3 1. Introduction 1.1. Background & Statutory Requirements Asbestos comprises highly toxic, naturally occurring silicate minerals found in the surface of the earth which have been extensively used commercially in the construction industry. Asbestos Containing Material or ACM is now a prohibited substance as it can cause a range of lung diseases that are often slow to develop but in many cases are deadly. Openshore is committed to the safety of all workers and members of the public who may be exposed to ACM due to the work we carry out on this worksite. This Asbestos Management Plan (AMP) details how the asbestos management requirements will be implemented and managed on site by Openshore. The aim of the AMP is to ensure that health and safety risks associated with the project are properly managed. The following Legislation, Regulations and Codes of Practice have been used in the preparation of this AMP: Work Health and Safety Act 2011; Work Health and Safety Regulations 2014; Code of Practice for The Safe Removal of Asbestos 2nd Edition [NOHSC:2002(2005)] Code of Practice for the Management and Control of Asbestos in Workplaces [NOHSC: 2018 (2005)] Safe Work Australia Code of Practice - How to Manage and Control Asbestos in the Workplace Safe Work Australia Code of Practice - How to Safely Remove Asbestos Safe Work Australia Code of Practice - How to Manage Work Health and Safety Risks Safe Work Australia Code of Practice - Managing the Work Environment and Facilities 1.2. Objective This AMP is a site or project specific plan developed to ensure that appropriate asbestos management practices are followed during this project. The objectives of this AMP are to: comply with applicable Work Health and Safety Legislation, Regulations and Codes of Practice; protect the health, safety and welfare of people on our worksites; have procedures in place for identifying hazards, assessing the risks and controlling the risks; make sure hazards are dealt with in a systematic way rather than in response to a crisis; minimise incidents, injuries and illnesses caused by work activities during the project; comply with customer Work Health and Safety and Asbestos Management practices as well as Asbestos Removal Plan requirements; and avoid additional business costs associated with work related injury and disease Workplace Consultation Openshore consults with all workers to create a safe and healthy workplace for workers. Through consultation, workers are able to contribute to health and safety issues that may affect them and their welfare. This consultation process allows us to become more aware of hazards and asbestos related issues experienced by workers, and involve them in addressing potential problems before they escalate and affect the business. Surrounding businesses and homes must be considered for consultation if it is likely the work site activities will impact them. 4 identifying hazards and assessing risks arising from asbestos; making decisions about ways to eliminate and minimise those risks; making decisions about the adequacy of facilities for the welfare of workers; carrying out any other activity prescribed by the Regulations; proposing changes that may affect the health and safety of workers; and making decisions about procedures for: consultation; resolving asbestos related issues; monitoring health of workers; monitoring conditions at the workplace; and providing information or training for workers. 2. Asbestos Management Policy Openshore will protect the health, safety and welfare of workers and others when in the workplace. In order to implement this policy, a program of activities and procedures will be set up, carried out and modified and / or updated when and where appropriate. These programs will relate to all aspects of work health and safety including: Workplace Consultation; Consultation with surrounding businesses and homes that may be impacted by asbestos removal at the work site; Providing information, training and supervision for workers exposed to Asbestos Containing Material (ACM); The Risk Management Process and systems; Site specific asbestos removal control plan and Safe Work Method Statements (SWMS); Roles and responsibilities; Using licensed asbestos removalists and competent workers for asbestos removal; Review of work methods and practices when required; Emergency procedures and drills; Providing appropriate WHS equipment, services and facilities, including personal protective equipment (PPE); Workplace inspections, evaluations and asbestos clearance certification; Reporting and recording of incidents and injuries; and Injury management, rehabilitation, suitable duties and return-to-work. Scope This policy applies across the organisation of Openshore and across all workplaces under this organisation s control, including subcontractors and visitors to the workplace. Responsibilities PCBU and Supervisors must: Make sure asbestos removalist consultants are appropriately licenced for work being undertaken; Make sure workers are appropriately trained in asbestos removal work; Develop and implement safe work practices and procedures; Investigate reported hazards and injuries and make appropriate corrective action; Make sure equipment is safe and properly maintained; Identify hazards, assess risks and eliminate or control risks; Provide and implement emergency procedures; Provide suitable first aid kits, facilities and trained first aid personnel; 4 Page All Hardcopy Documents Are Uncontrolled Version 2.0

