2 What is an EAP? A proactive problem-solving resource that provides: Assistance with a wide variety of personal concerns An easy way to get help before an issue turns into a bigger problem A positive resource that we all may need to use from time to time 2

3 Claremont EAP The Basics Eligibility: All employees/retirees covered by the health plan and their eligible dependents. Access: All care should be pre-authorized by calling Claremont at: Hours: 8:00 AM to 5:00 PM, Monday through Friday, PST for routine cases Master s level counselors available 24 hours a day, 7 days a week for urgent situations 3

5 Counseling 5 visits per family member per incident Help with problems including: Marital/family/relationship issues Emotional issues like stress, anxiety, depression, grief Work concerns Substance Abuse Panel of licensed clinicians in private practice Day and evening appointments available Referrals to specialist close to work or home 5

6 Claremont/Avante Integration Claremont focuses on short term counseling and covers first 5 visits Avante is FUSD s Managed Mental Healthcare provider and handles ongoing counseling visits, facility-based care, psychiatric medications, substance abuse treatment Claremont works closely with Avante staff to coordinate care Network match allows seamless transition from EAP to Mental Health services Not sure who to call?
