, but may opt out later should they choose. The pension scheme is provided by Scottish Public Pensions Agency. All benefits are explained on the SPPA website Employee contribution rates with effect from 1 April 2018 are as follows: Annual Pensionable Pay Contribution (Full-time equivalent) Up to 16, % 16,929 to 22, % 22,440 to 27, % 27,911 to 51, % 51,482 to 73, % 73,499 to 114, % 114,761 and above 14.7% Contributions are subject to tax relief and reduced National Insurance contributions.
