4 Patient Reminders CMS Definition: Use clinically relevant information to identify patients who should receive reminders for preventive/follow-up care and send these patients the reminders, per patient preference. Measure: More than 10 percent of all unique patients who have had 2 or more office visits with the EP within the 24 months before the beginning of the EHR reporting period were sent a reminder, per patient preference when available. Patient Preference The method of communication that patients prefer to receive their reminders such as (but not limited to) by mail, by phone or by secure messaging. Active Patients Patients with at least two office visits in the last 24 months.

5 Patient Reminders Patients Seen (denominator): This is the number of unique patients who have had 2 or more office visits with the EP within the 24 months before the beginning of the reporting period. 1. Select DM/HM Formula 2. Run list of patients

6 Patient Reminders The client must decide if they will use Triages, Todos, or Documents for patient reminders. Triages/Todos: The user who makes the reminder calls will need to add Patient Contacted to the bookmark toolbar within a Triage/Todo. Documents: A document will be created on each patient's chart who meets the criteria. OR

8 Patient Specific Education CMS Definition: Use clinically relevant information from Certified EHR Technology to identify patient-specific education resources and provide those resources to the patient. Measure: Patient-specific education resources identified by Certified EHR Technology are provided to patients for more than 10 percent of all unique patients with office visits seen by the EP during the EHR reporting period. Patient-Specific Education Resources identified by Certified EHR Technology Resources or a topic area of resources identified through logic built into certified EHR technology which evaluates information about the patient and suggests education resources that would be of value to the patient. Unique Patient If a patient is seen by an EP more than once during the EHR reporting period, then for purposes of measurement that patient is only counted once in the denominator for the measure.

9 Patient Specific Education In order to count positively in the numerator, the patient education documents that are provided to the patient must be suggested by the EHR. That means the documentation was suggested through the infobutton, which is available in the Problem List, Medications, Labs, and Diagnosis History, or setup in the CDS, Care Plan Package, Lab Test, Lab Profile, Immunization Master File, Xrays, or Orders. Rx education from Medi-Span that was generated with the CVS document will also count positively.

10 Patient Specific Education Patients Seen (denominator): This is the total number of unique patients with an office visit seen by this provider. A patient is considered "seen" by the doctor if there is an e-superbill status of Patient Seen or Closed for that doctor on that day. Medical Records->Preferences->My Preferences

12 Patient Specific Education In addition to using the patient education provided through the infobutton, various areas in MEDENT can be setup that will prompt the user to provide patient education. CDS: When setting up Clinical Decision Support rules, Patient Education is an option to setup. Medical Records > Setup > DM/PQRS > Formulas >Formulas > Enter a formula > Clinical Decision Support > Insert Link > Pat Edu Note: If Patient Education documents are setup under the Documents link in the CDS Setup, they will only count in the Patient Education report if they are also attached to the Patient Education chart button. 14 View, Download, Transmit CMS Definition: Provide patients the ability to view online, download and transmit their health information within four business days of the information being available to the EP. Measure 1: More than 50 percent of all unique patients seen by the EP during the EHR reporting period are provided timely (available to the patient within 4 business days after the information is available to the EP) online access to their health information, with the ability to view, download, and transmit to a third party. Measure 2: More than 5 percent of all unique patients seen by the EP during the EHR reporting period (or their authorized representatives) view, download, or transmit to a third party their health information. Exclusion: Any EP who: (1) Neither orders nor creates any of the information listed for inclusion as par

15 Secure Messaging CMS Definition: Use secure electronic messaging to communicate with patients on relevant health information. Measure: A secure message was sent using the electronic messaging function of CEHRT by more than 5 percent of unique patients (or their authorized representatives) seen by the EP during the EHR reporting period. Exclusion: Any EP who has no office visits during the EHR reporting period, or any EP who conducts 50 percent or more of his or her patient encounters in a county that does not have 50 percent or more of its housing units with 3Mbps broadband availability according to the latest information available from the FCC on the first day of the EHR reporting period.

20 Patient Secure Messaging Once the patient has registered and viewed their information clicking the messages tab and creating a message to send to the practice will help you achieve Core Measure 17!
