We will not require prior approval or impose any conditions for you to get services at these clinics. For enrollees age 65 years and older this includes Elderly Waiver (EW) services accessed through the tribe. If a doctor or other provider in a tribal or IHS clinic refers you to a provider in our network, we will not require you to see your primary care provider prior to the referral. Hennepin Health will accept all eligible recipients who select or are assigned to Hennepin Health without regard to physical or mental condition, health status, need for or receipt of health care services, claims experience, medical history, genetic information, disability, marital status, age, sex, sexual orientation, national origin, race, color, religion, creed, political beliefs, familial status, membership or activity in a local commission, or public assistance status. Hennepin Health shall not use any policy or practice that has the effect of such discrimination. Enrolling in this health plan does not guarantee you can see a particular provider on this list. If you want to make sure, you should call that provider to ask whether he or she is still part of this health plan. You should also ask if he or she is accepting new patients. 4 I N T R O D U C T I O N Program Description Hennepin Health PMAP/MNCare is a managed care program that provides health care for individuals between the ages of 0 and 64 who are eligible for Medical Assistance, or individuals eligible for MinnesotaCare. Prepaid Medical Assistance Program (PMAP): PMAP is a managed care program that covers health care for the following people who have Medical Assistance: Children under the age of 21 Parents and caretakers of a dependent child Pregnant women Certain low-income adults without a dependent child MinnesotaCare (MNCare): MinnesotaCare is a managed care program that covers health care for people who do not have access to affordable health care coverage. 5 Important Information I N T R O D U C T I O N Clinics The clinic information printed in this listing was correct as of 7/1/2016. You need to choose a primary care clinic when you enroll. Your primary care clinic can provide most of the health care services you need, and will help coordinate your care. Please confirm with Member Services that your clinic is still a provider with our health plan before signing up. You may choose a clinic from the list on pages 2-7 to be your primary care clinic. Write the clinic s name and code in the space provided on the enrollment form, or call Member Services to update your primary care clinic choice at any time. You will still be able to see any provider within the Hennepin Health PMAP/MNCare network for covered services. Cost Sharing You may be required to pay an amount toward some health care services. This is called cost sharing. Some examples of cost sharing are copays or deductibles for certain services. You are responsible to pay your cost sharing amount to your provider. Members will get an Evidence of Coverage with more information about cost sharing. Member Identification (ID) Cards and Member Packet Service Accessibility If you need special access to get services from a provider, you can obtain information by calling Member Services at or (TTY: ). This could include, but is not limited to, access for services such as availability of flexible hours, wheelchair access or parking lot access. Service Authorization Our approval is needed for some services to be covered. You must get the approval before you get certain services or before we pay for them. You do not need a referral or service authorization to see a specialist within the Hennepin Health PMAP/ MNCare network or a contracted Hennepin Health PMAP/MNCare provider. The Hennepin Health PMAP/MNCare provider network has a variety of specialists available. If your health care provider refers you outside of the Hennepin Health PMAP/MNCare network, authorization is required. The out-of-network specialist must be enrolled as a Minnesota Health Care Program (MHCP) provider, accept MHCP rates from Hennepin Health PMAP/ MNCare and be willing to see Hennepin Health PMAP/MNCare members. Members will get an Evidence of Coverage with more information about which services require authorization. Members will get a health plan Member Identification (ID) card. You must show this card whenever you get health care services, including when you get a prescription filled. We will mail you a member packet and ID card. 6 I N T R O D U C T I O N Services Care Coordination Hennepin Health PMAP/MNCare has care coordinators available to work with members who need assistance with accessing medical, behavioral health or social services. You can get more information on care coordination services by asking your primary care clinic about a community health worker, or by calling Member Services at or (TTY: ). Chemical Dependency Chemical Dependency services provide care and treatment for chemical substance abuse. This can include abuse of alcohol, narcotics or other drugs. Hennepin Health PMAP/MNCare provides chemical health services through a network of providers that are listed on pages If you prefer, you can use any other licensed chemical dependency provider in Minnesota who is enrolled as a MHCP provider, will accept MHCP rates and is willing to see Hennepin Health PMAP/MNCare members. The Hennepin Health PMAP/MNCare care team can help you choose a provider based on the type of services you need and location most convenient to you. If you need chemical dependency services, talk to your primary care clinic, or call Member Services at (TTY: ). Chiropractic Care You may see any chiropractor who is enrolled as a MHCP provider, will accept MHCP rates from Hennepin Health PMAP/MNCare and is willing to see Hennepin Health PMAP/MNCare members. You can get information on chiropractic services by calling Member Services at (TTY: ). Dental A list of dental providers is provided in your provider directory. If you have questions about dental benefits or your dental claims, please call Delta Dental of Minnesota (Delta Dental) at or (TTY: 711). Delta Dental can also assist you with finding a dental provider, setting up your appointment, and arranging transportation or interpreter services if needed. Durable Medical Equipment (DME) Durable medical equipment is any medical equipment you can use in the home to improve quality of living. It can also provide therapeutic benefits to people with certain medical conditions or illnesses. Examples are nebulizers, wheelchairs, crutches and many others. You can get information on durable medical equipment by talking with your Care Coordinator, primary care clinic, or by calling Member Services at (TTY: ). Eye Care Hennepin Health PMAP/MNCare provides eye care services, including eye exams and glasses. A list of eye care providers is on pages You can get information on eye care services by calling Member Services at (TTY: ). Home Care Home care services include the following: skilled nursing, rehabilitation therapies (for example speech, physical, respiratory, and occupational), home health aide, medical equipment and supplies, Personal Care Assistant (PCA) and Home Care Nursing (HCN) services. You can get information on how to access home care services by talking with your primary care clinic, or by calling Member Services at (TTY: ). 