5 Make sure work areas are kept safe and free from hazards; Provide the necessary information, instruction, training and supervision to all workers; Keep up to date with changes in WHS legislation, codes of practice and standards, update procedures accordingly and provide all workers with updates; Make sure Asbestos Containing Material (ACM) are correctly contained, sealed and labelled for controlled disposal; Make sure workers compensation insurance is up to date and procedures for prompt rehabilitation is provided to workers; Have a return to work program; Make sure contractors comply with the current statutory safety standards; Provide the training and facilities for the safe handling, storage and transport of plant, equipment and hazardous substances; Provide safety equipment and personal protective equipment (PPE) pertaining to asbestos removal to comply with Australian Standards and make sure workers are trained in its use for asbestos work - disposable coveralls, gloves and waste correctly disposed of after completion of work; Workers protective clothing will never be laundered in their homes, disposable coveralls meeting Australian Standards to be used at all times; Consult with workers about location, type and amount of asbestos and WHS matters so workers can contribute to decisions effecting their health, safety and welfare; Ensure an asbestos clearance certification is obtained at the completion for work undertaken; Keep up-to-date records of all injuries and make sure correct procedures are followed and appropriate forms filled out; Review WHS management plan and procedures regularly and make appropriate changes. Workers must: Work in a safe manner to protect their own health and the health and safety of other persons in the workplace; Participate in WHS consultation, procedures, training and wear appropriate personal protective equipment and clothing provided; Cooperate with PCBUs in their efforts to comply with work health and safety requirements by following the safety procedures, using equipment properly, keeping work areas clean and tidy and evacuating when told; Report all hazards, incidents, accidents, near misses, injuries and illness to their supervisor in a timely manner; and Participate in rehabilitation and return to work on suitable duties. Subcontractors must: Be competent in asbestos removal work and work in a safe manner to protect their own health and safety and the health and safety of others in the workplace; As part of their contract, comply with Openshore. work health and safety policies, procedures and programs; and Observe directions on health and safety from PCBU, supervisors or Health and Safety Representatives (HSR) appointed by Openshore Failure to comply or observe with a direction regarding asbestos management may be considered a breach of the terms of employment or contract and sufficient grounds for termination of employment or the contract. 5 Page All Hardcopy Documents Are Uncontrolled Version 2.0

6 3. Asbestos Management 3.1. Asbestos Management Structure and Responsibility The principle responsibilities of Openshore workers with respect to asbestos management are described below. The management structure is set out in the following diagram. A matrix of specific site responsibilities is set out in Table 1 below. Key project WHS contacts are set out in Table 2 below. Project Manager The Project Manager is responsible for promoting and maintaining good Work Health and Safety (WHS) management. The Project Manager is to ensure that the site specific asbestos removal control plan supplied by the Asbestos Removalist is implemented as part of this AMP. The Project Manager is required to make sure the Asbestos Removalist has the appropriate licence for work being undertaken. The Project Manager is to support the licensed Asbestos Removalist, competent workers and Site Supervisor and hold them accountable for their specific responsibilities. The Project Manager is responsible for taking prompt remedial action to eliminate any non-compliance or WHS issues. Site Supervisor The Site Supervisor is responsible for inducting all workers and subcontractors and directing site activities in accordance with this AMP. The Site Supervisor is responsible for taking all practical measures to ensure the site is operating according to this AMP, and without risks to the health and safety of workers and others who may be exposed to ACM due to the work being carried out on this site. The Site Supervisor is responsible for detecting any hazards, non-compliances or risky conditions. If the Site Supervisor does not have the necessary authority to fix a problem, they are responsible for reporting the matter promptly and recommending remedial action to the Project Manager. Asbestos Removalist Licensed asbestos removal work can differ greatly from work site to work site depending on the type, quantity and condition of the asbestos or ACM being removed. Therefore, there are a number of duties in the WHS Regulations to ensure the licensed asbestos removal work is carried out safely and without releasing airborne asbestos