Sage is launching a new and exciting Health Services program providing direct health care to seniors to complement our existing programs and services to provide a centrally-located, one-stop resource hub for seniors. Reporting to the Director of Health Services, this position is responsible for providing leadership and effective administration of the Health Services program. This position will be responsible for team development, support and training, and bringing the vision, mission and values to life for the Health Services program. The Health Services Manager will provide support to the Director of Health Services by ensuring that day to day financial and administrative activities are undertaken. Additionally, the Manager will support the development and implementation of the Performance Management and Evaluation Strategy and Plan, as well as the communication strategy for the launch of the services. The Health Services Manager will act as the Information and Privacy Office for Sage and will be responsible for the implementation of related policies and procedures. The successful candidate will have a degree in Business Administration and 5 years of related or applicable experience in a non-profit, human services, community, primary care, or seniors care setting. Preference will be given to individuals in leadership roles, and those with experience in financial reporting. Through a holistic, person-centred approach, we offer programs and services that directly benefit seniors as individuals. Additionally, at a societal level we work with various stakeholders to address issues associated with seniors at risk, and elevate the dialogue as it relates to seniors as valued members of our community. Sage believes seniors have unmet needs and untapped potential, and we aspire to prevent vulnerability as it relates to aging. Overview of the position The Health Services Manager, with support and direction from the Director, Health Services, oversees the planning, implementation, and evaluation of health care services at Sage. Sage and the University of Alberta, Faculty of Nursing have partnered to develop and operate an innovative model for person-centred health and social services for seniors to be delivered by an inter-professional team led by nurse practitioners. The Health Services program is expected to provide up to 5,500 seniors access to health care services, while fully integrating Sage s existing social services and life enrichment programming. The model is also unique in its focus on student learning, evaluation, inter-professional practice, and the engagement of seniors as co-producers of health and social services. The Health Services Manager is responsible for providing leadership and effective administration of the Sage Health Services program and is accountable to the Director, Health Services. The Health Services Manager will be responsible for team development, support and training, development of policies and procedures in consultation with the Director, Health Services and other Sage staff as appropriate, and to support adherence to policy and procedures by team members.

4 The Health Services Manager is responsible to bring the vision, mission, values and strategic plan of Sage to life for the Health Services program team and volunteers. The Health Services Manager is responsible for supervision and annual performance appraisals of administrative team members. The Health Services Manager, in partnership with the Director, Health Services, is responsible for the safety of the workplace and ensuring that employees have the necessary training and equipment and policies in place to function safely. The Health Services Manager reports to and will provide support to the Director, Health Services by ensuring the day to day financial affairs and personnel administrative activities are undertaken in a timely and a professional manner. The Manager will support the development and implementation of the Performance Management and Evaluation (PME) Strategy and Plan The Health Services Manager will work with the Community Engagement team to implement the communication strategy and will be responsible for representing the program and Sage externally. This may include public speaking, participating in advisory committees/working groups, and engaging with the media (with approval of the Executive Director). The Health Services Manager will act as the Information and Privacy Officer for Sage and will be responsible for the development and implementation of related policies and procedures. Structure of Position Area of Responsibility Tasks Executive Functions Assists the Director, Health Services with the development of quarterly and yearly reports to the Executive Director and the Board of Directors and Funder; Reports to the Director, Health Services in overseeing expenses on a daily, monthly, quarterly and yearly basis; Ensures safety in the workplace in conjunction with the Director, Health Services and the Executive Director by ensuring policy development, education of employees and supports adherence to policies; Participates in leadership team meetings. Information and Privacy Officer Acts as the Information and Privacy Officer for Sage; Ensures adherence to the Freedom of Information and Protection of Privacy Act, the Health Information Act and other relevant acts and regulations; Maintains familiarity and ensures compliance with all approved Privacy Impact Assessments and their related policies and procedures; Receives notification of all violations or suspected violations of privacy and security; Initiates an Incident Response upon becoming aware of a possible event that could adversely affect the confidentiality, integrity or availability of health information of one or more persons; Reports security breaches or privacy compliance issues to internal and/or external stakeholders, as appropriate; Exercises reasonable care with caution in protecting confidential