7 I N T R O D U C T I O N Hospitals Your primary care clinic will use one of the hospitals listed on page 13. You can get more information on hospital services by calling Member Services at (TTY: ). Interpreter Services Spoken language and sign language interpreter services are available to help you get services. Oral interpretation can be given to you in your language. You may be able to get written information in your language. Hennepin Health PMAP/MNCare, HCMC and NorthPoint all offer interpreter services in person and over the telephone. These services can be accessed by talking with your primary care clinic, specialty clinic, or by calling Member Services at (TTY: ). Mental Health Mental health services provide care and treatment for problems that affect your thoughts, mood and behavior. These can include depression, anxiety or other mental health issues that interfere with your daily functioning. A list of Hennepin Health PMAP/MNCare network Mental Health providers is available on pages If you prefer, you can use any other licensed mental health provider in Minnesota who is enrolled as a MHCP provider, will accept MHCP rates and is willing to see Hennepin Health PMAP/MNCare members. The Hennepin Health PMAP/MNCare care team will help you choose a provider based on the type of services you need and location most convenient to you. If you need mental health services, talk to your Care Coordinator, primary care clinic, or call Member Services at (TTY: ). Open Access Services For some services, you may choose to get care from any physician, clinic, hospital, pharmacy or familyplanning agency even if it is not in our network. These are called open access services and they include the following: family planning, diagnosis of infertility, testing and treatment of sexually transmitted diseases, and testing for AIDS or HIVrelated conditions. You can get more information on open access services by calling Member Services at (TTY: ). Pharmacy Members must use our network of pharmacies for drugs covered by our health plan. A list of pharmacies is included in the Provider Directory that your received in your Member Packet, or by going to Remember to have the following available when you call to refill your prescription: Hennepin Health PMAP/MNCare Member Identification (ID) Card Minnesota Health Care Programs card Cards for any other health coverage you have, such as Medicare Part D or private insurance It is important to remember to show both your Member Identification (ID) card and your Minnesota Health Care Programs card when picking up your prescriptions. People with Medicare must enroll in a Medicare Prescription Drug Program (Medicare Part D) to get most of their drugs. 8 I N T R O D U C T I O N Specialty Care Specialty Care describes services you can get from doctors or surgeons whose training focuses primarily in one specific field of medicine. Examples include neurology, cardiology, and oncology, among others. Hennepin Health PMAP/MNCare offers a specialty network. If your health care provider re fers you outside of the Hennepin Health PMAP/MNCare network, a service authorization is required. The out-ofnetwork spe cialist must be enrolled as a Minnesota Health Care Program (MHCP) provider, accept MHCP rates from Hennepin Health PMAP/MNCare and be willing to see Hennepin Health PMAP/MNCare members. You can get information on how to access specialty care by calling Member Services at (TTY: ). Transition of Care When you join Hennepin Health PMAP/MNCare, we will help you continue to get necessary health care services that were authorized before you joined our plan. Some examples are the following: orthodontia, mental health services, chemical dependency services, services for at-risk pregnancy and prescriptions. If you are currently getting care from a provider who is not in our provider network, we will help you transition to a participating provider. If you would like more information please call Member Services at (TTY: ). Hennepin Health PMAP/MNCare will authorize transportation services for eligible members who need assistance getting to and from their covered appointments based on their medical needs. Hennepin Health PMAP/MNCare reserves the right to determine the best and most cost effective way to transport the member. Transportation will be provided at Hennepin Health PMAP/MNCare s discretion via bus, common carrier (taxi) or special transportation services (STS). If you need a ride and there isn t a closer clinic in our network please call our Member Services at (TTY: ). Members will get an Evidence of Coverage with more information about transportation. Urgent/After Hours/Emergency Care Your care coordinator and care team will make sure you get the care you need any time, day or night. When you need medical care during regular business hours, it is best to call your primary care clinic. When you need care after hours, or when you have injuries or illnesses that are not life-threatening, you have options: Call HealthConnection, Hennepin Health PMAP/MNCare s 24/7 nurse line, at ; or Go to any of the urgent care clinics listed on page 14. In an emergency that needs treatment right away, either call 911 or go to the nearest emergency room. Transportation We are not required to provide transportation to your primary care clinic if it is over 30 miles from your home if another primary care clinic is available within 30 miles. Exceptions to this rule can be made in certain circumstances. Other Things That You Should Know Hennepin Health PMAP/MNCare lets you use providers from many cultures, which means you can work with people who understand your background.