and exposing workers and other persons. The Project Manager will make sure the Asbestos Removalist follows the summary of the specific duties listed in the following WHS Regulations (WHS Regulation reference noted in brackets below): ensuring an asbestos removalist supervisor is readily available or present when the work is being carried out (R.459); providing appropriate training and ensuring the asbestos removal worker has undertaken the relevant units of competencies associated with the asbestos removal (R ); communicating with various parties about the asbestos removal and providing them with appropriate information (R.462 and R ); obtaining the workplace s asbestos register (R.463); preparing an asbestos removal control plan (R ); notifying the regulator about the work before it starts (R.466); displaying signs and labels in the asbestos work area (R.469); limiting access to the asbestos work area (R.470); ensuring appropriate decontamination facilities are in place (R.471); ensuring waste containment and disposal procedures are in place (R.472); ensuring clearance inspections are conducted and issuing clearance certificates (R ); and ensuring air monitoring is conducted, where appropriate (R ). 6 Page All Hardcopy Documents Are Uncontrolled Version 2.0

7 These requirements apply to a number of duty holders including the licensed Asbestos Removalist, the person who commissioned the asbestos removal work, and the person with management and control of the workplace or worksite. An Asbestos Removalist must hold a licence before being permitted to remove friable ACM. Where required the licensed Asbestos Removalist shall notify the relevant WHS/OHS authority for each removal. Health and Safety Representative (HSR) The Health and Safety Representative (HSR) is responsible for communicating WHS information to all site workers in the lead up to and during the course of the project. The HSR is also responsible for coordinating and participating in regular WHS consultation arrangements in accordance with this AMP. First Aid Officer The First Aid Officer is responsible for assessing the project first aid requirements, maintaining sufficient first aid supplies at all times and providing first aid to workers in the event of an injury or exposure to asbestos as per the injury procedures documented in this AMP. Workers All workers are required to attend site inductions and follow this AMP and safety procedures including Safe Work Method Statements (SWMS). Workers are responsible for advising the Site Supervisor of any potential work health and safety issues. Subcontractors All subcontractors engaged to perform work for Openshore are required, as part of their contract, to comply with this AMP and to comply with directions from the company s designated officers. Failure to comply will be considered a breach of the contract and sufficient grounds for termination of the contract. Client The client shall provide a copy of the workplace s register of ACM to the asbestos removalist. This register is to be developed in accordance with the Code of Practice for the Management and Control of Asbestos in Workplaces [NOHSC: 2018 (2005)], before any removal work commences. 7 Page All Hardcopy Documents Are Uncontrolled Version 2.0

Typically, an Asbestos Register will: note the date when the ACM was identified or assumed as being present; record the type of ACM as well as the location and condition; list the results of any testing or analysis of assumed ACM; provide details of areas which are inaccessible. Openshore will ensure an Asbestos Register is prepared, maintained, reviewed and kept at the workplace during the course of the work. Where available, the customer will supply their current Asbestos Register to the Project Manager prior to work commencing. The Asbestos Register must be readily available to all workers, their health and safety representatives and any other persons who may be exposed to ACM. When management or control of the workplace is relinquished, a copy of the Asbestos Register is to be issued to the person assuming management or control of the site Induction Openshore will conduct site safety inductions for all persons on the site during the project including but not limited to the following: General WHS induction Work activity training Site induction All workers, inclusive of employees and subcontractors, must successfully complete the WHS General Induction for Construction Work course and hold the relevant card. Briefing for all visitors to construction sites on specific safety rules and arrangements that apply. A record of site inductions will be made on the Site Safety Induction Register. All workers are also encouraged to participate in Tool Box / Pre Start Talks when and if required to consult on health and safety issues before and during the project. A record of Tool Box Talks is maintained Risk Management Prior to commencing work on a site, a Risk Assessment will be undertaken by the Supervisor in accordance with the procedure WHS1 Hazard Management. All workers are involved in ongoing hazard identification, risk assessment and risk control, as well as the preparation and review of Safe Work Method Statements (SWMS) and access to the Asbestos Removal Control Plan. The site specific Risk Assessment are included AMP