5 and sensitive information related to clients and personnel; Maintains confidentiality of team, personnel and client information. Contract Management In conjunction with the Director, Health Services and the Executive Director prepares Request for Proposal (RFP) documents for engaging mobile providers; Oversees the RFP process; Oversees contract development and adherence on behalf of agency with mobile health providers; Coordinates requests for space, equipment and technology by mobile health service providers; Oversees contract development and adherence with health service vendors, contractors, and suppliers. Organizational and Team Development Promotes awareness of Sage s Health Services and other Sage programs with seniors, caregivers, professionals and the broader community; Keep up to date on relevant trends and research. Actively participates in staff, team and committee meetings as appropriate; Maintains current knowledge of policy manuals, reading minutes of meetings and keeping up to date with organizational events, policies and changes; Contributes to the efficient functioning of the organization and the attainment of goals; Arranges priorities as necessary to perform tasks; Uses time conscientiously, arrives on time for work in suitable attire and uses breaks, flex-time and sick leave appropriately. Team Leadership Models Sage s values; Promotes participatory leadership style; Fosters an atmosphere of professionalism, accountability, shared learning, celebration and positive outcomes. Human Resources Management Student and Volunteer Engagement Designs and develops administrative position job descriptions; Initiates recruitment of Health Services team members in consultation with the Director, Health Services and Human Resource Coordinator; Orients, supervises and mentors administrative employees; Assists administrative employees in the development of annual learning plans; Implements and maintains appropriate personnel resources and financial management information systems (including hardware and software) in order to track sick time, vacation time, and other leaves for all Health Services staff; Assumes accountability for implementing agency personnel policies and procedures with Health Services administrative staff. Sage values student learning and a culture of volunteerism as key aspects of our organizational culture. The Health Services Manager will take a lead role in the integration of students to

6 advance strategic and operational requirements within the program. Effective supervision of students and volunteers within the context of Sage s values, policies and procedures. Help to ensure that health services staff have the supports and training required to effectively work with students and volunteers. Coach, support and reinforce the volunteer s work; Provide ongoing individual or group meetings to review new policies and procedures, volunteer needs, customer service and training requirements; Address any performance or behavioral problems; Meet with each volunteer annually for position review; Maintain a log of volunteer hours. Evaluation Support the development and implementation of an evaluation system within the program; Support a system of evaluation that adequately provides for professional development and support of practitioners; Maintain links and collaborates where possible with other external data collection, evaluation and systems change efforts; Develops relationships with other agencies, academic institutions, government and providers to assure access and links with other data systems and evaluation efforts; Supports the development of data collection policies and practices and monitors uniform data collection compliance and quality across the organization. Financial Management In conjunction with Finance Manager and the Director, Health Services, assumes responsibilities for development and monitoring of the health services annual budget and financial reporting; Assists with and reviews all Health Services financial reports; In conjunction with Finance Manager assumes accountability for developing and implementing financial management policies and procedures for Health Services. Office Management Ensures adequate administrative resources for Health Services staff; Core trainer for Health Services staff re phone, security, and computer systems. Professional Development Prepares an annual learning plan that is in line with the strategic plan and individual needs of the employee to perform his/her role competently; Participates in annual performance appraisals; Maintains and develops professional competence through ongoing professional development. Communication Externally represents, along with the Director of Health Services, Sage and the Health Services program. This may include public speaking, advisory committee, etc. Communicates effectively with Health Services team members to create a cohesive team;

7 Communicates effectively with all Sage staff as well as clients, families, peers, colleagues and other health care professionals, service agencies and community partners Related Duties Assist with orderliness and cleanliness of space; Performs other associated duties as required. QUALIFICATIONS EDUCATION / EXPERIENCE: Degree in Business Administration; 5 years related or applicable experience in a non-profit, human services, community, primary care, and/or seniors care setting; Preference given to individuals with experience in leadership roles; Proficient in use of internet, Microsoft software, database programs; Experience with an Electronic Medical Record, an asset; Experience in financial reporting and management of professional staff; Current Alberta Driver s License. JOB REQUIREMENTS: Travel within the community; Adhere to Privacy and Occupational Health and Safety legislation and applicable policies and procedures; Adhere to all Sage policies and procedures; Ability to work flexible work hours. OTHER Travel related to the fulfillment of this position description will be reimbursed. HOURS OF WORK 35 hours per week with a 1 hour unpaid lunch break each day, unless otherwise advised or participating in the Compressed Work option. Hours of work will occur primarily during daytime business hours (approx. 8am 5pm). Evenings and/or weekend work will be required. LENGTH OF POSITION This position has been categorized as a permanent position and therefore qualifies for benefits and pension contributions. Note that this position is being funded through a two year grant and as with most roles in non-profit organizations, permanent positions are dependent on the renewal of funding at the end of the granting period.

8 Working Conditions Normally works a standard work week but may be required to work some evenings and weekends to meet project milestones.
