1 Tools and Resources for the New and Experienced Oncology Nurse Navigator & Oncology Care Coordinator The ONS Oncology Nurse Navigator (ONN) Toolkit was created by the leadership team of the Nurse Navigation and Care Coordination Community. For ease of use, the toolkit is divided into sections relevant to specific areas of navigation. A description of the resource and corresponding link are also provided. It is our hope that you will find the information you need to effectively build sustainable navigation programs or enhance existing ones. Navigation & Survivorship Toolkits and Resources Association of Community Cancer Centers -- Patient Navigation: Resources and Tools for the Multidisciplinary Team Tools for implementing a navigation program at a community cancer center including assessment tools, navigator job descriptions, and standard operating procedures. 3 GW Cancer Institute National Cancer Survivorship Resource Center Toolkit: Implementing Clinical Practice Guidelines for Cancer Survivorship Care This toolkit provides resources to help with implementing American Cancer Society cancer survivorship care guidelines for colorectal, head and neck and prostate cancers and the American Cancer Society/American Society of Clinical Oncology cancer survivorship care guideline for breast cancer. Information is included on training opportunities for primary care providers and patient materials to help activate cancer survivors to be full participants in their survivorship care. GW Cancer Center Cancer Control Tap Supporting Cancer Survivors through Comprehensive Cancer Control Programs A Resource Guide for CoC- Accredited Facilities: Meeting the Commission on Cancer Patient- Centered Standards A collection of services, programs and tools available across the country to help CoC accredited organizations meet Standards 3.1, 3.2 and 3.3. The guide is comprised of local, regional and national resources from Alliance member organizations. Networking opportunities and national speakers provide an exceptional conference for new and seasoned navigators. Non-membership organization. Oncology Nursing Society (ONS) Professional membership association of more than 39,000 members committed to promoting excellence in oncology nursing and the transformation of cancer care. Award-winning publications include: Oncology Nursing Forum, Clinical Journal of Oncology Nursing. Specific resources for oncology nurse navigators include: NN/Care Coordinator Community Platform enabling real time networking and problem solving with oncology nurses, care coordinators and navigators across the country, national conferences and ONS recommended tools and standards of care to ensure the delivery of quality cancer care across the continuum. 4

AHRQ Universal Precautions Health Literacy Toolkit Initially intended for primary care offices, this guide outline the steps and tools needed to effectively improve the delivery of patient information. Motivational Interviewing (MI) Article describes the steps for motivational interviewing and provides examples on the use of MI in clinical practice. 5

6 Nurse Navigation Training/Certificate/ Certification Programs AAACN Care Coordination and Transition Management (CCTM) certificate A 13 module online course offered by the AAACN Online Library for RNs in all settings from ambulatory care to hospitals. The course covers 9 dimensions of the RN s care coordination and transition management role and related competencies. The first module is offered at no cost and includes 2.4 contact hours. AONN + Nurse Navigation Certification Offers general nurse navigation certification exam. The exam is designed for the generalist nurse navigator. The George Washington Institute s Oncology Patient Navigator Training: The Fundamentals The competency-based training uses interactive web-based presentations to discuss evidence-based information and case studies to prepare patient navigators to effectively address barriers to care for cancer patients and survivors. GW Cancer Institute GW Cancer Institute develops first free online comprehensive training for patient navigators. Harold P. Freeman Patient Navigation Institute Training on patient navigation (Certificate of Completion) Offers a Certificate in Patient Navigation. The program teaches skills essential to the safe and effective movement of a patient across the care continuum, improved self-management, and enhanced patient-practitioner communication. Three courses with a prerequisite of a bachelor s degree or equivalent. An RN license is not required. Institute for Integrative Oncology Navigation A training program for new and experienced navigators across disciplines. This will focus on navigation in the practice of integrative cancer care. No specific prerequisites required healthcare professionals can attend but it is open to patient advocates and lay navigators. 6

7 National Consortium of Breast Centers (NCBC) Breast Patient Navigation Certification Program (6 certifications offered, individual designation based on licensure) Involves sitting for an exam: Must be a licensed medical professional, navigate breast patients at least 50% of the time, and a minimum of 2 years of experience is recommended. Oncology Nursing Certification Corporation (Certification) Offers initial certification in specific oncology specialties for eligible applicants: AOCNP, AOCNS, OCN, CPHON, CBCN and BMTCN and renewal of certification for eligible applicants: OCN, AOCNP, AOCN, CPON and CPHON. Certificate programs offered: Chemotherapy Biotherapy and Radiation Oncology Nursing. Nurses in ONN roles should possess certification through one of the National Commission for Certifying Agencies accredited certifications offered by the Oncology Nursing Certification Corporation minimally, Oncology Certified Nurse (OCN ). Patient Navigator Training Collaborative (Certificate of Completion) Offers a full curriculum of courses to prepare new navigators or improve patient navigation knowledge and skills. It includes workshops, online courses, online tutorials, and special topics webinars. Designed for new and experienced patient navigators, nurses, social workers, and community health care workers. For patient navigators, social workers, nurses and community health workers, price varies 7

(2013) AAACN Care Coordination and Transition Management Core Competencies. By Sheila Hass, Beth Ann Swan, & Traci Haynes Developing Ambulatory Care Registered Nurse Competencies for Care Coordination and Transition Management. Nine core competencies for care coordination and transition management in ambulatory settings are discussed. (2013) George Washington University Cancer Institute Core Competencies for Non- Clinically Licensed Patient Navigator Intended for use by health care professionals and institutions considering qualifications for patient navigators. Patient Navigation: A Resource Guide for Navigators and Care Coordinators By J. Smith & M. Teague, 2016 This practical and exhaustive manual covers a wide range of topics within health systems navigation and care coordination. It provides not only an overview of its history and the skills necessary for success but also touches on other important issues like health promotion strategies, emotional considerations, and a self-help reference for the navigator. Rooted in the field of oncology navigation, the model designed and implemented by the authors has application in all areas of health care both acute and chronic. 10