as attachments. When the work activity changes or new hazards are identified, the Supervisor will conduct a review of the Risk Assessment and SWMS for that work activity. On completion of the asbestos removal work, a clearance inspection will be performed by the Supervisor who can be contacted on Reporting The Project Manager will ensure control of all project asbestos related documentation and reports. Adequate records will be maintained to demonstrate conformance to specified asbestos management requirements. The records to be maintained for this project will include, but not be limited to the following: Notification to the state or territory regulators; completed SWMS applicable to all hazardous work activities; review of the Asbestos Removal Control Plan; review and updating the Asbestos Register where required; hazard identification and controls implemented; records of regular site inspections; non-conformance, corrective and preventive action; training and induction records; incident and injury records; records of WHS meetings and tool box talks; and permits, licenses, and approvals. These documents will be maintained in accordance with Procedure SP15 Control of Records 3.8. Subcontractor Management It is important that subcontractors and the work practices of their workers do not pose a risk to health, safety and the environment, and our workplace and workers do not pose a risk to the subcontractor or their workers. 11 Page All Hardcopy Documents Are Uncontrolled Version 2.0

12 All subcontractors engaged by Openshore are to adhere to the following requirements: Provide a Risk Assessment for the ACM site(s) and Safe Work Method Statements (SWMS) for the tasks for which they are contracted; Make sure their own or their workers actions do not endanger the health and safety of other workers, visitors or members of the public in or near the workplace; and Report incidents, near misses and hazards to the site supervisor and HSR. Any subcontractor non-conformances will be managed in accordance with Procedure SP16 Corrective & Preventative Action. Corrective action will be required by the subcontractor as recommended by Openshore 3.9. Emergency Contacts and Response This AMP sets out Openshore management of emergencies during the project. It includes: the names of key emergency response personnel and contact details (including all-hours telephone numbers); contact details for emergency services (e.g. ambulance, fire brigade, spill clean-up services); the location and contact details of the nearest hospital; the location of on-site information on hazardous materials, including SDS (Safety Data Sheets) and spill containment material; steps to follow to minimise damage and control the emergency; Instructions and contact details for notifying the Site Supervisor, authorities, local council, nearby residents or the community if necessary. Key Emergency Response Personnel Paul Martin will be the first point of contact when an incident or spill occurs. They can be contacted 24 hours a day. Emergency Response Procedures Evacuation Process Upon first discovery of fire, bomb or other threating situation, call 000 and immediately evacuate the area and contact others on site to do the same; Site supervisor to notify Project Manager; Encourage people to remain calm; Shut down any plant or equipment, especially items that may cause fire or other secondary emergency situation; Do not go back for valuables or tools; If escaping through smoked filled area, keep close to floor (where there is most oxygen) and do not move too quickly; Once clear of the structure, proceed to evacuation assembly area; those exposed or think they were exposed to asbestos are to go to decontamination area if safe to do so; Upon receiving all clear to return to the work site from the Project Manager, workers are to wear clean PPE before returning to site; and Any PPE discarded in the evacuation process is to be collected in asbestos bags, labelled, sealed and disposed of as per asbestos removal control plan First Aid Procedures Inhalation: Take precautions to ensure your own safety before attempting rescue (e.g. wear appropriate protective equipment). Move victim away from exposure area and into fresh air. Skin Contact: Wash gently and thoroughly with lukewarm, gently flowing water and nonabrasive soap for 5 minutes. Eye Contact: Immediately flush the contaminated eye(s) with lukewarm, gently flowing water for 5 minutes, while holding the eyelid(s) open. If irritation or pain persists, see a doctor. Ingestion: Have victim rinse mouth with water. Call a Poison Centre or doctor if the victim feels unwell. First Aid Comments: If exposed or concerned, see a doctor for medical advice. All first aid procedures should be periodically reviewed by a doctor familiar with the chemical and its conditions of use in the workplace Reviewing Emergency Plans THIESS emergency plans are reviewed and revised on a regular basis so they remain current and effective. A review is carried out at the following times as a minimum: When there are changes to the workplace such as re-location, refurbishments or new floor plan; When there are changes in the number or composition of workers including an increase in the use of subcontractors; When new work activities have been introduced, and After the emergency plans have been tested Incident and Injury Management Openshore has implemented incident and injury management procedures to provide resources, information, training and supervision for workers to allow them to adhere to the rules, have the knowledge and resources to follow the procedures and understand their roles and responsibilities. 15 current worker s compensation policy with a licensed worker s compensation insurer allowing workers a means to make compensation claims in the event an injury does happen. During the course of the project, the Asbestos Removalist and Site Supervisor will: monitor air quality in asbestos work area and surrounds; monitor work activities and make sure asbestos site safety rules are being applied; make sure a First Aid Officer(s) is available at all times to provide first aid and sufficient first aid stocks are readily available; follow the Openshore Incident Investigation procedure, WHS2 provide workers with the Openshore Incident Report form, EF27, to report incidents and injuries and claim for injuries; record all incidents and injuries in accordance with documented procedures; and take witness reports of any injury or incident. All workers will: carry out work in a safe manner that prevents harm to themselves and others or causes damage to property, the environment or loss of process or product; immediately inform the Site Supervisor when incidents, injuries or illness occur; and follow the documented Incident Management procedures in the event of an incident or injury Asbestos Site Safety Rules The licensed asbestos removalist will not direct or allow a worker to carry out licensed asbestos removal work unless they are satisfied the worker holds certification that is relevant to the class of licensed asbestos removal work they will be carrying out; The licensed asbestos removalist and site supervisor will provide appropriate training to a worker carrying out licensed asbestos removal work at the workplace and make sure the work is carried out in accordance with the asbestos removal control plan; Prior to any licensed asbestos removal work being carried out, the licensed asbestos removalist will inform the person with management or control of the workplace about the work and the date it is to commence; The licensed asbestos removalist will provide the following information to a person who is likely to be engaged to carry out the work: the health risks and health effects associated with exposure to asbestos; and the need for and details of health monitoring of a worker carrying out licensed asbestos removal work. Before commencing the licensed asbestos removal work, the licensed asbestos removalist will obtain a copy of the asbestos register for the workplace from the person with management or control of the workplace; The licensed asbestos removalist will make sure that signs indicate where the asbestos removal work is being carried out and barricades are erected to delineate the asbestos area; When carrying out licensed asbestos removal work the licensed asbestos removalist will make sure decontamination facilities are available for the asbestos removal work area, any plant and equipment used in that area and workers carrying out the asbestos removal work; When carrying out licensed asbestos removal work the licensed asbestos removalist will make sure that asbestos waste is contained and labelled in accordance with the Globally Harmonised System of Classification and Labelling of Chemicals (GHS) before the waste is removed from 15 Page All Hardcopy Documents Are Uncontrolled Version 2.0

16 the asbestos removal area. It will be disposed of as soon as is practicable at a site authorised to accept asbestos waste; Once the licensed asbestos removal work has been completed, a clearance inspection is to be carried out and a clearance certificate is to be issued by the licensed asbestos removalist before the workplace can be re-occupied; Always follow safety instructions provided in the Asbestos Removal Control Plan, SWMS and Safety Data Sheets (SDS) relevant to work being carried out on the site must be followed; Encapsulate and seal asbestos work areas monitoring air flows in and out as per Safe Work Australia Code of Practice How to Manage and Control Asbestos in the Workplace; Make sure current Safety Data Sheets (SDS) for the type of ACM encountered are available, read and understood by the relevant workers; Follow safety and environmental precautions for use, transport and storage of ACM as listed on the SDS; Air monitoring is to be conducted before and during Class A asbestos removal work. However, it won t be required before friable asbestos removal work commences when the glove bag removal technique is used. Air monitoring will be carried out as part of the clearance inspection, for instance at the conclusion of the asbestos removal work; Work at heights will not be undertaken if the task can be performed on the ground. If asbestos removal work must be undertaken at height, then a risk assessment will be conducted and Safe Work Method Statement completed and implemented as per WHS Regulations; All workers are to wear their appropriate PPE when exposed to ACM, make sure it is fitted correctly and well maintained and stored safely when not in use. Notify their officer/supervisor immediately when PPE is NOT provided or is unserviceable or unsuitable; Tools and equipment being used for asbestos removal work are required to generate a minimum amount of airborne fibres during use. High-speed abrasive power or pneumatic tools such as angle grinders, sanders, saws, and high-speed drills MUST NEVER be used. Hand tools only are permitted. At the end of the removal work all tools are to be either: Decontaminated (i.e. fully dismantled and cleaned under controlled conditions); Placed in a sealed container and used only for asbestos removal work; and Disposed of as asbestos waste. Asbestos vacuum cleaners - Asbestos vacuum cleaners will comply with the Class H requirements in Australian Standard AS/NZS Industrial vacuum cleaners or its equivalent. Asbestos vacuum cleaners will not be used on wet materials or surfaces. Attachments with brushes should not be used as they are difficult to decontaminate; Filters for the asbestos vacuum cleaners will conform to the requirements of AS High efficiency particulate air (HEPA) filters Classification, construction and performance or its equivalent; Household vacuum cleaners must never be used where asbestos is or may be present, even if they have a HEPA filter; Use and/or adhere to asbestos warning signs and labels when and where required (refer Section 3.4); A licensed asbestos removal contractor is to clean up and dispose of asbestos material; Engage and occupational hygienist to perform air monitoring and inspections and provide clearance certification if required; and 16 Page All Hardcopy Documents Are Uncontrolled Version 2.0

17 Workers whose behaviour has placed the safety of themselves or others at risk will be subject to disciplinary procedures. o Controlled, updated copies of all SDS will be kept on site by the Project Manager and Site Supervisor and prominently displayed at the worksite. SDS s relevant to the site will be recorded in the Safety Data Sheet Register Asbestos Warning Signs and Labels The following images show example asbestos warning signs and labels which may be present on worksites. 18 Source: Safe Work Australia Code of Practice How to Manage Asbestos in the Workplace Personal Protective Equipment (PPE) Openshore will make every effort to eliminate or control risks and only use PPE when there is no other alternative to protect workers and others from a hazard. Selection and use of PPE will be based on a risk assessment. Openshore management and workers will: Make sure the PPE issued is suitable for the identified ACM, fitted correctly and is appropriate for the tasks performed; Wear and use appropriate PPE when required in accordance with manufacturer s instructions and for its intended purpose; 18 Page All Hardcopy Documents Are Uncontrolled Version 2.0

19 Wear disposable coveralls of a suitable standard to prevent penetration of asbestos fibres so far as is practicable. Disposable coveralls rated type 5, category 3 (pren ISO ) or the equivalent meets this standard; Asbestos removal workers must wear approved respirators which comply with AS/NZS 1716:2012 Respiratory Protection Devices or its equivalent. Respirators must be selected, used and maintained in accordance with AS 1715 Selection, Use and Maintenance of Respiratory Protective Devices. Participate in consultation and training in regard to fit testing, use, maintenance and storage of PPE; Inspect PPE to make sure it is in good working condition, maintain and store their PPE safely; Where PPE is used and contaminated with asbestos, such PPE is to be sealed, decontaminated, labelled and disposed of in accordance with the WHS Regulations. If this is not reasonably practicable, the PPE must be laundered in accordance with the WHS Regulations. PPE that is not clothing and cannot be disposed of must be decontaminated and kept in a sealed container until it is reused for the purposes of asbestos-related work. Safety footwear should be lace less, as laces and eyelets can be contaminated and are difficult to clean. Lace less safety footwear should remain inside the barricaded area or dirty decontamination area once used for the duration of the asbestos removal work and should not be shared for hygiene reasons. When safety footwear is not in use, it should be stored upside down to minimise asbestos contamination inside the footwear. Storage facilities should be provided to allow for storage of the shoes. At the end of the removal work and each time the worker leaves the asbestos removal work area, safety footwear must be: o decontaminated o sealed in double bags for use on the next asbestos removal site (but not for any other type of work) o disposed as asbestos waste. Inform the officer/supervisor if PPE has not been issued or is unsuitable or unwearable; The Personal Protective Equipment (PPE) Register is used to record PPE issued to workers, the date issued and when a replacement is required. 4. Monitor and Review 4.1. Monitoring Work Areas Openshore will monitor the worksite through regular inspections. Safety controls documented in Work Method Statements will be monitored throughout the course of the project and during the inspections to ensure work is being carried out in accordance with this AMP and the relevant Work Method Statements Communication To minimise impacts on the public by the project, residents and adjacent property owners will be notified in writing before any asbestos removal work commences and at appropriate stages during the project. The letter will contain details of the intended work, the duration of the activities, information regarding any 19 Page All Hardcopy Documents Are Uncontrolled Version 2.0

20 access interruptions and details of whom to contact with questions regarding the work. Permission will be sought by the Project Manager if there is any need to access private property. This plan will include consultation at each step of the process. The consultation will include information sharing. The steps of the process include inspection, identification, evaluation, and control outcomes Project Signage will be erected at the site. Openshore will undertake external and on-site communication in case of incidents and emergencies, including communication with subcontractors. External communication will include informing nearby residents of the proposed work, incidents and emergencies and contacting regulatory agencies if required Notification The licensed asbestos removalist will notify the regulator in writing at least five days before the licensed asbestos removal work commences. The following information will be included in the notification: name, registered business name, Australian Business Number, licence number and business contact details of the licensed asbestos removalist; name and business contact details of the supervisor who will oversee the removal work; name of the licensed assessor or competent person engaged to undertake air monitoring and to issue the clearance certificate; client name and contact details; name, including registered business or corporate name, of the person with management or control of the workplace; address of the workplace, including the specific location if it is a large workplace; kind of workplace where the removal work will be performed (for example, whether it is an office building or construction site and the type of work that is carried out there, if any); date of notification; the start date of the removal work and an estimation of how long it will take; whether the asbestos to be removed is friable or non-friable; the type of the asbestos (for example, asbestos-contaminated sheeting, vinyl tiles, lagging, gaskets); if the asbestos is friable, the way the removal area will be enclosed; estimated quantity of asbestos to be removed; number of workers who will perform the removal work and details of their competency to carry out removal work; If it is not possible to provide five days notice, removal work may commence immediately in the following limited circumstances: a sudden expected event that may lead to a situation where there is a risk of exposure, for example a burst pipe that was lagged with asbestos or a forklift crashing into an asbestos cement sheet wall an unexpected breakdown of an essential service that requires immediate rectification, for example gas, water, sewerage or telecommunications services. If this is the case, the licensed asbestos removalist will notify the regulator immediately by telephone and in writing within 24 hours after the notice provided over the telephone Corrective Action A non-conformance occurs when a procedure or safety control is not followed, or does not perform as required by this AMP. Openshore will monitor non-conformances to this AMP and initiate corrective and 20 Page All Hardcopy Documents Are Uncontrolled Version 2.0

21 preventive action where required. All non-conformances will be managed in accordance with the procedure SP16 Corrective & Preventative Action. Openshore will undertake corrective action in case of incidents that have an impact on the health and safety of others or if safety controls are not followed according to Work Method Statements. Procedures for identifying corrective action include: an Asbestos Management Plan (AMP) review; review of Safe Work Method Statements (SWMS); investigation into the causes of incidents, injuries or illnesses including recording the results; evaluating further health and safety risks; review Asbestos Site Safety Checklist; and review Asbestos Removal Control Plan with licensed asbestos removalist. Asbestos Management Plan Review This AMP will be reviewed in conjunction with the Asbestos Removal Control Plan (ARCP) by the Project Manager as required to ensure its continuing suitability and to ensure it is conforming to the AMP s objectives and legal requirements. Reviews will be undertaken as necessary as a result of any of the following: when there is a change in the scope of the project that requires a change in safety controls; when there is a need to improve asbestos management procedures; as a result of changes in WHS Legislation, Regulations or Codes of Practice applicable and relevant to the project; and at customer request. Reasons for making changes to the AMP will be documented. A copy of the original AMP document will be kept for the project records. Only the Project Manager is authorised to change and re-issue the AMP. The Site Supervisor is to be informed of any changes made by the Project Manager. The Site Supervisor is responsible for ensuring all workers are complying with the AMP, and for informing all workers of any changes.
